Abstract

One cardiovascular condition known as venous thromboembolism (VTE) is believed to be brought on by hormonal medications like oral contraceptives. Women taking oral contraceptives are more likely to develop VTE over the age of 40 due to a genetic predisposition that can trigger the risk from the cytochrome gene. One of the genes associated with VTE and contraception is the CYP2C19, CYP3A4, CYP1A2, and CYP345 genes. Methods: A quality assessment of the method and risk was conducted using The Joanna Briggs Institute (JBI) Instrument after a number of articles collected from Scopus, Crossref, Ebsco, Pubmed, Science Direct, and Google Scholar were selected by the inclusion criteria. Results:Articles or journals on the JBI instrument showed good quality in the number of 10 research particles. From the results of the journal analysis, several cytochrome genes were obtained, namely the CYP2C19, CYP3A4, CYP1A2, and CYP345 genes. Conclusion: Several types of cytochrome genes that affect oral use as well as VTE. The cytochrome CYP2C19 gene may decrease the effects of clopidogrel. The use of CYP3A4 gene-inducing drugs should be considered at the same time as contraception and it is advisable to use other drug alternatives. Concomitant use of CYP1A2 gene-inducing drugs with contraception should be avoided because they may decrease the contraceptive effect. The use of cyclosporine drugs needs to be monitored because cyclosporine can increase estrogen levels in the liver.

Abstract

Personal hygiene, especially nail hygiene, is a basic need that should be cultivated from an early age. Neglecting this can lead to a decline in awareness of personal hygiene and increase the risk of infectious diseases later in life. Kindergarten students, aged 4-5 years, are not fully aware of this. The methods employed include socialization; initial discussion and situational analysis; material preparation; questionnaire validity and reliability testing; socialization of the activity plan; activity implementation; monitoring and evaluation, and discussion. In this case, the educational module includes instructional materials, educational games as visual aids, and mini experiments. The counseling activity was conducted by providing instructional materials on proper handwashing followed by a mini handwashing experiment. After this activity, it continued with an educational material on nail cutting and its experiment. Based on the average student scores before and after the counseling, there was an increase in the average student score from 66 to 80 (p-value = 0.002*). Thus, this counseling has a significant impact on increasing students' knowledge and average scores.

Abstract

Background: Juvenile rheumatoid arthritis, now more commonly known as juvenile idiopathic arthritis (JIA), is a chronic form of arthritis that occurs in children under 16 years of age. It is characterized by persistent inflammation in one or more joints for at least six weeks without an identifiable cause. Objective: Evaluate the clinical condition of a patient with polyarticular JIA who has received methotrexate therapy, and to assess the effectiveness of intra-articular triamcinolone acetonide as an adjunct treatment. Methods: This study based on a case illustration of a 4-year-old boy diagnosed with polyarticular JIA. Data were obtained through medical history, physical examination, methotrexate treatment records, and planned intra-articular injection procedures. A literature review was also conducted to evaluate the effectiveness of triamcinolone acetonide as adjunct therapy in polyarthritis. Results: The 4-year-old patient, weighing 18 kg and measuring 109 cm in height, had experienced symptoms for the past two years. Despite methotrexate therapy—gradually increased from 10 mg/m² to 15 mg/m² per week since December 2020—he continued to present with swelling in the hands and feet, stiffness in both knees and elbows, and pain that limited joint extension. Literature indicates that intra-articular triamcinolone acetonide demonstrates approximately 70% effectiveness in managing symptoms in patients with polyarticular JIA. Conclusions: JIA is a chronic inflammatory disease that requires comprehensive management. In this case, methotrexate alone did not fully alleviate symptoms, making additional therapy necessary. Intra-articular triamcinolone acetonide is effective and may serve as a viable option to improve joint function and reduce inflammation in polyarticular JIA.
