Abstract

Fibromatous epulis is one of the most common features of the gingival enlargement. This condition results from inflammation of the gingiva, leading to fibrotic overgrowth caused by local or systemic factors. These enlargements increase gingival pocket depth, which may lead to periodontal disease causing functional disturbances in mastication, altered speech, aesthetics, and even psychological problems. A- 42-year-old male patient with gingival fibrotic overgrowth known as fibromatous epulis on proximal teeth in the middle upper front region extended to the palatal and accompanied by slight pain. The clinical appearance of this lesion was firm, slightly soft, and fibrotic, with 6 mm probing depth and bleeding on gingival probing at the proximal of 11 and 21 teeth region. In this case, periodontal surgery to eliminate gingival enlargement was a gingivectomy under local anaesthesia. The fibromatous epulis was entirely eliminated by gingivectomy, also effectively treated the inflammation, facilitated oral prophylaxis procedure, and increased the aesthetics.

Abstract

Introduction: Periodontitis is a multifactorial disease that causes inflammation and affects the tooth-supporting tissues, characterized by loss of periodontal tissue attachment and bone destruction. The main cause of periodontitis is periopathogenic bacteria, but systemic conditions can be risk factors for the development and severity of periodontal disorders. This study aimed to describe the effect of photodynamic therapy as an adjunctive therapy for periodontitis patients with diabetes. Review: Diabetes mellitus (DM) and periodontitis are complex diseases with a bidirectional relationship, and can increase the risk of periodontitis and periodontal inflammation affecting glycemic control.  This study is a literature review using the scoping review method, with a systematic approach to map, analyze, and summarize previous research evidence. The article search used Pubmed, ScienceDirect, EBSCOhost, Researchgate, and Taylor and Francis Online databases from 2013 to 2023 with the inclusion criteria of articles from 2013 to 2023 that discussed the effect of photodynamic therapy as an adjunctive therapy in cases of periodontitis accompanied by diabetes mellitus. Five databases were used for the search method and four relevant studies were identified that stated that photodynamic therapy as an adjunctive therapy has a positive effect on the management of periodontitis in diabetic patients based on clinical and biochemical parameters. Conclusion: Photodynamic therapy showed positive results as adjunctive therapy in many cases of periodontitis with diabetes.

Abstract

exposure of more than 2 mm of gingival tissue during smiling, often leading to aesthetic concerns, particularly in female patients. The etiology of EGD can be multifactorial, involving dentoalveolar causessuch as gingival overgrowth, altered passive eruption, and short clinical crownsor non-dentoalveolar causes such as vertical maxillary excess and hyperactive upper lip. Lip repositioning surgery (LRS) offers a conservative and minimally invasive alternative to orthognathic surgery in appropriate cases. Case Report: This case involves a 36-year-old female presenting with symmetrical facial proportions, competent lips, and excessive gingival display above the esthetic norm. Upon clinical evaluation, the primary contributing factor was identified as a hyperactive upper lip. The patient underwent lip repositioning surgery, which included partial-thickness incisions and mucosal strip excision in the maxillary vestibule to reduce lip elevation during smiling. The procedure was completed under local anesthesia with no intraoperative complications. At follow-up, the patient demonstrated significant improvement in smile aesthetics, with a notable reduction in gingival exposure and high levels of satisfaction. No adverse postoperative effects were observed. Conclusion: Lip repositioning surgery is a simple, safe, and effective technique for managing EGD caused by a hyperactive upper lip. It provides predictable aesthetic outcomes with minimal morbidity and is best indicated in cases with mild to moderate gingival display. Although short-term results are promising, long-term studies are needed to evaluate the stability and efficacy of the procedure over time.

Abstract

The use of dental Implants in Indonesia has increased significantly. Public knowledge and demand for dental implant treatments are growing. The public’s need to replace missing teeth with removable dentures or bridges gradually shifting towards dental implant. The aesthetic needs for dental implant, especially in the anterior region are highly desired by patients. However, this is often not achieved due to several factor, one of it was the non-optimal emergence profile of the implant supra structure. A deep understanding of the gingival tissues in relation to implants to achieve maximum esthetics is utmost importance. This case report presented the treatment with an implant using a PEEK temporary abutment as a custom healing abutment The use of customized temporary abutment as a healing abutment could provide an effective method to enhance esthetic and guide tissue response during the healing phase, so that the aesthetic could be achieved.
