Abstract

Abstract Background: Chronic kidney failure is a health problem that needs special attention. The number of end-stage renal failure sufferers undergoing hemodialysis therapy increases every year. Hemodialysis therapy is an appropriate action to replace kidney function with a method of supplying fluid intake which results in most patients complaining of dry mouth (xerostomia). Objective: Fluid restriction generally causes dry mouth in hemodialysis (HD) patients. The observed effects of chewing gum on the amount of saliva, interdialytic weight gain (IDWG), and thirst levels in HD patients are inconsistent. We are considering whether gum can modulate this third parameter in patients undergoing HD. Method: This study used purposive sampling with 37 patients on HD. Thirst levels were measured using a self-rated visual analogue scale (VAS) and the summed xerostomia inventory (SXI). Results: The level of thirst after blood washing was significantly lower in the gum chewing group compared to the control group with a p value (0.000) <0.005. The amount of saliva was higher in the gum group than in the control group before dialysis, but the amount of saliva was comparable between the groups after dialysis. In addition, the amount of saliva was not associated with IDWG in patients undergoing HD before or after dialysis. Conclusion: Chewing gum can relieve thirst but does not affect the amount of saliva and IDWG in HD patients. Therefore, we suggest that chewing gum may be a useful approach to treat dry mouth and thirst in HD patients.

Abstract

Bubuk serbuk jahe merah adalah bahan olahan berbahan baku jahe merah yang banyak di produksi industri kecil dan menegah. Bahan baku jahe merah berasal dari petani jahe merah yang kebanyakan dihasilkan daerah pegunungan Kabupaten Simalungun. Tahun 2020, produksi jahe merah di desa dolok saribu mencapai 323 ton dengan harga 33.500 rupiah per kilo gram (Kantor Kepala Desa Dolok Saribu). Tahun 2021-2023 harga jahe merah menurun secara drastis 3.300 rupiah per kilogram. Hal ini menyebabkan kerugian petani diperkirakan mencapai 1,7-2,3 miliar rupiah tahun 2020. Tujuan dari Pengembangan Kemitraan Masyarakat ini adalah mengembangkan produksi serbuk jahe merah dengan pemasaran berbasis pariwisata sehat sebagai produk kearifan lokal di Bukit Indah Simarjarunjung (BIS), Kabupaten Simalungun. Produksi bubuk serbuk jahe merah dari bahan baku lokal dikelola oleh kelompok tani jahe merah yang terdiri dari 3 kelompok (11 petani). Tiap kelompok diberikan peralatan teknologi mesin pembubuk jahe sebanyak 3 unit. Petani diberikan pelatihan untuk melakukan proses dari penggilingan, pengeringan, pencampuran bahan baku, pengemasan, sampai pada pemasaran produksi. Hasil dari PKM ini telah memproduksi serbuk jahe merah sebanyak 30 bungkus ukuran 1 kg, 30 bungkus ukuran 500 gram, dan 30 bungkus ukuran 200 gram dengan nama produk kemasan JAHE DOLOK. Tiga kelompok petani jahe merah telah dilatih secara terampil untuk memproduksi secara hygienis produk serbuk jahe merah mulai dari proses pembubukan, pengeringan, pencampuran bahan, pengemasan, dan pemasaran. Kelompok PKM ini juga telah menjalin kerjasama dengan 10 outlet pemasaran di Tepat Pariwisata Bukit Indah Simarjarunjung dan Pantai Paris Tiga Ras. Pemasaran online dikembangkan dari tiap outlet kerjasama. Rata-rata penjualan pada tahap promosi dari 10 outlet mencapai peningkatan rata-rata 51,6% per bulan setelah 1 bulan promosi.

Abstract

Digital transformation in the health sector requires health service facilities to adopt electronic medical records as an effort to increase efficiency, effectiveness and quality of service. This research aims to analyze the application of electronic medical records to the quality of health services using the human organization technology-fit method at RSU Madani Medan in 2025. This type of research is quantitative, cross sectional in nature. The population in this study was all 222 health workers who used electronic medical records. The sampling technique used was purposive sampling with a sample size of 69 employees. The measuring instrument used is a questionnaire sheet with a Guttman measurement scale. The results of data analysis were processed using SPSS with the Chi-Square test. Data obtained from the human aspect shows no influence of the implementation of RME on the quality of health services with a p-value of 0.760 > α 0.05. From the organizational aspect, there is no influence of the implementation of RME on the quality of health services with a p-value of 0.688 > α 0.05. From the technological aspect, there is an influence of the implementation of RME on the quality of health services with a p-value of 0.035 < α 0.05. From the net-benefit aspect, there is no effect of implementing RME on the quality of health services with a p-value of 0.804 > α 0.05. Suggestions for hospitals to pay more attention to the process of implementing electronic medical records so that electronic medical records can be fully optimized at RSU Madani Medan so as to increase patient satisfaction in the future.
