Abstract

The increasing burden of cataract-induced blindness in Indonesia demands competent surgical responses, particularly through the phacoemulsification technique—a minimally invasive surgical procedure globally recognized as the gold standard. However, structural limitations in training policies, unequal access to resources, and political dynamics impede the readiness of new ophthalmologists in mastering this technique. This study explores the systemic, policy, and political contexts underlying phacoemulsification training in Indonesia. Using a qualitative descriptive approach supplemented by a literature review, this research analyzes dimensions of curriculum development, accreditation standards, infrastructure distribution, stakeholder politics, and professional ethics compliance. Findings indicate that to ensure equitable surgical competence among graduates of ophthalmology specialist programs, policymakers need to integrate national regulations with international standards, decentralize access to quality training, and enforce ethical governance across all medical education institutions.

Abstract

Background Lens disorders in pediatrics include cataracts and abnormalities in the shape, size, and location of the lens. These abnormalities are a significant source of visual impairment in children. Cataracts are responsible for nearly 10% of all childhood vision loss worldwide. Cataracts in children cause over one million cases of Blindness among Asian children. The purpose of this study is to analyze the differences in axial length (AL)—corneal curvature, and best-corrected visual acuity (BCVA) before and after surgery in pediatric cataract eyes. Methods This study used a comparative experimental analytical research design with a Pretest-Posttest on a single group without control, utilizing ocular biometry instruments and BCVA examination in pediatric cataract eyes before and after surgery. Results The BCVA and AL in 8 children showed significant differences. The average BCVA with logMAR decreased from 1.98 to 1.36 one month after surgery, with a paired t-test yielding a t-value of 2.595 and p-value of 0.018 < 0.05. Moreover, the initial BCVA average of 1.98 improved to 0.79 three months after surgery, with a t-value of 5.574 and a p-value of 0.001 (< 0.05). As for the AL, it increased from 22.21 mm to 22.57 mm one month after surgery, with a t-value of -2.475 and p-value of 0.021 < 0.05. Furthermore, the initial AL average of 22.21 mm increased to 22.75 mm three months after surgery, with a t-value of -4.375 and p-value of 0.003 < 0.05. Conclusions There were significant differences in BCVA and AL in the sample before pediatric cataract surgery, as well as one and three months after the surgery.

Abstract

Introduction : Eyelid lacerations refer to partial or full-thickness defects in the eyelid and constitute a significant subset of facial trauma which is often accompanied by other injuries including foreign bodies. Case Illustration : A 16-year-old boy was brought to the emergency department with a laceration and penetrating orbital injury in his right upper eyelid. On examination, there was a 4 cm x 0,5 cm upper eyelid laceration caused by tooth penetrating, CT scan revealed a high density foreign body measuring 1,5 cm x 0,5 cm in the right upper eyelid as deep as levator muscle and no globe injury. The patient underwent an emergency operation under general anesthesia, the foreign body was removed completely and the size of the foreign body was 2,6cm x 0,5cm. After the removal of the foreign body, debridement was performed and then sutured layer by layer. After surgery, he was treated with Tetanus prophylaxis, antibiotic, and analgetic. Discussion : Eyelid laceration is one of the most encountered ocular trauma, but needed to be evaluated and treated carefully. Although the rate of ocular foreign bodies found was low, it can lead to devastating ocular morbidities. After surgery and treatment, there is no other complaint from the patient. Visual acuity and ophthalmology examination are all in normal condition. ConclusionEyelid trauma with foreign bodies must undergo a complete examination. A good prognosis and outcomes can be achieved when appropriate medical and surgical treatment is performed.
