Abstract

Rational empirical antimicrobial therapy is an important component of sepsis patient management. This study aimed to assess the rationality of empirical antimicrobial therapy in patients diagnosed with sepsis admitted in intermediate care ward of internal medicine department (RPI) of Dr. Soetomo General Hospital from January 2016 to July 2017. Medical records of 91 patients diagnosed with sepsis were collected and studied retrospectively in period from July 2017 to November 2017. 91 (85.05%) medical records from 107 sepsis patients were evaluated. Cultures and antimicrobial sensitivity tests were carried out in 21 (23.07%) patients. 14 patients yielded positive culture results, 9 of which were MDRO positive with ESBL as resistant marker. Empirical antibiotic therapies for these patients were reviewed according to Gyssens method.73 (80.2%) of 91 patients were deemed receiving appropriate empirical antibiotic therapies. Ceftriaxone IV injection as monotherapy or combination therapy were the most common empirical antibiotic therapies (82 in 91 patients, 90.1%), despite local microbiologic flora and antibiogram show most pathogens were resistant to ceftriaxone. Mortality rate in this study was high, 92.3% (84 patients died) despite rational empirical antibiotic therapies were high. This study concluded that empirical antibiotic therapies in sepsis patients according to guidelines adopted in Soetomo General Hospital, albeit deemed rational, was no longer appropriate according to local antibiogram issued by microbiological department of Soetomo General Hospital. Keywords: Empirical Antibiotics Therapy, Gyssens criteria, Intermediate Care Ward, Sepsis, Septic Shock

Abstract

In this paper, we will discuss the ethical problems generated by Islam as was directly exemplified by the pioneer, enlightener and role model of all followers, namely the Prophet Muhammad SAW. The source of ethics in Islam (Islamic ethics) is the al-Qur'an and the Sunnah, both of which are always a measure of the merits of the actions committed by the Muslims. These two sources are a guide for human life to achieve happiness in this world and in the hereafter.    The Prophet's behavior is the most factual interpretation of the Koran. In fact, in many narrations it is emphasized that the Prophet did not act / behave but because of the guidance of revelation, and the Prophet's morals were al-Qur'an. That is why in Islamic scholarship, including Islamic ethics, it does not give up its premises from the Qur'an and the Sunnah of the Prophet SAW. In Islam, the Quran and Sunnah will teach adherents from all sides, be it social, religious, cultural and so on. So in this paper we will discuss the sources of ethics in Islam (Islamic Ethics).            The method of this research is library research (library research), namely research that uses literature in the form of books, notes and reports on the results of previous research which will later become an analysis tool regarding ethics in Islam.     The conclusion of this paper is that the source of ethics in Islam is al-Quran and as-Assunah, which  will be the life guide for Muslims and non-Muslims to achieve true happiness, which the Prophet had exemplified directly from the above source.

Abstract

The measure of the success of service delivery is determined by the level of satisfaction of service recipients. Satisfaction of service recipients is achieved if service recipients receive services in accordance with what is needed and expected. This research aims to measure excellent service quality based on the perspective of community participation through a community satisfaction index survey stipulated in the Decree of the Minister of Administrative Reform No. 14 of 2017 concerning Guidelines for Preparing IKM for Government Agency Service Units. The survey used a purposive survey method approach, taking 165 respondents who received sub-district office services in Banyuwangi Regency. Inferential analysis of community satisfaction survey data is used to measure the community satisfaction index and service quality from the perspective of the community receiving services. The results of this research show a value of 93.16 which explains that the public can feel very satisfied with the services of the sub-district office, while the IPA measurement obtained a value of 3.73, which means that employees at the sub-district office are able to provide excellent service and provide excellent service in the community service process. The results of this survey can be concluded that measuring the community satisfaction index is relevant and more effective for assessing the quality of services provided by local government officials.
