Abstract

Background: Bronchial asthma is a chronic inflammatory airway disease characterized by bronchial narrowing and hyperresponsiveness, leading to episodic dyspnea, wheezing, chest tightness, and coughing. In addition to pharmacological therapy, non-pharmacological interventions such as the Tripod Position and Diaphragmatic Breathing Exercise may improve respiratory status in acute exacerbations. Purpose: To evaluate the effect of combining Tripod Position and Diaphragmatic Breathing Exercise on respiratory status in a patient with acute bronchial asthma. Method: A descriptive case study using a nursing approach was conducted. Data collection included interviews, observation, physical examination, literature review, and documentation analysis. Results: The patient presented with dyspnea, respiratory rate of 28 breaths/min, SpO₂ 88%, tachypnea, nasal flaring, and wheezing. The primary nursing diagnosis was ineffective airway clearance. After 15 minutes of combined Tripod Position and Diaphragmatic Breathing Exercise, respiratory rate decreased to 26 breaths/min, SpO₂ improved to 92%, and dyspnea severity reduced to a score of 3. Conclusion: The combination of Tripod Position and Diaphragmatic Breathing Exercise improved respiratory parameters in acute bronchial asthma. This non-pharmacological intervention may serve as an adjunct to pharmacological therapy in nursing practice. Pendahuluan: Asma bronkial merupakan penyakit inflamasi kronis pada saluran napas yang ditandai dengan penyempitan dan hiperresponsivitas bronkus, sehingga menimbulkan gejala episodik berupa sesak napas, mengi, rasa berat di dada, dan batuk. Selain terapi farmakologis, intervensi non-farmakologis seperti Posisi Tripod dan Latihan Pernapasan Diafragma dapat membantu memperbaiki status respirasi pada kondisi eksaserbasi akut. Tujuan: Untuk Mengevaluasi efek kombinasi posisi tripod dan latihan pernapasan diafragma terhadap status respirasi pada pasien dengan asma bronkial akut. Metode: Penelitian menggunakan desain studi kasus deskriptif dengan pendekatan keperawatan. Pengumpulan data dilakukan melalui wawancara, observasi, pemeriksaan fisik, kajian literatur, dan studi dokumentasi. Hasil: Pasien dengan asma bronkial akut menunjukkan sesak napas, frekuensi napas 28 kali/menit, SpO₂ 88%, takipnea, napas cuping hidung, serta bunyi tambahan berupa mengi. Diagnosis keperawatan utama adalah bersihan jalan napas tidak efektif. Setelah intervensi kombinasi Posisi Tripod dan Latihan Pernapasan Diafragma selama 15 menit, frekuensi napas menurun menjadi 26 kali/menit, SpO₂ meningkat menjadi 92%, dan tingkat sesak napas menurun menjadi skor 3 (sedang). Simpulan: Kombinasi Posisi Tripod dan Latihan Pernapasan Diafragma terbukti memperbaiki parameter respirasi pada pasien dengan asma bronkial akut. Intervensi non-farmakologis ini dapat dijadikan terapi tambahan yang mendukung manajemen farmakologis dalam praktik keperawatan Kata Kunci: Asma Bronkial Akut; Diaphragmatic Breathing Exercise; Tripod Position Status pernafasan.

Abstract

Background: The implementation of the Indonesian Nursing Diagnosis Standards (IDHS) in clinical practice continues to encounter obstacles, including limited time for assessment, documentation burden, and inconsistency in diagnostic interpretation among nurses. Advancements in artificial intelligence (AI) offer opportunities to design clinical decision support systems that may enhance the accuracy, uniformity, and efficiency of nursing diagnoses. Objective: This study aimed to explore nurses’ experiences, challenges, and expectations regarding the use of IDHS and to assess the initial feasibility of developing an AI-based system to support standardized nursing diagnoses. Methods: A descriptive qualitative design was employed using in-depth interviews with five nurses from diverse clinical settings, including emergency, critical care, inpatient, outpatient, and community services. Thematic analysis was conducted to identify perceptions and needs related to IDHS and AI. Content validity of the preliminary system design was evaluated by three expert validators using the Item-Level Content Validity Index (I-CVI) and the Scale-Level Content Validity Index/Average (S-CVI/Ave). Results: Five major themes emerged: (1) varied experiences in formulating nursing diagnoses, (2) inconsistent use of IDHS in practice, (3) challenges related to documentation and diagnostic interpretation, (4) positive attitudes toward AI integration in clinical workflows, and (5) the need for features such as automated diagnostic suggestions, clarification of clinical criteria, and integration with electronic medical records. Content validity testing demonstrated strong agreement among experts, with I-CVI values ranging from 0.80 to 1.00 and an S-CVI/Ave of 0.92. Conclusion: The study indicates that integrating AI into the IDHS-based diagnostic process holds substantial potential to improve diagnostic quality, standardization, and clinical efficiency. The content validity results support further development and prototype testing in subsequent research phases
