Abstract

Fatigue and sleep disturbances are two common symptoms experienced by cancer patients undergoing chemotherapy. These conditions negatively impact quality of life and the effectiveness of treatment. One promising non-pharmacological approach is autogenic relaxation, a relaxation technique that combines self-suggestion, controlled breathing, and focused attention on bodily sensations to achieve physical and mental calmness. This study aims to evaluate the effectiveness of autogenic relaxation in improving sleep quality and reducing fatigue levels in cancer patients undergoing chemotherapy. The study employed a quasi-experimental design with a pretest–posttest and control group. A total of 30 patients were divided into two groups: intervention (n = 15) and control (n = 15). The sampling technique used was purposive sampling. The intervention was conducted for two weeks, twice a day, using an audio guide. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), and fatigue was assessed using the Fatigue Assessment Scale (FAS). The intervention group showed a decrease in mean PSQI scores from 11.2 to 8.1 (p = 0.017) and FAS scores from 34.5 to 28.3 (p = 0.031). Data analysis was performed using the Shapiro–Wilk normality test, followed by the Wilcoxon test for analysis within groups, and the Mann–Whitney test for between groups. The control group did not show any significant changes. These results indicate that autogenic relaxation stimulates a physiological relaxation response that contributes to improved sleep and reduced fatigue. Autogenic relaxation is an effective and applicable non-pharmacological supportive intervention for cancer patients during chemotherapy.

Abstract

Diabetic foot ulcers are one of the chronic complications of diabetes mellitus (DM) that can lead to serious infections and even amputation. Poor self-care ability is a major risk factor in the development of chronic wounds among DM patients. The Diabetes Self-Management Education (DSME) intervention has been considered effective in improving patients’ self-management abilities; however, contextual evidence in Indonesia remains limited. This study aimed to evaluate the effectiveness of DSME intervention in improving self-care abilities among patients with diabetic foot ulcers at RSUD Banjarmasin. The study employed a quasi-experimental design using a one group pre-test and post-test approach. A total of 30 patients were selected using total sampling. The DSME intervention was delivered over seven consecutive days through a structured, face-to-face educational approach. Self-care ability was measured using the Summary of Diabetes Self-Care Activities (SDSCA) questionnaire before and after the intervention. Data were analyzed using a paired sample t-test. Prior to the intervention, the majority of respondents (70%) were categorized as having poor self-care. After the intervention, 80% of respondents were categorized as having good self-care. The paired sample t-test showed a significant difference in self-care scores before and after the DSME intervention (p < 0.001). The DSME intervention was effective in enhancing self-care abilities among patients with diabetic foot ulcers.

Abstract

Preoperative anxiety is a common psychological condition that can negatively affect patients' physiological responses and decrease their quality of life. Patients undergoing spinal anesthesia are particularly vulnerable, as they remain conscious throughout the procedure. Non-pharmacological interventions such as Virtual Reality (VR) offer an innovative approach to reducing anxiety; however, evidence of its effectiveness in regional hospital settings remains limited. This study aimed to examine the effect of Virtual Reality intervention on anxiety levels and quality of life among preoperative patients undergoing spinal anesthesia at Banjarmasin Regional General Hospital. A quasi-experimental design with a non-equivalent control group was employed. The sample consisted of 30 patients purposively divided into two groups: 15 in the intervention group and 15 in the control group. The intervention group received VR exposure for 20 minutes prior to surgery, while the control group received standard care. The instruments used were the Amsterdam Preoperative Anxiety and Information Scale (APAIS) and the Short Form-36 (SF-36) questionnaire. Data were analyzed using the Mann-Whitney and Wilcoxon Signed-Rank tests. Before the intervention, there were no significant differences between the intervention and control groups in anxiety scores (p = 0.614) or quality of life scores (p = 0.791). After the intervention, the intervention group showed a significant reduction in anxiety scores (13.8 ± 2.7) compared to the control group (20.1 ± 2.6) with a p-value of 0.004. A significant improvement was also observed in the quality of life scores of the intervention group (73.1 ± 6.4) compared to the control group (60.2 ± 7.3) with a p-value of 0.007. Virtual Reality intervention was effective in reducing anxiety and improving the quality of life of preoperative patients undergoing spinal anesthesia. VR has the potential to serve as a practical, efficient, and feasible non-pharmacological intervention in healthcare settings.

Abstract

Spinal anesthesia is a commonly used regional anesthetic technique in surgical procedures, but it is often associated with delayed motor recovery postoperatively. Passive Range of Motion (ROM) exercise is a physiotherapy intervention that has the potential to accelerate motor recovery by stimulating the neuromuscular system and enhancing peripheral blood circulation. This study aimed to evaluate the effectiveness of passive ROM exercises on motor recovery in patients after spinal anesthesia. A quasi-experimental design with a posttest-only control group approach was used. This study used consecutive sampling to recruit respondents who met the inclusion criteria. The sample consisted of 30 patients who underwent spinal anesthesia and were evenly divided into an intervention group (n = 15) and a control group (n = 15). The intervention group received passive ROM exercises for 24 hours postoperatively in four scheduled sessions, while the control group received standard postoperative care. The data analysis used was univariate analysis to describe the characteristics of respondents, while bivariate analysis used the Mann-Whitney test to assess differences in Bromage scores. Motor recovery was measured using the Bromage Scale every four hours. A significant difference in Bromage scores was found between the intervention and control groups at the 24th hour (p = 0.019), with the intervention group showing a lower median score (0) compared to the control group (1), indicating faster motor recovery. Passive ROM exercise is effective in accelerating motor recovery in post-spinal anesthesia patients and can be recommended as part of postoperative care protocols.

Abstract

Sleep disturbance is one of the common complaints experienced by patients with Chronic Kidney Disease (CKD) stage V undergoing hemodialysis, which has an impact on decreasing the quality of life physically, psychologically, and socially. Non-pharmacological therapy such as the Benson relaxation technique is considered effective and safe in overcoming sleep disturbances and improving the emotional well-being of patients. This study aims to evaluate the effectiveness of the Benson relaxation technique in improving the quality of life of patients with CKD stage V. The research design used was a quasi-experimental with a pre-test and post-test approach with a control group, involving 60 respondents who were divided into intervention groups (n=30) and control groups (n=30) through a purposive sampling technique. The intervention was carried out for 14 days with a duration of 15–20 minutes of exercise per day. The instrument used was a quality of life (QoL) questionnaire consisting of five aspects: satisfaction with therapy, impact on life, fear of complications, social problems, and total QoL score.The Pittsburgh Sleep Quality Index (PSQI) instrument used in this study has been proven valid (r = 0.412–0.698; p < 0.05) and reliable (Cronbach's Alpha = 0.824; p = 0.000), making it suitable for measuring patient sleep quality. Data were analyzed using statistics, with Pearson validity test, Cronbach's Alpha reliability ( α = 0.824), Shapiro-Wilk normality test, and Wilcoxon and Mann-Whitney difference tests. The results showed a significant increase in the total QoL score from 2.63 ± 0.49 to 3.27 ± 0.39 (p = 0.001). All aspects of QoL also experienced a significant increase (p < 0.05), especially in the aspect of satisfaction with therapy and decreased fear of complications. Field findings also showed that patients felt calmer, could fall asleep faster, and showed increased social interaction after the intervention. The conclusion of this study is that the Benson relaxation technique is effective in improving the quality of life of CKD stage V patients and is recommended as a nursing intervention that can be applied routinely and independently in hemodialysis service practices.
