Abstract

Indonesia is one of the countries with the largest number of tuberculosis (TB) cases in the world. Environmental factors play significant roles in infection and disease development in children living with adult active TB cases. The aim of this study was to explore the environmental factors affecting TB risk in children, including humidity and number of people living in the same house with the children.  Using a cross-sectional study design, children living with active TB adults for at least 8 weeks were recruited. The subjects underwent clinical examination, tuberculin skin test (TST) and radiological investigations. Home visits were conducted three times daily, namely in the morning, at midday, and in the evening, to measure humidity by digital hygrometer and to observe home conditions. Of 56 index cases living with active TB adults in an urban low socio-economic setting, 64 contact TB children were recruited. These children were classified as class I with negative TST and no clinical signs, class II with positive TST and no clinical signs, and class III with both positive TST and clinical signs. There were 32 (50%) class I, 10 (15.6%) class II and 22 (34.3%) class III children. However, a sub-sample of 43 showed positive results for Mtb 16S rRNA, indicating that all children were infected. The humidity cut-off point was set at 75%, p=0.04 and EP 2.09 (CI= 1.32-3.29), signifying that children living in houses with a humidity higher than 75%, were twice more likely to be tuberculin positive. Environmental conditions playing a role in disease development were humidity and number of people living in the house.

Abstract

Rational empirical antimicrobial therapy is an important component of sepsis patient management. This study aimed to assess the rationality of empirical antimicrobial therapy in patients diagnosed with sepsis admitted in intermediate care ward of internal medicine department (RPI) of Dr. Soetomo General Hospital from January 2016 to July 2017. Medical records of 91 patients diagnosed with sepsis were collected and studied retrospectively in period from July 2017 to November 2017. 91 (85.05%) medical records from 107 sepsis patients were evaluated. Cultures and antimicrobial sensitivity tests were carried out in 21 (23.07%) patients. 14 patients yielded positive culture results, 9 of which were MDRO positive with ESBL as resistant marker. Empirical antibiotic therapies for these patients were reviewed according to Gyssens method.73 (80.2%) of 91 patients were deemed receiving appropriate empirical antibiotic therapies. Ceftriaxone IV injection as monotherapy or combination therapy were the most common empirical antibiotic therapies (82 in 91 patients, 90.1%), despite local microbiologic flora and antibiogram show most pathogens were resistant to ceftriaxone. Mortality rate in this study was high, 92.3% (84 patients died) despite rational empirical antibiotic therapies were high. This study concluded that empirical antibiotic therapies in sepsis patients according to guidelines adopted in Soetomo General Hospital, albeit deemed rational, was no longer appropriate according to local antibiogram issued by microbiological department of Soetomo General Hospital. Keywords: Empirical Antibiotics Therapy, Gyssens criteria, Intermediate Care Ward, Sepsis, Septic Shock

Abstract

Objective: to identify the effect of the use of family planning counseling media on improving vasectomy knowledges and counseling skills.Methods: This article used  quasi-experimental method with a non randomized control group pre test - post test design. The research subjects were 35 male heads of the male family planning group divided into mannequin, video, leaflet, poster, and control group.  Results: Average variable of knowledges of the male family planning group leader before debriefing was 43.66 and after it became 66.86 with average knowledges in the control namely 43.43, leaflet 68.71, poster 68.57, video 74.29, and mannequin 78.29 (p = 0.0001) or smaller than 0.05. While the average variable of counseling skills before debriefing was 26.74 and after it became 84.00 with the average of them were control group 27.43, leaflet 65.14, poster 70.85, video 72.57, and mannequin 71.57, (p = 0,0001) or smaller than 0.05.  Conclusion: The use of family planning counseling medias increase vasectomy knowledges and counseling skills. For knowledges, mannequin is the best media. While the counseling skills, video is the best one.Keywords:  counseling skills, knowledges, media

Abstract

Introduction : Cataract is a cloudiness in the fibers or lens material inside the lens capsule. If the cataract was not treated immediately, the patient might experience blindness. Aim: This study aimed to analyze the correlation between the level of knowledge about cataract, education, and socioeconomic status with preoperative visual acuity in patients with senile cataract in hospital PHC Surabaya. Methods : This study was observational analytic with sampling through accidental sampling. The sample of the study was senile cataract patients preoperatively on August 24th to 27th September 2016. The variables of the study was the level of knowledge about cataract, education level, socioeconomic status, and preoperative visual acuity. Data analysis used Spearman correlation test. Results : Results obtained preoperatively senile cataract patients was highest in the age group 60-64 years (28%) and the lowest in the age group 75-79 years (6%). By gender, patients of senile cataract was highest in the group of male (52%) than female (48%). There was a significant correlation between the level of knowledge about cataract (p = 0.008) and level of education (p=0,006) with preoperative visual acuity in patients with senile cataract. There was no correlation between socioeconomic status with preoperative visual acuity in patients with senile cataract (p = 0.124). Conclusion : The results showed that the factors affecting patients in seeking treatment of cataract was the level of knowledge about cataract and level of education. Patients with a good level of knowledge and good education were expected to recognize the symptoms of cataracts immediately and could search for cataract treatment to prevent blindness.

Abstract

Abstrak: Kegiatan ini dilaksanakan melalui kerjasama dengan Kelompok Keluarga Berencana Pria di Propinsi Jawa Timur. Adapun tujuannnya adalah untuk menyelesaikan masalah yang dihadapi oleh Kelompok Keluarga Berencana Pria di Propinsi Jawa Timur, yaitu terbatasnya pengalaman dan bekal untuk memberikan konseling kepada calon peserta vasektomi yang ada di wilayahnya. Permasalahan tersebut menjadi salah satu penyebab rendahnya kesertaan vasektomi di Propinsi Jawa Timur. Hasil pelaksanaan kegiatan menunjukkan bahwa, pemberian pelatihan konseling dan peralatan pendukung konseling vasektomi dapat meningkatkan kemampuan konseling para ketua kelompok Keluarga Berencana Pria, sehingga hal tersebut meningkatkan jumlah peserta vasektomi. Alat pendukung konseling yang paling banyak menarik peserta vasektomi adalah manekin/boneka tiruan dari organ kelamin pria.Abstract: This activity was carried out in collaboration with the Men's Family Planning Group in East Java Province. The purpose was to solve the problems faced by the Male Family Planning Group in East Java Province. They were limited experience and provisions to provide counseling to prospective vasectomy participants in their area. They were some of the causes of low vasectomy in East Java Province. The results of the implementation of the activity show that the provision of counseling training and supporting equipment for vasectomy counseling improve the counseling capacity of the leaders of the Men's Family Planning group, so that they increase the number of vasectomy participants. The counseling supporting tools that attracts most vasectomy participants is mannequins / artificial dolls from the male genital organs.

Abstract

Model intervensi Diabetes Self Management Education (DSME) memiliki variasi dalam durasi, intensitas pemberian, bentuk edukasi, faktor demografi, dan karakteristik klinis pasien, sehingga menyebabkan inkonsistensi hasil klinis dan luaran lain. Penelitian ini bertujuan menganalisis pelaksanaan DSME berbasis Health Promotion Model terhadap perilaku kepatuhan klien DM tipe 2, menggunakan desain cross sectional. Subyek penelitian adalah 65 klien DM tipe 2 di Puskesmas Mojo yang dipilih dengan metode quota sampling dan 10 orang untuk FGD. Variabel penelitian adalah faktor personal, behavior spesific cognition and affect, komitmen, DSME, dan perilaku kepatuhan. Data dikumpulkan dengan menggunakan kuesioner dan dianalisis dengan PLS dengan hasil yaitu faktor personal dibentuk oleh pendidikan dan motivasi. Behavior spesific cognition and affect dibentuk oleh perceived benefit, perceived barrier, perceived self efficacy, activity related affect, dukungan keluarga, dan pengaruh situasional. Faktor personal berhubungan dengan behavior spesific cognition and affect, behavior spesific cognition and affect berhubungan dengan komitmen, komitmen berhubungan dengan DSME dan DSME berhubungan dengan perilaku kepatuhan. Diabetes Self Management Education perlu peningkatan motivasi, perceived benefit, perceived barrier, perceived self efficacy, activity related affect, dukungan keluarga, pengaruh situasional, dan komitmen klien untuk menjalankan program edukasi yang diberikan petugas kesehatan sehingga dapat meningkatkan perilaku kepatuhan klien DM tipe 2.

Abstract

Stunting is the height according to age below -2 the median standard of the child's growth curve due to chronic malnutrition problems since the first 1,000 days of a child's life. One of the risks of stunting in children is the lack of nutritional intake during pregnancy. This study aims to identify factors related to the preventive behavior of stunting children during pregnancy. This study used a cross-sectional design. Samples in the study were 74 first-trimester pregnant women in the work area of Grujugan, Tenggarang, and Maesan health centers in Bondowoso Regency obtained using simple random sampling. The results of the analysis had a family support relationship (p-value 0.057), environmental support (p-value 0.010) but there is no relationship between stunting prevention behavior with age (p-value 0.361), education (p-value 0.230) and income (p-value 0.240). Recommendations from this study for health workers provide health education to pregnant women and families to provide an understanding of the prevention of stunting from early pregnancy. Keywords: Behavior; Prevention; stunting; pregnant women ABSTRAK Stunting merupakan tinggi badan menurut usia di bawah -2 standar median kurva pertumbuhan anak disebabkan masalah kurang gizi kronis sejak 1.000 Hari Pertama Kehidupan anak. Salah satu resiko terjadinya stunting pada anak adalah kurang asupan gizi pada masa ibu hamil. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang berhubungan dengan perilaku pencegahan anak stunting pada saat ibu hamil. Penelitian ini menggunakan rancangan cross sectional. Sampel pada penelitian sebanyak 74 ibu hamil trimester 1 diwilayah kerja puskesmas Grujugan, Tenggarang, dan Maesan Kabupaten Bondowoso yang diperoleh menggunakan simple random sampling. Hasil analisis ada hubungan dukungan keluarga (p-value 0,057), dukungan lingkungan (p-value 0,010) namun tidak ada hubungan antara perilaku pencegahan stunting dengan usia (p-value 0,361), pendidikan (p-value 0,230) dan pendapatan (p-value 0,240). Rekomendasi dari penelitian ini bagi tenaga kesehatan memberikan pendidikan kesehatan pada ibu hamil dan keluarga untuk memberikan pemahaman tentang pencegahan stunting sejak usia kehamilan dini Kata kunci: Perilaku, Pencegahan, stunting, ibu hamil

Abstract

Kota Surabaya menduduki peringkat pertama dalam hal jumlah kasus DBD di Jawa Timur. Puskesmas Rangkah merupakan salah satu Puskesmas yang berada di wilayah Kota Surabaya. Setiap tahunnya Puskesmas Rangkah mengalami kejadian DBD. Pemberdayaan Wamantik diharapkan mampu menurunkan angka kepadatan jentik di wilayah Puskesmas Rangkah sehingga peluang terjadinya kejadian DBD bisa berkurang juga. Tujuan penelitian ini adalah untuk menganalisis hubungan antara pelatihan Siswa Pemantau Jentik (Wamantik) dengan House Index dan Container Index jentik nyamuk Aedes sp. di Puskesmas Rangkah Surabaya Tahun 2015. Metode penelitian ini menggunakan rancangan penelitian eksperimen dengan desain posttest only control group design. Hasil dari penelitian ini menunjukkan bahwa terdapat perbedaan signifikan nilai House Index pada kelompok sampel perlakuan dengan kemompok kontrol dengan nilai p = 0,000 atau p < 0,05. Terdapat perbedaan signifikan nilai Container Index pada kelompok sampel perlakuan dengan kemompok kontrol dengan nilai p = 0,000 atau p < 0,05. Kesimpulan dalam penelitian ini terdapat hubungan yang signifikan antara pelatihan wamatik dengan persentase House Index dan Container Index.

Abstract

Latar belakang. Hidrosefalus merupakan kelainan sistem saraf pusat yang paling umum terjadi baik pada bayi, anak, maupun remaja dan dapat menyebabkan konsekuensi serius berupa mortalitas. Informasi yang menyediakan faktor risiko mortalitas pada anak dengan hidrosefalus masih sangat terbatas.Tujuan. Mengevaluasi dan mengidentifikasi faktor risiko yang berhubungan dengan terjadinya mortalitas pada anak dengan hidrosefalus.Metode. Penelitian analitik observasional pada 89 pasien anak yang menderita hidrosefalus dengan menggunakan data rekam medis pasien anak yang dirawat di bagian instalasi rawat inap RSUD dr. Soetomo periode Januari 2014 hingga September 2016. Analisis menggunakan chi-square dan regresi logistik.Hasil. Didapatkan 89 subjek yang memenuhi kriteria inklusi. Mortalitas pasien anak dengan hidrosefalus adalah 17,97%. Analisis bivariat menunjukkan adanya hasil signifikan pada infeksi meningoensefalitis dengan OR 8,12 (95% CI 2,38-27,6) p=0,001. Sepsis memiliki OR 6,18 (95% CI 1,53-24,9) p=0,01. Kelainan struktur SSP berupa brain edema memiliki OR 4,27 (95% CI 1,25-14,6) p=0,02. Gagal nafas memiliki OR 56,0 (95% CI 6,16-508,9) p=0,001. Hasil analisis multivariat menunjukkan gagal nafas dan brain edema memiliki nilai OR (95% CI) berturut-turut 192,8 (9,92-3745,8) dan 10,07(1,23-82,5) dengan nilai p<0,05.Kesimpulan. Gagal nafas dan brain edema merupakan faktor risiko mortalitas pada anak dengan hidrosefalus.

Abstract

Introduction: Preschool-aged children are not able to manage their nutrition independently. They need help, especially from their mother. Mother's behavior has effect on preschool's nutritional status. The aim of this study was to develop nutrition demand enquiry model for preschool-aged children based on Health Promotion Model.Method: Design used in this research was an observational analytic with cross sectional approach that was conducted in Karangturi Village, Gresik, East Java. The population was mothers of preschool aged children listed on Integrated Health Post for Underfives (Posyandu Balita) along April–May 2012. Simple random sampling was used to take the sample. Sample size was 90 respondents. The independent variables were personal factors  (age, motivation, and ethnicity), perceived benefit, perceived barrier, perceived self-efficacy, and commitment. The dependent variable was mother's behavior on nutrition of preschool children. Data were collected using questionnaire and food recall 24 hour. Data were analyzed using PLS (Partial Least Square) Regression.Result: The result showed that 1) personal factors had correlation with perceived benefit, perceived barrier, and perceived self-efficacy; 2) perceived benefit and perceived barrier had correlation with commitment; 3) commitment had no correlation with mother's behavior on nutrition. Perceived benefit and perceived barrier had direct correlation with mother's behavior on nutrition. Discussion: It can be concluded that mother's perceived benefit and perceived barrier was correlated with mother's behavior on nutrition. Nurses are key health care professionals responsible for increasing health education activities about how to manage nutrition of preschool age children. So, mother should have good behavior on preschool age children's nutrition.
