Abstract

Transforming primary healthcare in Indonesia through integrated, lifecycle-based approaches is a strategic effort to achieve a high-value, high-quality health system. This study analyzes the role of family medicine within Indonesia’s primary care model, highlighting how family doctors contribute to improved healthcare accessibility, continuity, and responsiveness. Family doctors serve as frontline providers across lifecycle clusters—including maternal and child health, productive adult care, elderly care, and infectious disease control. Their role in managing chronic illnesses, mental health, and preventive care is essential within the Indonesian context, as they deliver comprehensive services that address the biological, psychological, and social determinants of health. Digital integration through the Platform Satu Sehat and the Pemantauan Wilayah Setempat (PWS) dashboard supports real-time health monitoring, enabling family doctors to make data-driven decisions and provide proactive care. In addition, family doctors extend their impact through community engagement, leading immunization campaigns, health education, and public health initiatives. They also serve as patient advocates by coordinating multidisciplinary teams and guiding individuals through complex healthcare pathways. This Indonesia-based model demonstrates the potential of family medicine to drive sustainable, community-based primary care transformation and provides a replicable framework for other low- and middle-income countries. The findings reinforce the critical role of family doctors as a foundation for equitable access, improved quality of care, and resilient health systems.

Abstract

During prenatal and post-partum period, 10-20% of women experience depression. The quality of life and functional capacity of these women were also significantly impacted by depression. This condition also has an adverse effect on fetal development and newborn. Furthermore, home visit have been used to reduce maternal depression. However, the evidence of this still low. Hence, this review aims to evaluate the effectiveness of home visiting in preventing maternal depression. Articles for this systematic review were collected from several databases (PubMed, Scopus, Google Scholar, and ScienceDirect) using terms related to maternal depression, prevention, and home visiting. The quality of included studies were assessed using the Newcastle-Ottawa Scale (NOS). This systematic review is comprised of 13 high quality randomized clinical trials with 4.804 participants. Eleven studies indicate that home visiting effectively reduce depressive symptoms and a study shows that mothers receiving home visiting are twice less likely to develop depressive symptoms. Home visiting are also beneficial for low-income women amidst the low rate of mental health services. Only two studies state that there is no evidence that home visiting effectively reduces depressive symptoms and increases caregiving quality. However, some studies state that the mother's cognitive, child growth, and maternal depression may improve with home visiting integrated with cognitive behavioral therapy, counseling, education such as lectures, or video. Home visiting are effective in preventing maternal depression. Further studies are needed to compare the effectiveness of the intervention plan in home visiting.

Abstract

The caregiver among cancer patients often experiences hardships while providing emotional, physical, and financial support for their family members. Cancer is one of the leading causes of death. However, due to improved cancer treatment, there is a reduced mortality rate and increased survival rate among patients with cancer, resulting in an increased and prolonged burden for both patients and caregivers. The caregiver is a professional in treating patients requiring extended outpatient care, assisting patients in their daily activities, and performing medical treatment. The method we used is a systematic review of the correlation between burnout rates in cancer patients' caregivers. Was also conducted a researcher's method uses a cross-sectional/study. We used 14 articles that correlated and searched the keyword cancer, patient cancer, burnout, and caregivers. Also, the results stated that caregivers are highly related to the level of burnout; thus, research shows caregivers have low self-esteem, adverse health effects, and depression. Ultimately, there is a high correlation between burnout in caregivers and cancer patients. Caregivers play a crucial role in the management of cancer patients. Giving caregivers the social support they need, educational resources, and supporting programs may decrease burnout because they need to provide for their mental health, which correlates with low self-esteem, adverse health effects, and depression. There is great potential for improvement in the design of cancer caregiver intervention studies to plan for future implementation, so more in-depth studies on this topic are needed.

Abstract

Evaluasi CCM mengkonfirmasi perlunya transformasi sistem layanan proaktif. Diskusi: Peningkatan pengetahuan signifikan mendukung HBM bahwa isyarat bertindak (cue to action) memperkuat persepsi manfaat.

Abstract

Background: Indonesia faces an 85% depression treatment gap, with only 0.27 psychiatrists per 100,000 population. Culturally adapted training for primary care physicians is urgently needed. This study evaluated the training effectiveness and early implementation outcomes of an Indonesian-adapted Train-the-Trainer (TtT) MDD Minds module. Methods: This Type 1 mixed-methods implementation study involved 30 primary care physicians who completed centralized TtT training (October–November 2024). Eighteen trained scholars subsequently delivered community training to 125 healthcare workers across 12 provinces (December 2024–February 2025). Patient outcomes were assessed over 3 months among 96 patients (58 with comorbid hypertension) treated by trained providers. No control group was included, consistent with the proof-of-concept design. Knowledge, perceived self-efficacy, practicality, and patient-level outcomes (PHQ-9, MMAS-8, blood pressure) were measured using validated instruments. Results: Knowledge scores improved significantly from 67.48% to 93.24% (Cohen's d=2.87, p<0.001), with sustained perceived self-efficacy at 4-week follow-up (2.3 to 3.7/5). Practicality was rated highly (3.65/4.0). Among patients, mean PHQ-9 decreased by 6.1 points (95% CI: 5.3–6.9, p<0.001); 64.6% achieved treatment response. Hypertensive patients showed improved medication adherence (MMAS-8 +1.8, p<0.001) and blood pressure control (SBP −14.4 mmHg, p<0.001). Conclusion: The Indonesian-adapted TtT MDD Minds module demonstrated significant training effectiveness, high practicality, and promising preliminary patient benefits. These proof-of-concept findings support broader dissemination pending Phase III randomized evaluation.

Abstract

Background: Indonesia continues to face major challenges in tobacco control, with a high prevalence of smoking, including among adolescents. Orphaned adolescents constitute one of the most vulnerable populations, owing to minimal parental supervision and high exposure to psychosocial stress, both of which increase the risk of smoking behavior. This study aimed to evaluate the effectiveness of an educational intervention on the dangers of smoking for orphaned adolescents at the Maktabul Aitam Orphanage for Orphaned and Underprivileged Children (Panti Asuhan Yatim Piatu & Dhuafa Maktabul Aitam), Ivory Serpong, South Tangerang. Methods: The study employed a quasi-experimental pretest–posttest design involving 23 adolescents who received interactive education. The material covered the dangers of conventional and electronic cigarettes, the effects of passive smoking, and smoking cessation strategies. Data were collected through written pretests and posttests and analyzed descriptively and inferentially using a paired t-test. Results: The results showed that the mean baseline knowledge score was only 43/100, which increased significantly to 95/100 after the intervention (p < 0.001). These findings indicate that contextually tailored education can enhance orphaned adolescents’ understanding of and awareness regarding the dangers of smoking. Conclusion: Life-cycle-based educational programs delivered by family physicians can serve as a key strategy for disease prevention in vulnerable adolescent populations. The integration of such programs into the primary health care system is essential for long-term tobacco control and sustainable health development.
