Abstract

Lumihe is one of the villages in the working area of the Tabongo Community Health Center, Tangongo District, Gorontalo Regency and is one of the villages that is a priority for preventing and reducing stunting. In 2021, Tabongo Community Health Center states that most children have participated in the Posyandu program and received complete immunization, but public knowledge and awareness about stunting is still weak. Untreated stunting can cause various health problems such as degenerative diseases, diabetes and ARI so that efforts are needed to resolve these problems, one of which is through community empowerment consisting of mothers who have stunted toddlers, Family Health Empowerment (PKK) and health cadres. Through the revitalization of the GEMAS CETING village (Aware Community Movement to Prevent Stunting) in Lumihe, it is hoped that the community can see, recognize and overcome existing problems and have solutions to problems related to stunting.

Abstract

To determine the effect of supportive psychotherapy and video-directing observed therapy interventions on family compliance in assisting patients with mental disorders in the working area of the Kabila Health Center, Bone Bolango District, Gorontalo City. Research design quasi-experimental design with a one group pre- and post-test design, where the variables to be measured are family adherence in providing treatment assistance, which are included in the dependent variable. The independent variables in this study were supportive psychotherapy and video-directed observation therapy. The sampling technique used was a total sampling technique with a sample size of 39 respondents. Data analysis was performed using univariate analysis, namely descriptive analysis (frequency, percentage, median, minimum value, and maximum value) and bivariate analysis (Wilcoxon test), with a significance level of <0.05, which was performed using SPSS version 25 software. Supportive psychotherapy and video-directing observed therapy increased family compliance accompanying the treatment of patients with mental disorders (p-value of x1=0.020, x2=0.000, p<0.05). Supportive psychotherapy interventions and video-directed observed therapy have been proven to increase family compliance in treatment assistance, which can improve the quality of life of patients with mental disorders.

Abstract

Background: Diabetes mellitus is a chronic disease that occurs when the body cannot produce enough insulin or cannot use insulin effectively, resulting in high blood sugar levels. Most people with diabetes are unaware of or unaware of their condition. Data shows that 17.3% of men and 26.3% of women die from diabetes complications in Lamahu Village, with diabetes mellitus cases ranking the 8th highest in the region. The limited achievement of DM management targets, limited knowledge of health workers about screening and early detection, and suboptimal skills in reporting and managing diabetes are some of the contributing factors to this problem. Purpose: To increase caregiver knowledge and enhance the skills of health workers in managing and preventing diabetes mellitus. Method: This activity was conducted over two days from April to November 2024 at the Lamahu Village Office, with the support of partners such as the Village Head, health workers from the Tapa Community Health Center, the Family Welfare Movement (PKK) group, village officials, and health cadres. This activity involved eight health cadres and 16 caregivers. The outreach activities included providing education on diabetes mellitus management and prevention to the caregivers and training on screening for early detection to the health cadres. The instruments used were posters, leaflets, pocket books, and screening tools. Monitoring was conducted through a WhatsApp group application. The pre-activity assessment data served as the pre-test and the post-activity assessment data served as the post-activity. Results: The majority of caregivers' knowledge before the education activity was in the poor category (13 people (81.2%), while the majority of caregivers' knowledge after the education activity was in the good category (14 people (87.5%). The level of ability of health cadres before the training activity was mostly in the low category, namely 6 people (75.0%), while the level of ability of health cadres after the training activity was mostly in the good category, namely 7 people (87.5%). Conclusion: Educational activities to increase caregiver knowledge and provide screening training to health workers about diabetes mellitus are highly effective intervention models for managing and preventing diabetes mellitus in the community. Interventions based on caregiver education and health worker empowerment are synergistic in supporting early detection of diabetes mellitus at the community level. Suggestion: Health institutions are encouraged to develop similar education and training programs in broader areas, particularly in areas with high diabetes mellitus prevalence. Further research can be conducted to assess the long-term impact of these programs, including changes in health behavior and the effectiveness of early detection in reducing the incidence of diabetes mellitus in the community. It is also hoped that similar intervention models can be applied to the prevention of non-communicable diseases at the village level. Keywords: Caregiver; Diabetes mellitus; Health education; Health worker empowerment; Training Pendahuluan: Diabetes melitus merupakan penyakit kronis yang terjadi ketika tubuh tidak dapat memproduksi insulin yang cukup atau tidak dapat menggunakan insulin secara efektif, menyebabkan kadar gula darah menjadi tinggi. Sebagian besar pendeita diabetes juga tidak menyadari dan memahami penyakit yang dideritanya. Data menunjukkan 17.3% laki-laki dan 26.3% perempuan meninggal akibat komplikasi diabetes di Desa Lamahu, dengan kasus diabetes melitus menjadi peringkat ke-8 tertinggi di wilayah tersebut. Minimnya target capaian pengelolaan DM, rendahnya pengetahuan kader tentang skrining dan deteksi dini, serta belum optimalnya keterampilan dalam pelaporan dan manajemen DM menjadi beberapa faktor yang berkontribusi menjadi penyebab permasalahan ini. Tujuan: Memberikan peningkatan pengetahuan caregiver dan meningkatkan kemampuan kader kesehatan dalam upaya mengelola dan pencegahan diabetes mellitus. Metode: Kegiatan ini dilaksanakan selama dua hari pada bulan April-November 2024 di Kantor Kelurahan Lamahu, dengan dukungan mitra seperti Kepala Desa, tenaga kesehatan dari Puskesmas Tapa, kelompok PKK, perangkat desa, dan kader kesehatan. Kegiatan ini melibatkan 8 orang kader kesehatan dan 16 orang pendamping (caregiver). Kegiatan penyuluhan dilakukan dengan memberikan edukasi tentang pengelolaan dan penyakit diabetes melitus kepada kelompok pendamping, dan memberi pelatihan tentang skrining untuk deteksi dini kepada kader kesehatan. Instrumen yang digunakan berupa poster, leaflet, buku saku, dan alat skrining.   Monitoring dilakukan melalui aplikasi grup WhatsApp. Data penilaian sebelum kegiatan adalah sebagai data pre-test dan penilaian setelah kegiatan adalah sebagai data post-test. Hasil: Menunjukkan bahwa sebagian besar tingkat pengetahuan caregiver sebelum kegiatan edukasi adalah dalam kategori kurang yaitu sebanyak 13 orang (81.2%), sedangkan tingkat pengetahuan caregiver sesudah kegiatan edukasi menjadi sebagian besar dalam kategori baik yaitu sebanyak 14 orang (87.5%). Untuk tingkat kemampuan kader kesehatan sebelum kegiatan pelatihan adalah mayoritas dalam kategori rendah yaitu sebanyak 6 orang (75.0%), sedangkan tingkat kemampuan kader kesehatan sesudah kegiatan pelatihan menjadi sebagian besar dalam kategori baik yaitu sebanyak 7 orang (87.5%). Simpulan: Kegiatan edukasi peningkatan pengetahuan caregiver dan pemberian pelatihan skrining pada kader kesehatan tentang diabetes melitus merupakan model intervensi yang sangat efektif dalam upaya pengelolaan dan pencegahan diabetes melitus di masyarakat. Intervensi berbasis edukasi caregiver dan pemberdayaan kader adalah sinergisitas tindakan dalam mendukung deteksi dini diabetes melitus di tingkat komunitas. Saran: Diharapkan kepada lembaga kesehatan untuk membuat program edukasi dan pelatihan serupa di wilayah yang lebih luas, terutama di daerah yang memiliki status diabetes melitus tinggi. Penelitian lanjutan dapat dilakukan untuk menilai dampak jangka panjang program ini, termasuk perubahan perilaku kesehatan dan efektivitas deteksi dini dalam menekan angka kejadian diabetes melitus di masyarakat. Diharapkan juga bahwa model intervensi serupa dapat diterapkan untuk pencegahan penyakit tidak menular di tingkat desa.
