Abstract

According to WHO, hospitals provide comprehensive curative and preventive services, including quality medical record (MR) management, assessed by completeness, accuracy, timeliness, and legal compliance. This study aimed to analyze factors related to incomplete MR documentation at Sibuhuan Regional Hospital in 2022 using an analytical observational cross-sectional design with 74 health workers as total respondents. Data were analyzed using binary logistic regression. Results showed that supervision, training, and sanctions were significantly related to incomplete MR filling (p<0.05), while knowledge, attitude, and length of work were not (p>0.05). Sanctions were the most dominant factor. It is recommended that the hospital conducts monthly audits to improve MR documentation completeness

Abstract

Hypertension is also known as "the silent killer" because hypertension sufferers often go for years without experiencing any disturbances or symptoms and thus experience complications in vital organs. Hypertension is still a public health problem today because its prevalence is increasing. The aim of this research is to analyze the relationship between obesity and the incidence of hypertension at the Medan Tembung Mandala Health Center in 2023. The type of research used is analytical descriptive with a cross sectional study approach. The sample in this study was 92 people according to the inclusion and exclusion criteria using a purposive sampling technique. Data processing in this research is processed by cleaning, editing, coding and data entry. Data processing and data analysis were carried out using univariate analysis to describe the characteristics of the frequency distribution of respondents, and bivariate analysis using the chi-square test. The research results show that the P-value is 0.002, meaning there is a significant relationship between obesity and hypertension, and it is known that OR = 3.974, which means that people who are obese are 3.9 times more likely to suffer from hypertension compared to people who are not obese. It is recommended that patients need to maintain their diet, namely by reducing junk food and consumption of high sugar and high cholesterol foods, patients also need to understand about hypertension so they can make further prevention efforts, namely preventing heart failure complications.

This study aims to determine the effect of playing plasticine on the level of anxiety due to hospitalization in children aged 3-6 years at RSUD DR RM Djoelham Binjai. This type of research is a quantitative research with a Quasi-Experimental method and uses a pretestposttest design with a control group. The sample in this study was 30 people, collecting data using a direct questionnaire. Sampling used the slovin formula technique, the normality test used the Shapiro-Wilk and data analysis used the paired t-test. The results of the normality test before being given education were 0.938 and Sig. .082 after education 0.897 and Sig. 0.007 which is normally distributed. The results of the study were obtained before being given plasticine play therapy, the level of severe anxiety was 15 children (50.0%). And after being given plasticine play therapy in children, the level of severe anxiety decreased to 6 children (20.0%). The results of the paired ttest obtained a significance value obtained from the calculation results, namely p value 0.023.
