Abstract

The important role of aeroallergens and food allergens as the most common environmental allergens in exacerbationof atopic dermatitis (AD) iswell known. Sensitization is an essential processwhich correlateswith clinicalmanifestationof AD. The study of AD in Indonesia, especially in Yogyakarta, has not been reported, yet. The aim of study is toevaluate the association between sensitization of environmental allergens with clinical severity of AD in children andyoung adult at Dr. Sardjito General Hospital, Yogyakarta. An analytic observational cross sectional study wasperformed on 33 children and young adult with AD. The severity of disease was determined by SCORing AtopicDermatitis (SCORAD). Assessment of sensitization was performed using specific IgE serum, atopy patch test, andprick test. Data were analyzed using chi-square or Fisher exact test and prevalence ratio (PR) with significancevalue of p<0.05 and 95%confidence interval (CI). The results showed that specific IgE positivity was associatedwith severity of AD. Percentage of specific IgE positivity to house dust mite was significantly higher in subject withmoderate/severe AD compared to those of mild AD (p=0.049; PR 1.13; 95% CI 1.01-1.59). The result was alsosimilar for cat dander (p=0.041; PR 1.1; 95%CI 1.09-4.98), cow’s milk (p=0.038; PR 1.21; 95% CI 1.02-2.2),and egg white (p=0.027; PR 1.23; 95% CI 1.15-2.97). Whereas specific IgE positivity to fish allergen was notstatistically different in subject with moderate/severe AD compared to those with mild AD (p=0.061; PR 0.8; 95%CI 0.76-2.8). According to atopy patch test and prick test result, no association was found between allergensensitization and severity of AD. If allmethodswere combined to increase the sensitivity of sensitizationmeasurement,then the association was found for all allergens. It could be concluded that environmental allergens sensitization isassociated with severity of AD in children and young adult at Dr. Sardjito General Hospital, Yogyakarta.Key words: atopic dermatitis-disease severity-sensitization-aeroallergen-food allergen

Abstract

Abstrak Latar Belakang: Psoriasis merupakan gangguan inflamasi sistemik kronis dan rekuren yang mempengaruhi 2-3% populasi. Patogenesis psoriasis melibatkan neutrofil, limfosit, trombosit, sitokin dan kemokin. Penentuan derajat keparahan psoriasis tipe plak yang sering digunakan adalah Psoriasis Area Severity Index (PASI) memiliki kelemahan yaitu subyektivitas antar pemeriksa. Rasio neutrofil limfosit (N/L) dan rasio trombosit limfosit (T/L) adalah penanda sederhana respon inflamasi sistemik yang mudah diukur dan murah sebagai bagian dari pemeriksaan darah rutin dan umum digunakan dalam penyakit inflamasi kronis. Tujuan: mengetahui perbedaan dan hubungan antara rasio N/L dan rasio T/L dengan derajat keparahan psoriasis tipe plak berdasarkan PASI. Metode: Penelitian menggunakan rancangan potong lintang dengan subyek telah memenuhi kriteria inklusi dan eksklusi. Analisis komparatif dan korelatif digunakan untuk menilai perbedaan dan hubungan kedua rasio dengan skor PASI. Hasil: Jumlah subyek penelitian 48 orang dibagi menjadi 3 kelompok psoriasis (derajat ringan, sedang dan berat). Terdapat perbedaan antara rasio N/L dan rasio T/L dengan derajat keparahan psoriasis tipe plak (p<0,05). Terdapat hubungan antara rasio N/L dan rasio T/L dengan derajat keparahan psoriasis tipe plak berdasarkan PASI (p<0,05 dan r: 0,550, r: 0,314). Kesimpulan: terdapat perbedaan dan hubungan antara rasio N/L dan rasio T/L dengan derajat keparahan psoriasis tipe plak berdasarkan PASI. Abstract

Background: This paper reports a segmental Stiff Skin Syndrome (SSS) case in a four-year-old girl.  SSS is a rare disease characterized by skin hardening and joint stiffness due to a mutation of the fibrillin-1 (FBN-1) gene encoding the fibrillin protein. The effective therapies for this disease are limited. Case Report: The patient presented with hardened skin and a limping gait.  Clinically, there were hard, hyperpigmented patches with hypertrichosis on the skin of the left femur and left gluteal. The left coxae and left genu joint were in fixed flexion, causing a limping gait. The SSS histology revealed thickened collagen fibers, trapping adipocytes between them, but no inflammatory cells.  The UV-A phototherapy and physiotherapy session resulted in a modest improvement. Discussion: This case's segmental SSS diagnosis was based on clinical and histological findings. Establishing a diagnosis of SSS is a challenge for clinicians because it can resemble other skin disorders, particularly sclerosing diseases. The recommended management for SSS remains limited. In this case, after the UV-A phototherapy and physiotherapy, complaints of hardened skin were said to soften.

Abstract

Vascular anomalies are classified into vascular tumors and vascular malformations. Hemangiolymphangioma (HLA) is a rare vascular malformation that contains both blood and lymphatic components, most commonly diagnosed in infancy or early childhood. Adult cases, especially in atypical locations such as the medial thigh, are exceedingly rare and pose diagnostic challenges. A 27 yo female presented with a slowly enlarging, painless lump on the left medial thigh. Initially misdiagnosed as condyloma acuminata, the lesion was subsequently identified as a vascular malformation through dermoscopic visualization of characteristic vascular lacunae. Histopathological and immunohistochemical analyses confirmed the diagnosis of HLA. Surgical excision was performed with clear margins, and no recurrence was observed after 3 mo. Hemangiolymphangiomas are benign but may exhibit local infiltration and recurrence, especially after incomplete resection. Diagnosis requires a multimodal approach including clinical assessment, dermoscopy, histopathology, and immunohistochemistry. Differential diagnosis includes hemangioma, lymphangioma, and malignancies such as lymphangiosarcoma. Complete surgical excision remains the treatment of choice, with other modalities like electrocautery or cryotherapy considered in selected cases. Long-term follow-up is crucial due to the risk of recurrence. In conclusion, this rare adult case of medial thigh HLA highlights the importance of considering vascular malformations in atypical anatomical sites. Early recognition and comprehensive diagnostic evaluation facilitate appropriate management and improve patient outcomes

Abstract

Background: Radiotherapy is an essential treatment in reducing the risk of breast cancer but can cause side effects such as acute radiodermatitis. This condition is the result of ionizing radiation damaging deoxyribonucleic acid (DNA) through reactive oxygen species (ROS) and inducing inflammatory responses, which lead to variable degrees of skin damage. Hyaluronic acid (HA) plays a role in triggering cell proliferation and keratinocyte differentiation while inhibiting lipid peroxidation caused by oxidative stress. Purpose: This study aims to determine the effectiveness of hyaluronic acid in reducing the degree of acute radiodermatitis using the Radiation Therapy Oncology Group (RTOG) score in breast cancer patients undergoing radiotherapy. Methods: This study was a double-blind randomized controlled trial. The samples were from patients diagnosed with breast cancer receiving radiotherapy. The treatment group received 0.2% HA cream, while the control group received a placebo The clinical appearance was evaluated weekly from the beginning of radiotherapy until two weeks post-radiotherapy using the RTOG score. Result: A total of 41 subjects were divided into two groups. RTOG score evaluation with intention-to-treat analysis and per-protocol analysis at weeks 3, 4, and 5 showed that the hyaluronic acid cream group experienced delayed onset of acute radiodermatitis compared to the placebo group. The log-rank test showed a significant difference in effectiveness between 0.2% HA cream compared to base cream in reducing the degree of acute radiodermatitis (p=0.035). Conclusion: The use of 0.2% HA cream was effective in reducing the severity of acute radiodermatitis in breast cancer patients undergoing radiotherapy.
