Abstract

Background: Diabetes mellitus can cause various complications if it is not controlled, resulting in the risk of increasing the number drugs prescribed. The number of medications is one factor that can describe the incidence of Potentially Inappropriate Medications (PIMs). Objectives: This Research is to determine the percentage of PIMs based on the Beers Criteria 2023 and to determine the relationship between the number of drug prescriptions and the incidence of PIMs. Methods: used is descriptive observational. A sample of 120 recipes was carried out using purposive sampling which met the inclusion criteria. Data analysis using the chi square test and correlative contingency coefficient test. Results: the research showed that the demographic characteristics of geriatric diabetes mellitus patients at RSU Asy Syifa' Sambi Boyolali for the period January-March 2025 were dominated by female patients of 76 patients (63.33%), the majority aged 60-69 years, namely 83 patients (69.17%), the number of drugs ? 5 namely of  78 patients (65%), and 114 patients (95%) had comorbidities. There is a percentage of PIMs in the amount of 32.24% of the total drug items, the highest number of PIMs is category 2, namely 32.49%. Based on the results of statistical tests, it shows a p-value of 0.217 (p > 0.05), which means there is no statistically significant relationship between the amount of medication and the incidence of PIMs. Apart from that, the correlation coefficient value for the two variables is 0.112, indicating that the level of strength of the relationship is in the very weak category. Conclusion: An increasing the number of prescribed medications is not the only factor contributing to PIMs. The association between polypharmacy and PIM was not statistically significant (p=0.217) and showed a very weak value. These findings emphasize that in geriatric diabetes care, quality prescribing, appropriate management of complex comorbidities, and regular clinical monitoring effectively reduce the risk of polypharmacy. Therefore, evaluating the appropriateness of therapy is far more important than simply limiting the number of medications.

Abstract

Vaccines are the best way to tackle the virus outbreak because vaccines can reprogram immunity systems to create their antibodies The vaccine is unlikely to be initially available to children. The risk of children spreading the virus is not zero, and a surge of cases in the community will be reflected in children as well. The policymakers will also consider all other potential benefits against the risks of vaccination in children. Muslims are obliged to consume only halal products. Vaccines needed to follow halal standardization to be used in Islamic countries. This study has the purpose of observing the connection between halal and vaccines in research. This study uses papers indexed in the Scopus website as secondary data. There are two kinds of data, based on the organization of co-authorship, and co-occurrence of authors’ topic. There are 46 numbers of data, including the organization of co-authorship or where the authors affiliated and 68 of co-occurrence of authors’ topic. The data is analyzed using Vosviewer software. The result shows that the topic of vaccines has a connection to halal and fatwa/ or ulama council’s statement. Thus, no Muslim countries such as the USA and Australia have more relevant points than Muslim countries. It can be concluded that halal and vaccine are intertwined with each other, especially in Muslim countries. Halal is not only related to vaccines based on the result above but also has a connection to other topics such as fatwa, Islamic marketing, developing countries, Islam and acceptance. Halal and vaccines have vast varieties of sub-topics to be developed in studies. Based on the keywords in the papers, non-Muslim countries such as the USA and Australia have more relevant points in the study about halal and vaccines. There are vaccine perceptions from society, things related to halal, as well as things related to COVID-19.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a chronic lung disease characterized by progressive and not fully reversible airflow limitation. One of the main therapies for COPD is inhaled medication delivered through Metered Dose Inhalers (MDI) and Dry Powder Inhalers (DPI). The success of therapy is strongly influenced by the correctness of inhaler technique, as improper use can reduce drug deposition in the airways and affect clinical outcomes. This study aimed to evaluate the accuracy of MDI and DPI inhaler use and its relationship with the FEV₁/FVC ratio in COPD patients at RS UNS. This study employed a non-experimental design with a cross-sectional approach. A total of 100 COPD patients who met the inclusion criteria were selected using purposive sampling. Sociodemographic data and inhaler-use accuracy were obtained through direct interviews, while FEV₁/FVC values were collected from medical records. The data were analyzed using the Chi-Square test with a significance level of α = 0.05. The results showed that most respondents were over 45 years old (80%) and female (56%). The level of correct inhaler use was low in both MDI users (22.2% correct) and DPI users (13.2% correct). There was no significant relationship between sociodemographic factors and inhaler-use accuracy (p > 0.05). However, a highly significant relationship was found between inhaler-use accuracy and achievement of the FEV₁/FVC ratio (p < 0.001), in which all patients with correct inhaler technique achieved the target FEV₁/FVC. In conclusion, appropriate inhaler technique plays a crucial role in achieving optimal pulmonary function outcomes in COPD patients. Therefore, continuous education and regular evaluation of inhaler technique are essential to support the success of COPD therapy.
