Abstract

Infeksi saluran kemih (ISK) berulang didefinisikan sebagai setidaknya tiga episode ISK dalam dua belas bulan, atau setidaknya dua episode dalam enam bulan. Menurut World Health Organization (WHO), penyakit infeksi saluran kemih (ISK) adalah penyakit infeksi kedua tersering dan sebanyak 8,3 juta kasus dilaporkan per tahun. Terdapat banyak faktor yang dapat mempengaruhi perkembangan dan kekambuhan ISK. Wanita yang pernah mengalami ISK berulang memiliki resiko lebih tinggi dibandingkan mereka yang tidak pernah terdiagnosis ISK. Antibiotik merupakan tatalaksana profilaksis pada ISK berulang. Namun ada alternatif yang tersedia, seperti cranberry, probiotik, dan obat tradisional cina. Cranberry merupakan alternatif yang dapat dipilih dalam pencegahan ISK di era tingginya angka resistensi terhadap antibiotik dan ISK berulang. Cranberry bekerja terutama dengan mencegah perlekatan pili tipe 1 dan strain p-fimbriae ke epitel saluran kemih. Penelitian ini bertujuan untuk mengetahui manfaat penggunaan cranberry terhadap ISK berulang serta efektivitas penggunaan cranberry terhadap profilaksis terjadinya ISK berulang dibandingkan dengan antibiotik profilaksis. Metode penelitian ini merupakan literature review dengan pencarian dan pemilihan literatur-literatur yang diterbitkan dalam rentang 10 tahun terakhir kemudian telah dilakukan seleksi jurnal dengan kriteria akses terbuka dan yang memenuhi didapatkan 47 jurnal. Hasil penelitian menunjukkan jus Cranberry dikatakan dapat menurunkan pH urin untuk mengobati ISK dan memiliki zat proanthocyanidins sehingga mencegah perlekatan bakteri dalam saluran kemih. Studi lain melaporkan bahwa konsentrasi asam hipurat dalam urin tidak cukup untuk efek antibakteri. Selain itu beberapa studi menyatakan bahwa tidak ada perbedaan yang signifikan antara penggunaan produk Cranberry dengan pasien yang menerima placebo. Sehingga konsumsi ekstrak Cranberry ini tidak dapat menggantikan antibiotik saat terjadi ISK yang sedang aktif. Konsumsi ekstrak cranberry dapat disarankan untuk terapi tambahan pada ISK yang sedang aktif, maupun sebagai profilaksis ISK berulang, namun harus tetap bersamaan dengan antibiotik..

Abstract

Hospitals as places of health services must provide quality services so they are required to improve the quality of their services. These service efforts are carried out comprehensively and continuously. To achieve services that are in line with people's expectations, there are indicators of 5 dimensions of service that can be carried out, namely reliability, responsiveness, assurance, empathy, and physical evidence. This research was conducted to analyze whether there is a relationship between service quality and patient satisfaction at YPK Mandiri Hospital, Central Jakarta. Patient satisfaction is an important indicator for measuring health services because it influences clinical outcomes and medical malpractice claims. Assessing patient satisfaction can provide good input on hospital quality in terms of compiling evaluations that refer to patient assessments. In this study, there were 310 samples selected using random selection techniques from small segments of individuals or members of the entire population and then analyzed using PLS-SEM. The research results show that there is a relationship between service quality and patient satisfaction, as seen from the results of the analysis which obtained significant results and a positive influence. The two units have the same relationship but the relationship is different, the outpatient unit has a stronger relationship than the inpatient unit. This implies that the hospital, both in inpatient and outpatient units, must maintain a good quality of service for patient satisfaction. Rumah sakit sebagai tempat pelayanan kesehatan harus memberikan pelayanan yang bermutu sehingga dituntut untuk meningkatkan mutu pelayanannya. Upaya pelayanan tersebut dilakukan secara menyeluruh dan berkesinambungan. Untuk mencapai pelayanan yang sesuai dengan harapan masyarakat, terdapat indikator 5 dimensi pelayanan jasa yang dapat dilakukan, yaitu keandalan (reliability), daya tanggap (responsiveness), jaminan (assurance), empati (empathy), dan bukti fisik (tangible). Penelitian ini dilakukan untuk menganalisis apakah terdapat pengaruh antara kualitas pelayanan terhadap kepuasan pasien di RS YPK Mandiri Jakarta Pusat. Kepuasan pasien merupakan indikator penting untuk mengukur pelayanan kesehatan karena memengaruhi hasil klinis dan klaim malpraktek medis. Menilai kepuasan pasien dapat memberikan input yang baik kepada kualitas rumah sakit dalam hal menyusun evaluasi. Penelitian ini terdapat 310 sampel yang dipilih dengan menggunakan teknik pemilihan acak dari segmen kecil individu atau anggota dari seluruh populasi dan kemudian dianalisis menggunakan PLS-SEM. Hasil penelitian menunjukkan bahwa terdapat pengaruh positif antara kualitas pelayanan dengan kepuasan pasien, dilihat dari hasil analisa yang didapatkan hasil signifikan. Pada kedua unit memiliki hubungan yang sama namun kaitannya berbeda, unit rawat jalan memiliki kaitan yang lebih kuat dibanding unit rawat inap. Hal ini menyiratkan bila pihak rumah sakit baik di unit rawat inap maupun rawat jalan harus menjaga kualitas pelayanan yang baik untuk kepuasan pasien.

Anemia is a health issue that commonly arises, characterized by a condition in which the body's hemoglobin levels fall below the normal range. According to the 2018 Riskesdas data, the prevalence of anemia among adolescent girls reaches approximately 32%, meaning that 3 to 4 out of every 10 adolescent girls experience anemia. Anemia in adolescent girls has the potential to result in various negative consequences, such as reduced immunity, decreased academic performance, and diminished productivity. These observations emphasize the necessity of conducting a community diagnosis to enhance awareness and reduce cases of anemia among adolescent girls in the Puskesmas Gembong working area. The primary objectives include improving knowledge about anemia and decreasing the number of anemia cases among adolescent girls in the Puskesmas Gembong working area. An intervention plan in the form of educational sessions about anemia was developed. Following the intervention, there was an improvement in knowledge, with post-test scores exceeding 60% for over 50% of the participants. Therefore, this community diagnosis serves to enhance knowledge about anemia among adolescent girls and has resulted in the identification of anemia cases among them, leading to appropriate referrals for treatment.

Abstract

ABSTRACT Chronic suppurative otitis media (CSOM) is a troubling condition that encompasses a range of factors contributing to its etiology. In this context, we present an intriguing and atypical case that adds a novel dimension to the understanding of CSOM. This particular case is marked by its prolonged and treatment-resistant nature, defying conventional medical treatments and prompting us to explore less common etiological pathways. Our case, however, deviates from the norm. It presents a conundrum that led us to investigate an alternative and less explored avenue. During our comprehensive diagnostic evaluation, our attention was drawn to the possibility of a hematogenic infection that may have originated from dental caries. This scenario emerged as the primary candidate for the source of the patient's chronic suppurative otitis media. This intriguing case represents a unique instance of a hematogenic infection stemming from dental caries. The hematogenic pathway, while previously considered uncommon, may be the precipitating factor behind the development of chronic suppurative otitis media in this patient. This perspective widens our understanding of the various potential etiological factors contributing to CSOM and highlights the importance of considering less conventional sources of infection. In the context of otologic disorders, the case we present underscores the necessity of a thorough and holistic diagnostic approach, acknowledging that the origins of such conditions can often be multifaceted and not limited to the conventional causes. Furthermore, this case serves as a reminder for clinicians to maintain a high level of suspicion for atypical etiological pathways when faced with treatment-resistant and persistent manifestations of CSOM. Keywords: Chronic Suppurative Otitis Media, Dental Caries, Odontogenic, Hematogenic
