Abstract

Polip antrokoanal merupakan lesi polipoid jinak yang muncul dari mukosa sinus maksilaris, melewati ostium sinus dan meluas ke koana. Polip antrokoanal biasanya memiliki dua komponen, yaitu bagian polipoid kistik dan padat. Meskipun ada beberapa kasus polip antrokoanal bilateral yang disajikan dalam literatur, polip antrokoanal hampir selalu unilateral. Penyebab dari polip antrokoanal belum pasti dan lebih sering terjadi pada anak-anak dan dewasa muda, serta mencakup sekitar 10% dari seluruh kasus polip hidung pada orang dewasa dan 35% dari seluruh kasus polip hidung pada anak-anak. Seorang pasien laki-laki berusia 7 tahun datang ke poliklinik THT-KL RS Sumber Waras Jakarta dengan keluhan hidung kiri tersumbat sejak 6 bulan yang lalu dan disertai pilek yang sulit dikeluarkan. Pada pemeriksaan endoskopi hidung tampak kavum nasi kiri tertutup massa polipoid berwarna merah muda pucat disertai sekret serous, massa tidak mudah berdarah ketika disentuh. Pemeriksaan Tomografi Komputer Sinus Paranasal tanpa kontras menunjukkan gambaran massa hipodens dengan karakteristik benign yang memenuhi sinus maksilaris kiri, kavum nasi kiri, dan mencapai koana sisi kiri sugestif polip antrokoanal yang menutup osteomeatal kompleks kiri. Pasien dilakukan bedah sinus endoskopi fungsional (BSEF) dan polipektomi. Jaringan polip yang diangkat diperiksa secara histopatologi dan hasilnya menunjukkan gambaran histologis sesuai dengan polip antrokoanal. Pada evaluasi pasca-operasi dua bulan kemudian, pasien tidak lagi mengeluhkan adanya gejala, dan pemeriksaan nasoendoskopi tidak menunjukkan adanya kekambuhan polip.

Abstract

Antrochoanal polyps (ACPs) are predominantly benign growths originating from the mucosal lining of the maxillary antrum, constituting a minor percentage of nasal polyps. This case report examines a unique presentation of a substantial ACP in a 17-year-old male, highlighting the challenges in achieving resolution despite attempted medical intervention. The patient, despite seeking medical attention and undergoing surgery, experienced persistent nasal obstruction and related symptoms. Clinical examinations and imaging revealed a sizable ACP causing pronounced obstruction within the sinonasal complex, emphasizing the potential impact of these polyps on multiple anatomical regions. Computed tomography scans delineated additional sinonasal pathologies, including sinusitis, nasal concha hypertrophy, and nasal septal deviation. These findings underscore the complexity of ACP cases and the need for comprehensive diagnostic assessments. The patient is currently undergoing pharmacological management with cetirizine and tranexamic acid, aiming to alleviate symptoms. A scheduled Functional Endoscopic Sinus Surgery (FESS) underscores a holistic therapeutic approach to address underlying sinus issues and optimize long-term health. ACPs, though rare, present challenges in diagnosis and management. Swift and accurate diagnosis is crucial to mitigate potential complications, and a nuanced understanding of the interplay between ACPs and chronic sinusitis enhances diagnostic accuracy. The lack of response to previous treatments underscores the necessity for adaptive therapeutic strategies. This case highlights the intricate nature of ACPs, emphasizing the importance of a comprehensive diagnostic approach and a tailored therapeutic strategy. Ongoing research into disease mechanisms contributes to the expeditiousness and accuracy of diagnostic approaches, optimizing the overall management of ACPs and improving patient outcomes.

Abstract

ABSTRACT Chronic suppurative otitis media (CSOM) is a troubling condition that encompasses a range of factors contributing to its etiology. In this context, we present an intriguing and atypical case that adds a novel dimension to the understanding of CSOM. This particular case is marked by its prolonged and treatment-resistant nature, defying conventional medical treatments and prompting us to explore less common etiological pathways. Our case, however, deviates from the norm. It presents a conundrum that led us to investigate an alternative and less explored avenue. During our comprehensive diagnostic evaluation, our attention was drawn to the possibility of a hematogenic infection that may have originated from dental caries. This scenario emerged as the primary candidate for the source of the patient's chronic suppurative otitis media. This intriguing case represents a unique instance of a hematogenic infection stemming from dental caries. The hematogenic pathway, while previously considered uncommon, may be the precipitating factor behind the development of chronic suppurative otitis media in this patient. This perspective widens our understanding of the various potential etiological factors contributing to CSOM and highlights the importance of considering less conventional sources of infection. In the context of otologic disorders, the case we present underscores the necessity of a thorough and holistic diagnostic approach, acknowledging that the origins of such conditions can often be multifaceted and not limited to the conventional causes. Furthermore, this case serves as a reminder for clinicians to maintain a high level of suspicion for atypical etiological pathways when faced with treatment-resistant and persistent manifestations of CSOM. Keywords: Chronic Suppurative Otitis Media, Dental Caries, Odontogenic, Hematogenic

Abstract

Abscess Bezold, pertama kali dijelaskan pada tahun 1881 oleh Dr. Friedrich Bezold, merupakan komplikasi langka namun serius dari otitis media kronis yang terjadi akibat perforasi dan infeksi prosesus mastoideus, umumnya disebabkan oleh bakteri aerob dan anaerob. Kondisi ini ditandai dengan gejala nyeri leher, otalgia, otore, kehilangan pendengaran, dan pembengkakan di sekitar mastoid. Pengenalan dini dan penanganan cepat sangat penting untuk mencegah komplikasi yang mengancam jiwa, seperti mediastinitis dan infeksi intrakranial. Laporan ini memaparkan kasus seorang anak laki-laki berusia 7 tahun yang mengembangkan abses Bezold setelah mengalami riwayat keluarnya cairan telinga kronis dan gangguan pendengaran selama satu tahun akibat trauma dengan cotton bud. Pasien hadir dengan pembengkakan yang membesar dan nyeri di belakang telinga kanan serta leher lateral, disertai otore berbau tidak sedap. Pemeriksaan fisik menunjukkan tanda-tanda peradangan, kerak, nyeri tekan, serta membran timpani yang perforasi. Pemeriksaan computed tomography (CT) menunjukkan destruksi tulang mastoid dengan pembentukan abses yang meluas ke daerah leher. Kasus ini menekankan pentingnya mempertimbangkan abses Bezold sebagai diagnosis diferensial pada pasien otitis media kronis yang disertai pembengkakan dan nyeri leher. Pencitraan, khususnya CT scan, berperan penting dalam memastikan diagnosis dan menilai luasnya infeksi. Drainase bedah dini dikombinasikan dengan terapi antibiotik yang tepat sangat penting untuk mencegah komplikasi berat dan mencapai hasil yang baik.
