Abstract

Abstract: One of the complications of hypertension is hypertensive retinopathy with a prevalence of 2-17% in patients with hypertension without diabetes. Application of optical coherence tomography (OCT) tool enables the clinician to simplify the pathophysiological process of hypertensive retinopathy. This study was aimed to compare the central retina thickness of essential hypertensive patients without decreased vision to of normal people. This was a prospective comparative study using a quantitative method. There were 56 eyes in the hypertension group and 71 eyes in the normal group. The t test showed that central retina thickness in hypertension group was 201.66 μm (SD ±38,870) meanwhile in the normal group was 249.59 μm (SD±19,245) (p=0.004). In conclusion, there was a significant difference in central retinal thickness between essential hypertensive patients and normal people. It is suggested that ophthalmologic examination should be performed on patients with essential hypertension even without decreased vision.Keywords: hypertension, decrease of vision, retinal thickness Abstrak: Salah satu komplikasi hipertensi yaitu retinopati hipertensi dengan prevalensi 2-17% pada penyandang hipertensi tanpa diabetes. Aplikasi alat optical coherence tomography (OCT) memungkinkan klinisi untuk meneliti lebih lanjut mengenai proses patofisiologi retinopati hipertensi. Penelitian ini bertujuan untuk membandingkan ketebalan retina sentral pasien hipertensi esensial tanpa penurunan visus dibanding orang normal. Jenis penelitian ialah prospektif komparatif menggunakan metode kuantitatif. Hasil penelitian mendapatkan 56 mata untuk kelompok hipertensi dan 71 mata untuk kelompok normal yang memenuhi kriteria penelitian. Uji t menunjukkan terdapat perbedaan ketebalan retina sentral pada kelompok hipertensi dibanding kelompok normal. Rerata ketebalan retina pada kelompok hipertensi ialah 201,66 μm (SD±38,870) dibanding pada kelompok normal ialah 249,59 μm (SD±19,245) (p=0,004). Simpulan penelitian ini ialah terdapat perbedaan bermakna antara ketebalan retina sentral pasien hipertensi esensial tanpa penurunan visus dibanding orang normal. Disarankan untuk melakukan pemeriksaan mata pada penyandang hipertensi esensial meskipun belum disertai keluhan penurunan tajam penglihatan.Kata kunci: hipertensi, penurunan visus, ketebalan retina

Abstract

The increasing burden of cataract-induced blindness in Indonesia demands competent surgical responses, particularly through the phacoemulsification technique—a minimally invasive surgical procedure globally recognized as the gold standard. However, structural limitations in training policies, unequal access to resources, and political dynamics impede the readiness of new ophthalmologists in mastering this technique. This study explores the systemic, policy, and political contexts underlying phacoemulsification training in Indonesia. Using a qualitative descriptive approach supplemented by a literature review, this research analyzes dimensions of curriculum development, accreditation standards, infrastructure distribution, stakeholder politics, and professional ethics compliance. Findings indicate that to ensure equitable surgical competence among graduates of ophthalmology specialist programs, policymakers need to integrate national regulations with international standards, decentralize access to quality training, and enforce ethical governance across all medical education institutions.

Abstract

Introduction and Objective: The various types of primary interventions for rhegmatogenous retinal detachment (RRD) include pars plana vitrectomy (PPV), scleral buckling (SB), and combination of both. Intraocular tamponade with silicone oil (SO) or expanding gas. This research was conducted to evaluate the clinical outcomes of SB combined with PPV as a treatment for RRD. Methods: This research was prospective descriptive study with total sampling for RRD treated by SB combined with PPV from August 2022 to January 2023 in RSUP. Prof. Dr. R.D. Kandou. BCVA, IOP, clinical findings, intraocular tamponade and complications after the surgery were observed. Results: Most of the patients was male with average age of 28 ± 40 years. RRD was observed in 6 eyes and laterality on right eye. BCVA was 2.5 ± 2 logMAR before the surgery and 2.3 ± 1 logMAR 1 month after the surgery. BCVA day 1, 1 week, 2 weeks and 1 month showed no significant difference after the surgery compared to baseline (p = 0.317, p = 0.949, p = 0.611, p= 0.611 respectively). Preoperative IOP was 7 ± 11 mmHg and 13.6 ± 10 mmHg 1 month after the surgery. IOP 1 week and 1 month showed significant difference (p = 0.027, p=0.011). IOP day 1, 2 weeks and 1 month showed there was no significant difference after the surgery on day 1 and 2 weeks. Conclusion: There were several clinical condition seen such as BCVA after the surgery didn’t improve as expected, subconjunctival bleeding was found the most as postoperative complications. Macular detachment, 1 Quadrant Detachment, 1 Number of retina break mostly on superotemporalquadrant were found the most. Proliferative vitreoretinopathy was found in 1 eye.Intraoperative triamcinolone used in 4 eyes and perfluorocarbon in 1 eyes. Subconjunctival bleedingwas found the most, followed by corneal edema and subretinal hemorrhage as postoperativecomplications respectively. ConclusionThere were several clinical condition seen such as BCVA after the surgery that didn’t improve asexpected, High IOP found in several eyes and subconjunctival bleeding was the complication foundthe most in these eyes.
