Abstract

Puberty is an important period in the development of young girls, marked by various physical, emotional, and social changes. A lack of understanding about puberty and menstruation often leads to feelings of anxiety, shame, and poor hygiene practices. Therefore, early education is essential to help female students recognize their body changes and maintain reproductive health properly. This community service was conducted at MI Tamrinussibyan 1 Al Hikmah Benda, specifically targeting early adolescent female students. The activity was carried out through interactive counseling and group discussions, supported by presentation media and illustrations. The topics covered included an introduction to puberty, understanding menstruation, healthy eating habits, and practices for maintaining personal hygiene and reproductive health. The results showed an improvement in the students' understanding of reproductive health, accompanied by a positive change in their attitudes toward self-care during menstruation. This program is expected to provide essential knowledge for students to navigate puberty in a healthy, clean, and confident manner.

Abstract

Liver cirrhosis is a chronic disease characterized by progressive fibrosis, altered liver architecture, and the formation of regenerative nodules. Although it develops slowly, cirrhosis often leads to serious complications such as ascites, variceal bleeding, jaundice, and hepatic encephalopathy. According to the Global Burden of Disease Study 2019, cirrhosis is among the leading causes of death worldwide, with more than 1.4 million deaths annually. In Indonesia, the prevalence of cirrhosis is reported at 0.4% (Riskesdas, 2021) and shows an increasing trend over the years. This case study aims to describe the application of comprehensive nursing care in a patient diagnosed with liver cirrhosis using the nursing process approach. The research design was a case study conducted on Mr. R, who was admitted to Dahlia Ward at RSUD dr. Soeselo, Tegal. Data collection was carried out through interviews, observations, physical examinations, and medical record reviews. The results revealed that the main nursing problems identified were ineffective peripheral perfusion, ineffective breathing pattern, and knowledge deficit. Nursing interventions provided included positioning the patient in semi-fowler, monitoring vital signs, educating the patient about low-sodium diet and fluid restriction, and collaborating in the administration of diuretics and albumin. After three days of nursing care, the evaluation indicated clinical improvement, evidenced by reduced edema, decreased shortness of breath, and increased patient understanding of his disease and treatment plan. In conclusion, nursing care with a comprehensive process approach has a significant positive impact on the condition of patients with liver cirrhosis. Nurses play an essential role in assessment, planning, implementation, evaluation, and providing health education to prevent complications and improve patients’ quality of life.

Abstract

Non-Communicable Diseases (NCDs) remain the leading cause of global morbidity and mortality, and one of the chronic conditions included in this group is rheumatoid arthritis (RA). RA is an autoimmune disease that affects the synovial joints, resulting in persistent inflammation, chronic pain, stiffness, and eventual deformities if left untreated. In Indonesia, the burden of RA continues to grow; the prevalence reaches 11.9% based on clinical diagnosis and rises to 24.7% when measured using symptom-based diagnosis. This case study aimed to describe family nursing care for a client with RA through a structured nursing process approach to improve quality of life and family involvement. The study took place in Purwodadi Village, Tonjong Subdistrict, Brebes Regency, focusing on the family of Mr. AS with Mrs. W as the client diagnosed with RA. Data collection involved comprehensive interviews, direct observation, physical examination, and documentation review. Assessment findings included chronic pain with an intensity score of 4, limited mobility, and inadequate family knowledge regarding RA management. Nursing interventions implemented were family education on RA, simple rheumatic exercises tailored to the client’s ability, and the application of warm ginger compresses as a non-pharmacological pain relief method. Post-intervention evaluation showed pain reduction to a score of 2, improved independence in performing light daily activities, and enhanced family understanding and support. In conclusion, family-centered nursing care combined with non-pharmacological interventions can effectively reduce pain, improve mobility, and strengthen the family’s role in supporting RA management.

Abstract

Health is a state of complete physical, mental and social well-being, not just freedom from disease or weakness (Maulina et al., 2024). According to WHO, (2022) Mental health is a state of mental well-being that allows a person to overcome the stresses of life, realize their abilities, contribute to their community, learn and work well. Abstract

Thalassemia is an inherited hemolytic anemia caused by genetic abnormalities in chromosomes 11 and 16 that impair globin chain production, leading to insufficient hemoglobin and red blood cells. This study aims to describe nursing care for An. W with thalassemia at the Anggrek 2 Ward of RSUD dr. Soeselo, Tegal Regency. The method used was a descriptive approach through anamnesis, documentation, observation, and literature review. Nursing care was carried out over two days (January 13–14, 2026). Physical examination results on January 12, 2026 showed a pale face, anemic conjunctiva, weakness, CRT >3 seconds, decreased skin turgor, weak peripheral pulse, cold extremities, and laboratory hemoglobin of 5.9 g/dL, with vital signs of pulse 111x/minute, temperature 36.7°C, and respiration 22x/minute. Two nursing diagnoses were established: ineffective peripheral perfusion related to decreased hemoglobin concentration (D.0009), and activity intolerance related to weakness (D.0056). Nursing interventions included circulatory care (I.02079) and energy management (I.05178). Nursing implementation was carried out according to plan. Evaluation on January 14, 2026 showed that both nursing diagnoses were resolved; the patient's hemoglobin improved to 10.7 g/dL after two blood transfusions, and the patient was declared fit for discharge. It is recommended that hospitals establish support groups for thalassemia patients to provide adequate emotional and social support.

Abstract

Stroke is a neurological condition caused by disruption of blood flow to the brain due to blockage or rupture of blood vessels, leading to nerve cell death. WHO (2022) recorded more than 12 million new stroke cases annually, with a prevalence in Brebes Regency reaching 27.83 per mil. The high incidence of stroke accompanied by physical mobility impairment and the family's lack of knowledge about its management highlights the importance of comprehensive family nursing care. This study aims to apply a complete nursing care process including assessment, diagnosis, intervention, implementation, and evaluation to the family of Mr. K with a stroke case. The method used is descriptive with a case study approach through family nursing care. Assessment findings revealed that the patient had experienced stroke for approximately 7 years with a history of hypertension, weakness of the right extremities, decreased muscle strength, blood pressure of 160/90 mmHg, and the family did not understand how to care for stroke at home. Two nursing diagnoses were established: ineffective health maintenance in the family and impaired physical mobility. Interventions included health education about stroke, Range of Motion (ROM) exercises, and rubber ball grip therapy involving the family as caregivers. After 2 days of implementation, both nursing diagnoses were resolved: ineffective health maintenance in the family was resolved and impaired physical mobility was resolved.
