Abstract

Background: With advancements in medical technology, hospitals can provide fast and accurate medical services, serving as referral hospitals according to their level. The waiting time for prescription services also affects patients' expectations of hospital services, particularly pharmaceutical services. Efforts to reduce waiting times are necessary to enhance patient satisfaction, treatment effectiveness, and the workload of medical staff. Objective: The aim of this study is to identify the factors that influence the waiting time for outpatient medication services. Method: This research employs a mixed-method approach. The population in this study consisted of 10,607 prescription sheets and 23 pharmacy staff. The sample was taken using Slovin's formula, resulting in 385 prescription sheets and 10 staff members selected through consecutive sampling. Data analysis was conducted using Chi-square tests and qualitative analysis. Results: The study found that 93.5% of prescription sheets had waiting times that complied with standard of the minister of health, while 6.5% did not. The research also revealed no significant effect between the type of prescription and the compliance of waiting times for medication services at Dr. OEN KANDANG SAPI SOLO HOSPITAL, with a p-value of 0.134 (p>0.05). However, interviews indicated that factors such as drug availability, prescription completeness, human resources availability, and the availability of facilities and equipment do influence waiting times for medication services. Conclusion: The waiting time for medication services at the Pharmacy Installation of Dr. OEN KANDANG SAPI SOLO HOSPITAL mostly meets the standards. The type of prescription does not have a statistical impact; however, factors such as drug availability, prescription completeness, human resources, and facilities and equipment can affect waiting times.

Abstract

Background: Cardiovascular emergencies are a leading cause of mortality and morbidity in Indonesia and worldwide. Approximately 80% of sudden deaths are caused by coronary heart disease. Nurses play an important role in the management of patients experiencing cardiovascular emergencies. Consistent and comprehensive implementation of the emergency nursing process is key to providing quality nursing care. Purpose: to analyze nursing care for patients with cardiovascular emergencies. Method: A case study design. Data were collected through direct observation, interviews with patients and families, and review of medical records. Variables collected included medical diagnoses, assessment data, physical examination and supporting examination results, problem analysis, interventions, implementation, and nursing evaluation. Results: STEMI patients experience chest pain, shortness of breath, cold sweats, ST elevation on electrocardiography, and elevated troponin levels. The nursing problem identified is ineffective myocardial perfusion. The actions taken included administering nasal cannula oxygen therapy at 3 lpm, recording a 12-lead ECG, administering ASA 300 mg, CPG 300 mg, parenteral fluids, ISDN 5 mg (sublingual), Candesartan 8 mg, inserting a urinary catheter, and observing vital signs. Evaluation after treatment in the ER showed that the problem of ineffective myocardial perfusion was partially resolved, so the patient was admitted to the ICCU for further treatment. Conclusion: Patients with cardiovascular emergencies experience ineffective myocardial tissue perfusion. Patients are given several treatments to address their emergency conditions, including ASA, CPG, ISDN, and Candesartan therapy. Keywords: Cardiovascular System; Emergency Care; Nursing Care.   Pendahuluan: Kegawatdaruratan sistem kardiovaskular merupakan penyebab mortalitas dan morbiditas utama di Indonesia dan di dunia. Sekitar 80% kematian mendadak disebabkan oleh penyakit jantung coroner. Perawat memiliki peran yang penting dalam penanganan pasien yang mengalami kegawatdaruratan sistem kardiovaskular. Penerapan proses keperawatan gawat darurat secara konsisten dan komprehensif menjadi kunci keberhasilan dalam memberikan asuhan keperawatan yang berkualitas. Tujuan: Untuk menganalisis asuhan keperawatan pada pasien kegawatdaruratan sistem kardiovaskuler. Metode: Desain studi kasus. Data dikumpulkan dengan melakukan observasi langsung, wawancara dengan pasien dan keluarga, serta review rekam medis. Variabel yang dikumpulkan meliputi diagnosa medis, data hasil pengkajian, hasil pemeriksaan fisik dan pemeriksaan penunjang, analisis masalah, intervensi, implementasi, serta evaluasi keperawatan. Hasil: Pasien STEMI mengalami manifestasi nyeri dada, sesak napas, keringat dingin, Gambaran ST elevasi pada elektrokardiografi, serta adanya peningkatan troponin. Masalah keperawatan yang ditemukan adalah perfusi jaringan miokard tidak efektif. Tindakan yang dilakukan berupa pemberian terapi oksigen nasal canul 3 lpm, perekaman EKG 12 lead, pemberian terapi ASA 300 mg, CPG 300 mg, cairan parenteral, ISDN tab 5 mg (sub lingual), Candesartan 8 mg, pemasangan kateter urin dan observasi TTV. Evaluasi setelah dilakukan perawatan di UGD didapatkan hasil masalah perfusi jaringan miokard tidak efektif teratasi sebagian, maka untuk perawatan lebih lanjut pasien dirawat di ICCU Simpulan. Pasien dengan kegawatdaruratan sistem kardiovaskuler mengalami masalah perfusi jaringan miokard tidak efektif. Pasien diberikan beberapa tindakan untuk menangani kondisi kegawatdaruratannya berupa pemberian terapi ASA, CPG, ISDN, dan Candesartan. Kata Kunci: Asuhan Keperawatan; Kegawatdaruratan; Sistem Kardiovaskuler.

Abstract

Hospitals are work environments with high levels of occupational safety and health risks for medical and non-medical personnel, requiring the implementation of Hospital Occupational Health and Safety (K3RS) management through the application of risk management and preparedness in dealing with emergency situations.This study used a descriptive quantitative design with a cross-sectional approach.The research subjects consisted of 120 respondents, including medical and non-medical personnel, who were selected using purposive sampling. Data were collected using questionnaires and analyzed univariately. The implementation of risk management was mostly in the good category, although there were still obstacles in the consistency of incident reporting and the application of ergonomic principles in the work environment. The level of preparedness of medical and non-medical personnel was also mostly in the good category, as indicated by their understanding of hazard reporting mechanisms, risk documentation, and emergency response procedures. Respondent characteristics such as length of service and education level also influenced the implementation of K3RS. The implementation of risk management and preparedness has been good, but still needs improvement through strengthening the reporting culture, continuous monitoring, and improving workplace ergonomics.

Abstract

Patient satisfaction is an important indicator of healthcare service quality, one of which is influenced by waiting time. Initial observation showed that the waiting time in the general polyclinic was still long, around 90–120 minutes. This study aims to determine whether there is a relationship between waiting time and patient satisfaction in outpatient installations. We conducted an observational study with a cross-sectional design. The population was 100 outpatients at Dr. Oen Hospital, Solo Baru, who visited in 2024. Using a purposive sampling technique, a sample of 64 patients was taken. There were two main variables studied, namely waiting time (measured by an observation sheet) and patient satisfaction (measured by a questionnaire). Data were analyzed using chi-square test. Most patients (79.7%) waited > 60 minutes with an average of 74.98 minutes, but 71.9% were still very satisfied. The chi-square analysis showed no significant relationship between waiting time and patient satisfaction (p = 0.399). Based on the results of the study, it can be concluded that the length of waiting time for registration does not affect patient satisfaction in outpatient installations. However, it is still recommended that the hospital continue to strive to reduce patient waiting time so that service capacity increases.

Abstract

Since cardiovascular disease is the world's biggest cause of death, it poses a threat to the entire planet. Common signs of cardiac issues include shortness of breath, changes in breathing patterns, breathing frequency, and breathing depth. Patients with coronary heart disease require intervention to enhance their respiratory condition since respiratory issues can negatively impact their quality of life. It is necessary to demonstrate how a combination therapy of pursed lip breathing (PLB) and peppermint aromatherapy affects the respiratory condition of individuals with coronary heart disease. Only one intervention—breathing exercises to lessen dyspnea or aromatherapy to lessen tension and anxiety in individuals with coronary heart disease—has been offered by prior research. In order to enhance the respiratory status of patients with coronary heart disease, this study combines pursed lip breathing with peppermint aromatherapy interventions. Purposive sampling strategies were used to split a sample of 44 coronary heart disease patients into an intervention group and a control group for the quasi-experimental study design. Stopwatches, oximeters, and observation sheets were the tools utilized. The Mann Whitney U Test and the Wilcoxon Signed Rank Test were used for statistical analysis. Following the intervention, the data demonstrated an increase in oxygen saturation and a decrease in respiratory rate. Both the rise in oxygen saturation and the decrease in respiratory rate were greater in the intervention group compared to the control group. The combination therapy of pursed lip breathing and peppermint aromatherapy had an impact on respiratory status, according to the statistical test findings, which indicated a p-value < 0.05. In patients with coronary heart disease, the combination therapy of pursed lip breathing and peppermint aromatherapy was found to significantly lower respiratory rate and raise oxygen saturation.
