Abstract

Abstract

Background: Patient transfer between hospital units, especially from the Emergency Department (ED) to the High Care Unit-Intensive Care Unit (HCU-ICU), carries a high risk of adverse events due to communication failures during handover. The Introduction, Situation, Background, Assessment, Recommendation (ISBAR) checklist is a structured communication tool designed to enhance handover quality but is underutilized in written patient transfer. This study aimed to evaluate the effectiveness of the ISBAR checklist in improving the quality of nurse handovers during ED to HCU-ICU patient transfers. Method: A pre-experimental study with a one-group pre-posttest design was conducted, in which ED nurses used the ISBAR checklist during patient handovers. Forty HCU-ICU nurses assessed the quality of the ED nurse handovers using the Handover Evaluation Scale (HES) questionnaire. A paired sample t-test was used to analyze the differences in handover quality before and after implementation. Result: The ISBAR checklist significantly improved nurses’ handover quality, with a p-value of 0.000 (p < 0,05). The effectiveness of the ISBAR checklist was analyzed using an N-gain score, resulting in a moderate improvement level of 0.34 (0.3 ≤ g ≥ 0.7). Conclusion: The ISBAR checklist is an effective tool for improving nurse handover quality in patient transfers. Hospitals should adopt ISBAR as a standardized handover protocol to enhance patient safety and continuity of care and establish a minimally interruptive environment during handover. Further research should explore long-term implementation and integration into electronic health records.

Abstract

Chronic obstructive pulmonary disease (COPD) remains a significant global health burden. Effective self-management by patients is crucial and involves regular monitoring by healthcare providers, patient education, and a collaboratively agreed-upon written action plan. This study aimed to design and apply a self-management guidebook incorporating a written action plan for individuals with COPD. The study used a pre-experimental design with 9 participants. Data were collected through self-management knowledge questionnaires and evaluations of the guidebook implementation. Results showed a significant increase in self-management knowledge after the intervention (p < 0.05), along with good acceptance from the participants. In conclusion, the written action plan-based guidebook shows promise as a tool to facilitate ongoing care and self-management in COPD patients. Larger-scale studies are recommended to validate these results.

Abstract

Pneumonia is a health problem in the world with a high mortality rate both in developing countries and in developed countries such as America, Canada and European countries. In Indonesia, the number three cause of death after cardiovascular disease and tuberculosis. Low socioeconomic factors increase mortality. This infection is generally spread from someone who is exposed in the neighborhood or has direct contact with infected people through their hands or by breathing air (droplets) due to coughing or sneezing. In the pandemic era, pneumonia is very feared because it is related to respiratory diseases which are becoming a trend and issue, so it requires special treatment in this case. This case report aims to improve the provision of nursing care for pneumonia patients to mantaining hydration status with airway clearance problems. Nursing assessment of pneumonia patients was carried out on September 13, 2020, a 52-year-old man who has a history of DM and smoking comes with complaints of coughing but no phlegm, fever since one week ago has disappeared, the patient has diarrhea since 2 days before entering the house pain and stomach pain, the patient also has nausea and vomiting and has no appetite. Nursing intervention for 3 days in accordance with the established outcome criteria. From the nursing intervension carried out, the problem of clearing the airway was resolved, marked by no coughing, normal breathing, no ronchi, moist mucosa and no cyanosis. Nursing intervention to maintaining the patient's adequate hydration status can overcome the problem of ineffective airway clearance.

Abstract

COPD patients often experience shortness of breath as a result of inflammation in the lungs that develops in the long term. COPD is generally characterized by difficulty breathing, coughing with phlegm, and wheezing or wheezing. In this condition, the patient often finds a comfortable position to reduce shortness of breath. The aim of this study was to look at appropriate and effective interventions in reducing spasms in COPD patients. This literature study was created by analyzing scientific articles, at least in retrospective research from 2012 to 2020 and in English. Data obtained from databases include Pubmed, CINAHL, Scopus, Proquest and Google Scholar with the keywords position for breathing in COPD, position forward lean trunk for short breathing in COPD, and best position for COPD. The results of this literature obtained 5 articles that match the inclusion and exclusion criteria. The results have been found that the forward leaning position is effective in reducing shortness of breath in COPD patients because of the involvement of the respiratory accessory muscles so that the pulmonary ventilation process is good.
