Abstract

Background: Childbirth is the birth of the baby, placenta, and amniotic membranes from the uterus to the outside world. During labor, it can cause trauma to the mother because of the pain experienced. Some mothers are even traumatized to get pregnant and give birth again because they are afraid they will experience the same pain, especially for mothers who have given birth for the first time. Nonpharmacological therapy to reduce pain can be done counterpressure with a combination of birth balls. Objective: To determine the effect of a combination of counter pressure therapy with a birth ball on reducing active phase I labor pain. Method: This research used a quasi experiment with a two group only post test design. The population in this study were all mothers giving birth at the Citra Marendal clinic with a total sample of 36 people, each group consisting of 18 people. The instruments used were Numeric Rating Scale sheets and observations, with the Mann-Whitney Test hypothesis test. Conclusion: There is an effect of a combination of counter pressure therapy with a birth ball on reducing active phase I labor pain at the Citra Marendal Delitua Clinic with a p value = 0.001 <0.05. Suggestion: For the future, it is hoped that they can develop research by examining various complementary methods that can reduce labor pain such as: the effectiveness of music therapy, aroma therapy, back massage, gym ball/birth ball techniques, relaxation techniques, remembrance, guided imagery.

Abstract

Budaya keselamatan pasien diterapkan di Puskesmas sebagai salah satu upaya untuk meminimalkan timbulnya risiko dan mencegah terjadinya cedera yang disebabkan oleh kesalahan akibat melaksanakan suatu tindakan atau tidak mengambil tindakan yang seharusnya diambil. Tujuan: untuk menganalisis implementasi budaya keselamatan pasien Puskesmas terhadap standar keselamatan pasien di Indonesia. Pencarian database melalui google scholar, pubmedportal garuda dengan kata kunci budaya keselamatan pasien, fasilitas Kesehatan primer, puskesmas, Indonesia. Kriteria artikel yang dipilih yaitu terbit tahun 2014 sampai 2024, full text, free acces, berbahasa Indonesia atau Bahasa Inggris. Systematic review ini menggunakan 15 artikel yang sesuai dengan kriteria. Analisis dari 15 artikel menunjukkan masih ada Puskesmas yang belum memenuhi 7 langkah keselamatan pasien yang terdapat didalam standar keselamatan pasien. Implementasi budaya keselamatan pasien Puskesmas di Indonesia ada yang sudah optimal dan ada yang belum optimal dalam memenuhi standar keselamatan pasien. Budaya keselamatan pasien yang optimal dalam pelaksanaannya dapat mencegah insiden keselamatan pasien sehingga meningkatkan kesehatan pasien yang berujung pada peningkatan kepuasan pasien serta akuntabilitas Puskesmas.

Abstract

Prevention of diabetes mellitus complications can be done through joint movement exercises, which can be effective, efficient, and minimal risk through Buerger Allen Exercise. ABI describes the severity of peripheral arterial disease in DM patients. ABI measurements are carried out on systolic arm and systolic leg pressures. Buerger Allen Exercise is able to improve peripheral circulation of the lower extremities. This training activity aims to educate and train people at the Kabu level so that they can apply Buerger Allen Exercise with increasing knowledge. Training activities involve a series of processes through providing materials, demonstrations, training, and questions and answers. PkM participants are people from the Aras Kabu sub-district represented by 20 participants. The instruments used include Teaching Aids, Laptops, Infocus, SOPs. The increase in knowledge and skills of PkM participants is measured based on the increase in post-test scores compared to pre-test scores. Buerger Allen Method Training has been proven to be able to reduce ABI rates to the mild category by 60%, which was initially only 35%. In addition, PkM participants have understood the theory of Buerger Allen, ABI, and its benefits for type 2 DM patients. PKM participants are also able to apply Buerger Allen exercises according to SOP. PkM training activities provide benefits in the form of increasing the knowledge and skills of PkM participants, which is confirmed through the post-test knowledge value being categorized as good and very good.

Abstract

Asthma is a respiratory disorder characterized by inflammation, airway narrowing, and excessive mucus production, which leads to shortness of breath. A commonly applied therapy during acute asthma attacks is the use of a nebulizer, which delivers medication in the form of vapor inhaled directly into the airways. The effectiveness of nebulizer therapy is influenced by the physiological condition of the patient’s airway prior to the treatment. Warm water consumption has been shown to support respiratory function by maintaining mucosal hydration and softening mucus secretions. This community service program aimed to educate the public on the potential benefits of warm water consumption in enhancing the effectiveness of nebulizer therapy in relieving airway obstruction among asthma patients. The program was implemented with 25 participants, all of whom were asthma patients at Grandmed Hospital in Lubuk Pakam. The educational activity included delivering the intervention material, followed by interactive sessions such as Q & A and group discussions. Knowledge improvement was assessed by comparing post-test scores with pre-test scores. The results demonstrated a notable increase in participants’ knowledge, with an average score improvement of 34.1 points or 64.6%. Additionally, approximately 98% of participants reported that the educational session was beneficial and easy to understand. Participants showed improved comprehension of nebulizer mechanisms and recognized the importance of warm water intake prior to therapy. The educational intervention successfully enhanced participants understanding and encouraged the application of warm water consumption before nebulizer therapy, potentially leading to more effective treatment outcomes.

Abstract

Background: Patients in Intensive Care The intensive care unit (ICU) is at high risk for sudden deterioration. Early detection of changes in clinical status is crucial for improving patient outcomes. Modified Early Warning Score (MEWS) is a simple scoring system based on vital signs that is used to predict the likelihood of patient deterioration. Objective: This study aims to evaluate the effectiveness of using the MEWS score in early detection of worsening patient conditions in the ICU. Methods: The study used an observational quantitative design with a prospective cohort approach. The study sample was 60 ICU patients at Hospital “X” who were assessed using the MEWS score during 7 days of treatment. The observed variables were the MEWS score and the incidence of deterioration (increased need for mechanical ventilation, higher ICU transfer, or death). Analysis was performed using sensitivity, specificity, and logistic regression tests. Results: A MEWS score ≥5 had a sensitivity of 85% and a specificity of 78% in detecting patient deterioration. Regression analysis showed that the MEWS score significantly predicted the incidence of deterioration (p < 0.01). Conclusion: The MEWS score has been shown to be effective as an early detection tool for patient deterioration in the ICU. Routine use of this score can help nurses and physicians intervene more quickly.
