Abstract

Fertility has been a major issue in the management of cystic ovarian neoplasm. This case report presents an extreme case of ultra-low AMH levels in a young female with recurrent cystic ovarian neoplasm and analyzes the potential causes. A twenty-two-year-old female presented with stomach discomfort. The patient had undergone two surgeries for ovarian neoplasm within the last six years. The patient had another abdominal lump suspected to be a residual tumor mass. After further examination, the patient was diagnosed with cystic ovarian neoplasm. The laboratory findings showed low T4 levels, increased TSH levels, and ultra-low AMH levels (0.023 ng/mL). Management of cystic ovarian neoplasm should concern the effect on fertility. The AMH level can be used for pre-treatment counseling in these patients.

Abstract

Drug-resistant epilepsy presents significant challenges in treating epileptic patients, leading to recurrent seizures and necessitating the use of polypharmacy with anti-epileptic drugs. Both of these conditions contribute to increased oxidative stress, which is detrimental to the brain. The aim of this study was to determine the role of vitamins C and E in reducing oxidative stress and seizure frequency in drug-resistant epileptic patients. This was a double-blinded, randomized clinical trial with a placebo, parallel design, and block randomization. The subjects were drug-resistant epileptic patients aged 1–18 years who received routine treatment. Randomization was performed on 100 patients who were divided into the treatment or placebo groups. The patients received a combination of vitamin C (100 mg/day) and vitamin E (200 IU/day for those <5 years or 400 IU/day for those ≥5 years) or a placebo for eight weeks. Malondialdehyde (MDA) levels and seizure frequency were measured prior to and after the intervention. A total of 42 and 46 patients were followed till the end of the study in the intervention and placebo groups, respectively. Our data indicated that the MDA levels prior to treatment were not significantly different between the treatment and placebo groups (0.901 vs 0.890 mmol/mL, p=0.920) and were significantly reduced after the treatment in both the treatment group (p<0.001) and placebo group (p=0.028). The changes in MDA levels (between post- and pre-treatment) were also not significantly different between the two groups (p=0.181). Our per-protocol analysis indicated that the reduction in seizure frequency was significantly higher in the treatment group compared to the placebo group (95% vs 35%, p<0.001), with 92% and 60% relative and absolute risk reduction, respectively. The intention-to-treat analysis also indicated that the reduction in seizure frequency was significantly higher in the intervention group than in the control group (80% vs 32%, p<0.001), with relative and absolute risk reduction of 70% and 48%, respectively. There was no significant relationship between changes in MDA levels and seizure frequency in either group. In conclusion, vitamins C and E could reduce seizure frequency and, therefore, could be considered as adjuvant therapy in drug-resistant epileptic patients.

Abstract

Turnaround time (TAT) merupakan tolak ukur mutu kinerja laboratorium medik. TAT prokalsitonin tidak mampu mencapai target TAT yang ditetapkan laboratorium. Tujuan: mengkaji tahap keterlambatan TAT, melakukan upaya perbaikan, dan mengevaluasi upaya perbaikan. Metode: Penelitian prospektif ini dilakukan di Laboratorium RS Cipto Mangunkusumo pada tanggal 3 hingga 31 Januari 2022. Untuk mengukur strategi peningkatan TAT dengan tepat, penelitian ini membagi alur kerja prokalsitonin menjadi beberapa checkpoint. Hasil: Tahapan terpanjang adalah dari masuknya spesimen ke Abbott Architect c8000 hingga keluar (50 menit). Tahap terlama kedua terjadi pada alat analisa prokalsitonin (21 menit). Ketiga adalah pemindahan spesimen dari Abbott Architect c8000 ke penganalisis prokalsitonin (18 menit). Keempat, waktu dari keluarnya spesimen dari alat analisa hingga keluarnya hasil (16 menit). Contoh waktu check-in untuk masuk ke Abbott Architect c8000 (14 menit). Upaya perbaikan pun dimulai, terdiri dari optimalisasi kinerja LIS dan analis. Evaluasi menunjukkan TAT prokalsitonin

Abstract

BACKGROUND: Amino acids are one of the essential metabolites, especially the 20 amino acids that are preserved as the building blocks of protein. Alterations in amino acid concentrations are related to disease such as inborn error of metabolism, cancer, as well as nutritional status. Hence, it is necessary to define reference values of 20 plasma-free amino acids for Indonesian toddlers and to establish a robust measurement technique using chromatography with tandem mass spectrometry (MS).METHODS: This study was a cross-sectional preliminary study to establish reference values. The sample was prepared by mixing plasma with 20% sulfosalicylic acid. Plasma-free amino acids were measured with high-performance liquid chromatography (HPLC) non-derivatization technique using column XTerra for chromatographic separation coupled with tandem MS. Amino acids reference values were taken from 101 healthy Indonesian toddlers aged 1-3 years old. Since amino acids data were not Gaussian distributed, the lower and upper of the reference value was established from the 5th percentile and the 95th percentile, respectively.RESULTS: Analysis for 20 amino acids was validated. The accuracy ranged from 90.53-105.39% and the precision ranged from 0.06-3.80%. The limit of detection range was 1-2 nmol/mL, and the limit of quantification range was 2-4 nmol/mL. The result was linear, with R2 higher than 0.998. There was no significant difference between boys and girls for all amino acids except for glycine.CONCLUSION: HPLC with tandem MS method can be used to evaluate amino acids in clinical practice. The reference values obtained are specific for aged 1-3 years old from urban areas in Indonesia.
