Abstract

Enhanced Recovery After Surgery (ERAS) is a multimodal protocol applied to perioperative patient care. The ERAS program is implemented by a multidisciplinary, patient-focused team. The application of ERAS in the field of spinal surgery and the literature that discusses it is currently not widely available. The aim of this review article is to discuss the application of ERAS in spine surgery. ERAS in spine surgery has several components ranging from pre-hospital, preoperative, intraoperative, postoperative, post-discharge phases, to continuous activity improvement teams. The methods used include preoperative education and multimodal management of pain and anxiety, multimodal analgesia, blood loss management, the total parenteral nutrition, and early rehabilitation. The ERAS protocol in spine surgery consists of several components that are interconnected with each other. The application of the ERAS protocol to spine surgery focuses on reducing pain, length of stay and morbidity, and reducing the preoperative anxiety.

Abstract

Penuaan kulit merupakan proses yang dipengaruhi oleh faktor intrinsik dan ekstrinsik. Merokok, sebagai faktor ekstrinsik, diketahui dapat mempercepat penuaan kulit melalui penurunan produksi kolagen dan peningkatan aktivitas matriks metalloproteinase (MMP). Penelitian ini bertujuan untuk menganalisis hubungan antara kebiasaan merokok dan tingkat keparahan kerutan wajah pada karyawan kantor di Universitas Sebelas Maret. Penelitian ini merupakan studi observasional analitik dengan desain cross-sectional yang melibatkan 68 laki-laki perokok aktif berusia 30–39 tahun. Kebiasaan merokok dikategorikan berdasarkan pack-years menjadi perokok ringan, sedang, dan berat. Penilaian kerutan wajah dilakukan dengan Daniell’s 6-point wrinkle scale oleh dua dokter spesialis kulit. Analisis hubungan dilakukan dengan uji korelasi Spearman (p<0,05). Hasil menunjukan sebanyak 45,6% subjek tergolong perokok ringan, 39,7% perokok sedang, dan 14,7% perokok berat. Penilaian menunjukkan 70,6% memiliki kerutan ringan, 25% kerutan sedang, dan 4,4% kerutan berat. Terdapat korelasi signifikan antara kebiasaan merokok dan keparahan kerutan wajah (p=0,001; P=0,474). Terdapat hubungan positif yang signifikan antara kebiasaan merokok dan tingkat keparahan kerutan wajah. Hasil ini memperkuat bukti bahwa merokok mempercepat penuaan kulit, khususnya dalam pembentukan kerutan.

Abstract

Background: Brachytherapy is radiation therapy that will cause inflammation and tissue damage that leads to inflammation, characterized by the release of pro-inflammatory cytokines, including hs-CRP levels in the plasma. Preoperative administration of glucose solution is known to reduce the increase in cortisol, and is thought to reduce postoperative hs-CRP levels. So far there has been no research that specifically examines the effect of preoperative carbohydrate administration on hs-CRP values after brachytherapy. This study aims to determine the effect of preoperative carbohydrate administration on post-brachytherapy hs-CRP values. Subjects and Method: This study was designed a double-blind randomized controlled trial was conducted at the Radiotherapy Unit of Dr. Moewardi General Hospital, Surakarta, from April to September 2022. Sixty-four cervical cancer patients with ASA physical status I or II undergoing brachytherapy under general anesthesia were recruited using consecutive sampling. Participants were randomly assigned into two groups: an intervention group receiving 12.5% maltodextrin solution and a control group receiving mineral water. The independent variable was preoperative carbohydrate administration, and the dependent variable was hs-CRP level measured 2 hours before and 4 hours after brachytherapy. Data were analyzed using paired t-test Results: Post-brachytherapy hs-CRP levels were significantly lower in the treatment group (mean = 2.565 ± 2.0 mg/dL) compared to the control group (mean = 3.69 ± 2.58 mg/dL; p = 0.019). Both groups showed significant increases in hs-CRP from pre- to postoperative values (p < 0.001). The change in hs-CRP (Δhs-CRP) was also significantly lower in the treatment group (p = 0.025). Conclusion: The administration of maltodextrin was able to significantly reduce post-brachytherapy hs-CRP levels compared to placebo.

Abstract

Breast cancer remains one of the leading causes of cancer-related mortality among women worldwide. Early and accurate diagnosis is crucial for improving survival rates, particularly in regions with limited access to advanced diagnostic facilities. The ultrasound (USG) examination with Breast Imaging Reporting and Data System (BI-RADS) classification standardizes breast imaging interpretation and aids in risk stratification. However, its correlation with histopathological outcomes requires further evaluation. This study aims to analyze the correlation between ultrasonographic BI-RADS classifications and histopathological findings in patients with breast tumors in Gorontalo. This study is a multicentre retrospective observational analytic with a cross-sectional approach conducted at four hospitals in Gorontalo, Indonesia. The study included patients who underwent breast ultrasonography with BI-RADS categories 3, 4, or 5, followed by histopathological confirmation between January 2023 and January 2025. The results show 75 patients, 25 (33.3%) were classified as BI-RADS category 3, 26 (34.7%) as BI-RADS category 4, and 24 (32%) as BI-RADS category 5. Histopathological examination revealed that 92% of BI-RADS 3, 53.8% of BI-RADS 4, and 8.4% of BI-RADS 5 cases were benign, while 8% of BI-RADS category 3, 46.2% of BI-RADS category 4, and 91.6% of BI-RADS category 5 were confirmed as malignant. The chi-squared test demonstrated a significant correlation between BI-RADS classifications and histopathological findings (p < 0.001). Thus, the study confirms the strong correlation between ultrasonographic BI-RADS classifications and histopathological outcomes, emphasizing its reliability in breast cancer evaluation.
