Abstract

Introduction: The choice of therapy for cervical cancer strongly relies on the cancer stage. Stage IIB cervical cancer entails radiation therapy and chemotherapy, excluding surgery due to the risk of serious complications. This case is particularly interesting for discussion as the patient underwent a radical hysterectomy, bilateral salpingo-oophorectomy, and pelvic lymph node dissection following chemoradiation, resulting in a partial response.Case Presentation: A 56-year-old woman presented with a complaint of continuous, progressive clear, and odorless fluid discharge from her birth canal, diagnosed with cervical cancer grade IIB. The patient underwent three cycles of cisplatin chemotherapy and radiation at a total dosage of 70.4 Gy over 38 sessions. Post-chemoradiation evaluation revealed a residual tumor with a diameter of 2 cm, and the patient underwent a radical hysterectomy, bilateral salpingooophorectomy, and pelvic lymph node dissection.Conclusion: Surgery post-chemoradiation for cervical cancer in centers that don’t have any brachytherapy can be considered as an option, yielding positive effects with minimal side effects. Healthcare centers with limitations in brachytherapy can utilize EBRT boosters, which may be continued with surgery depending on the patient’s response to the earlier therapy.

Abstract

Abstrak Kanker payudara merupakan penyakit keganasan yang disebabkan oleh pertumbuhan sel-sel abnormal di jaringan payudara. Protein Ki67 terdapat di inti sel dan berkaitan dengan proliferasi dan prognosis sel. Mitosis adalah pembelahan sel yang diatur oleh gen dalam sel. Indeks mitosis digunakan untuk menilai persentase sel tumor yang sedang membelah. Sebanyak 50 sampel data hasil pemeriksaan pasien kanker payudara diambil dari populasi 100 pasien RSUD Abdul Wahab Sjahranie tahun 2022 menggunakan teknik purposive sampling. Jenis penelitian observasional analitik dengan pendekatan cross sectional. Analisis data dengan uji chi square. Hasil penelitian ditemukan mayoritas sebanyak 35 pasien (70,0%) pada usia 41–60 tahun menderita kanker payudara. Terdapat 4 pasien (8,0%) memiliki ekspresi Ki67 rendah (low expression), 22 pasien (44,0%) memiliki ekspresi Ki-67 sedang (intermediate expression), dan 24 pasien (48,0%) memiliki ekspresi Ki67 tinggi (high expression). Sehubungan dengan penilaian indeks mitosis, terdapat 9 pasien (18,0%) dengan skor 1,17 pasien (34,0%) dengan skor 2 dan 24 pasien (48,0%) dengan skor 3. Hasil analisis statistik menunjukkan hubungan yang signifikan antara ekspresi Ki67 dan indeks mitosis dengan nilai p value (0,014) < α (0,05). Ki67 dan indeks mitosis berkorelasi dengan prognosis kanker payudara, ketika sel membelah protein Ki67 yang terikat mengekspresikan selnya. Kata kunci: ekspresi Ki67, indeks mitosis, kanker payudara Abstract Breast cancer is a malignant disease characterized by abnormal cell growth in breast tissue. The Ki67 protein, found in the cell nucleus, is associated with cell proliferation and prognosis. Mitosis is the regulated division of cells controlled by genes. The mitotic index is used to assess the percentage of dividing tumor cells. A total of 50 samples of breast cancer patient examination data were taken from a population of 100 patients at Abdul Wahab Sjahranie Hospital in 2022 using purposive sampling techniques. This observational research employed a cross-sectional approach and analyzed the data using the chi-square test. The findings revealed that the majority of 35 patients (70.0%) with breast cancer were aged 41-60 years. Among them, 4 patients (8.0%) exhibited low Ki67 expression, 22 patients (44.0%) had intermediate Ki67 expression, and 24 patients (48.0%) showed high Ki67 expression. Regarding the mitotic index assessment, 9 patients (18.0%) scored 1, 17 patients (34.0%) scored 2 and 24 patients (48.0%) scored 3. The results of statistical analysis showed a significant relationship between Ki67 expression and mitotic index with p value (0.014) < α (0.05). Ki67 and mitotic index correlate with breast cancer prognosis, when a cell cleaves its cell-expressing bound Ki67 protein. Keywords: expression of Ki67, mitotic index, breast cancer
