Abstract

Abstract

Background: Early Warning Score (EWS) is a system for requesting help to overcome patient health problems early which is based on an assessment of changes in the patient's condition through systematic observation of all changes in the patient's physiology. Inconsistent implementation of EWS can be caused by decreased awareness of the importance of monitoring vital signs and the level of knowledge of nurses. Objective: To determine the relationship between nurses' knowledge regarding EWS tools and their application to patients at Siloam Manado Hospital. Method: This research is a correlational descriptive quantitative research with a cross sectional approach with a sample of 50 people. The sampling technique used purposive sampling, namely 50 people. Results: Based on the results of the chi square test, 40 respondents (95.2%) had high knowledge of the correct application of EWS, 6 respondents (75.0%) had low knowledge. The results of the bivariate test using the chi square test showed that the level of nurses' knowledge did not have a significant relationship with the implementation of EWS with a value of (p=0.000) supported by a contingency coefficient of 0.000. Future research should examine performance behavior by observing the population to see a more complete implementation of EWS. Keywords: Early warning score; Level of knowledge; implementation of EWS.

Abstract

Introduction: Patient satisfaction is a key indicator of healthcare quality. The Emergency Department (ED) of Prof. Dr. R. D. Kandou General Hospital, Manado, is one of the busiest hospital units, handling various critical cases. However, preliminary data indicated a NPS of –37%, suggesting that most patients were dissatisfied and unlikely to recommend the service. This prompted the need for a systematic evaluation and managerial intervention to improve the quality of nursing care and patient experience. Objective: To evaluate patient satisfaction with emergency department services at Prof. Dr. R. D. Kandou General Hospital using the NPS method, to identify key influencing factors, and to formulate improvement strategies based on nursing management principles. Method: This study applied a quantitative descriptive approach with an evaluative design. Data were collected from non-trauma ED patients using the NPS questionnaire. Root cause analysis was conducted using the Fishbone Diagram, problem prioritization with the USG (Urgency, Seriousness, Growth) method, and strategy development through SWOT analysis. Interventions included AIDET-based effective communication training and optimization of service flow and management in the emergency department. Result: Initial findings showed an NPS score of –37%. After implementing communication training and standard operating procedure (SOP) reinforcement, the NPS improved to +57%, with 70% promoters, 17% passives, and 13% detractors. This demonstrates a significant improvement in patient satisfaction and service quality following the managerial intervention. Conclusion: There was a substantial improvement in patient satisfaction after applying the NPS-based nursing management intervention. Effective communication, interprofessional collaboration, and data-driven quality evaluation significantly enhanced the quality of care in the emergency department. The NPS method proved to be a practical and reliable tool for measuring patient satisfaction and loyalty in hospital services.

Abstract

ABSTRACT Nurses performance is a key indicator of nursing service quality and patient safety in hospitals. From a nursing management perspective, nurses’ performance is influenced by various internal organizational factors, including organizational culture, leadership style, and work motivation. Although these factors are theoretically assumed to play an important role in shaping nurses’ behavior and performance, empirical findings remain inconsistent, particularly in private hospital settings. This study aimed to analyze the influence of organizational culture, leadership style, and work motivation on nurses’ performance at Siloam Manado Hospital. This study employed a quantitative approach with a cross-sectional design. A total of 110 nurses were selected as respondents using a simple random sampling technique. Data were collected through structured questionnaires that had been tested for validity and reliability. Data analysis was conducted using dummy linear regression, preceded by classical assumption tests including normality, multicollinearity, heteroscedasticity, and linearity tests. The simultaneous test results indicated that organizational culture, leadership style, and work motivation did not have a significant joint effect on nurses’ performance (p 0.05). Partial analysis also revealed that each independent variable did not significantly influence nurses’ performance. The coefficient of determination (R²) was 0.034, indicating that the regression model explained only 3.4% of the variance in nurses’ performance. In conclusion, nurses’ performance at Siloam Manado Hospital appears to be relatively stable and not significantly influenced by organizational culture, leadership style, or work motivation. This finding may be attributed to the strong implementation of standardized operating procedures and structured work systems, which minimize performance variation attributable to internal organizational factors. Keywords: Leadership Style, Nurses Performance, Organizational Culture, Work Motivation.

Abstract

Nurse performance is a key indicator of nursing care quality and patient safety in hospitals. One managerial factor that may influence nurse performance is the leadership style of head nurses. Differences in work characteristics between critical and non-critical care units require appropriate leadership approaches to maintain optimal staff performance. This study aimed to analyze the relationship between head nurses’ leadership styles in critical and non-critical care units and nursing staff performance at Siloam Hospital Manado. This study employed a quantitative approach with a correlational cross-sectional design. The sample consisted of 110 nurses and midwives working in critical and non-critical units at Siloam Hospital Manado, selected using a purposive sampling technique. Data were collected using validated and reliable questionnaires on leadership style and nurse performance. Data analysis was conducted using the Chi-Square test and contingency coefficient. The findings showed that democratic leadership was the most dominant leadership style perceived by respondents in both critical and non-critical units. The Chi-Square test result revealed a p-value of 0.065 (p > 0.05), indicating no statistically significant relationship between leadership style and nurse performance. The contingency coefficient value of 0.217 indicated a weak association between the two variables. Conclusion: Head nurses’ leadership at Siloam Hospital Manado is predominantly democratic. However, statistically, leadership style was not significantly associated with nursing staff performance. Nurse performance appears to be more strongly influenced by other factors such as professional competence, nursing practice standards, and hospital work systems.
