Abstract

Kewaspadaan klinis perawat ICU merupakan elemen fundamental dalam perawatan pasien kritis, yang memungkinkan deteksi dini perubahan kondisi, pengambilan keputusan tepat, dan peningkatan keselamatan pasien di lingkungan intensif yang dinamis. Penelitian menunjukkan perawat berpengalaman memiliki akurasi deteksi sepsis 30% lebih tinggi, meskipun faktor seperti shift panjang menurunkan tingkat kewaspadaan. Tujuan : Analisis ini bertujuan mengklarifikasi definisi, atribut, antecedent, konsekuensi, serta implikasi kewaspadaan klinis perawat ICU untuk mendukung pengembangan intervensi berbasis bukti yang meningkatkan praktik keperawatan intensif. Metode Walker and Avant (2018) diterapkan melalui delapan langkah sistematis, meliputi identifikasi penggunaan konsep dari kamus dan literatur, penentuan atribut (pemantauan sistematis, pengenalan pola, dll.), analisis antecedent-konsekuensi, kasus model/borderline/terkait/sebaliknya, serta referensi empiris dari studi terkini. Kewaspadaan klinis didefinisikan sebagai kemampuan mempertahankan perhatian berkelanjutan, mendeteksi perubahan dini, dan merespons cepat; antecedent mencakup pendidikan ICU, pengalaman >2 tahun, staffing optimal; konsekuensi meliputi penurunan komplikasi 30% dan mortalitas; model konsep dihasilkan dari kasus empiris, didukung survei vigilance skor 3,86/5. Model konsep ini memperkuat pemahaman aplikatif kewaspadaan klinis, mendorong pelatihan simulasi, instrumen pengukuran seperti Nursing Vigilance Scale, dan kebijakan rumah sakit untuk outcome pasien optimal. Clinical vigilance of ICU nurses is a fundamental element in the care of critically ill patients, enabling early detection of changes in patient conditions, accurate decision-making, and enhanced patient safety in a dynamic intensive care environment. Research indicates that experienced nurses have a 30% higher accuracy rate in detecting sepsis, although factors such as prolonged shifts may decrease levels of vigilance. Objective: This analysis aims to clarify the definition, attributes, antecedents, consequences, and implications of clinical vigilance among ICU nurses to support the development of evidence-based interventions that improve intensive nursing practice. Methods: The Walker and Avant (2018) concept analysis method was applied through eight systematic steps, including identifying concept usage from dictionaries and literature, determining attributes (systematic monitoring, pattern recognition, etc.), analyzing antecedents and consequences, constructing model/borderline/related/contrary cases, and incorporating empirical references from recent studies. Clinical vigilance is defined as the ability to maintain sustained attention, detect early changes, and respond rapidly; antecedents include ICU education, more than two years of experience, and optimal staffing; consequences involve a 30% reduction in complications and mortality; the conceptual model was developed from empirical cases, supported by vigilance survey scores of 3.86/5. This conceptual model strengthens the practical understanding of clinical vigilance, promoting simulation-based training, measurement instruments such as the Nursing Vigilance Scale, and hospital policies to optimize patient outcomes

Abstract

Abstrak Lansia merupakan kelompok rentan yang mengalami perubahan fisik, psikologis, dan kognitif seiring proses penuaan. Hipertensi sebagai penyakit kronis yang sering dialami lansia dapat memicu kecemasan dan mempercepat penurunan fungsi kognitif. Terapi reminiscence merupakan salah satu intervensi nonfarmakologis yang berpotensi mengatasi permasalahan tersebut. Penelitian ini bertujuan untuk menelaah efektivitas terapi reminiscence dalam menurunkan kecemasan dan meningkatkan fungsi kognitif pada lansia melalui kajian literatur. Metode yang digunakan adalah literature review dengan penelusuran artikel melalui database PubMed dan Google Scholar. Kriteria inklusi meliputi artikel berbahasa Indonesia dan Inggris, terbit pada periode 2021–2025, menggunakan desain randomized controlled trial dan quasi experimental, serta tersedia dalam bentuk full text dan open access. Dari hasil seleksi, diperoleh lima artikel yang memenuhi kriteria. Hasil kajian menunjukkan bahwa terapi reminiscence efektif menurunkan tingkat kecemasan dan meningkatkan fungsi kognitif pada lansia, termasuk lansia dengan hipertensi. Terapi ini bekerja melalui stimulasi memori autobiografis, peningkatan interaksi sosial, dan penurunan stres emosional. Kesimpulan kajian ini menunjukkan bahwa terapi reminiscence merupakan intervensi nonfarmakologis yang aman, efektif, dan mudah diterapkan dalam pelayanan keperawatan lansia. Abstract

In-depth conceptual analysis of the NURSE Program (Nurturing, Understanding, Responding, Supporting, Empowering) intervention for postpartum depression based on Cheryl Tatano Beck's theory is still limited. To analyze and operationalize the NURSE concept into specific nursing actions and test its application in community public health center. Using sequential explanatory mixed methods. The qualitative phase involved concept analysis, followed by a quantitative quasi-experimental one-group pretest-posttest phase involving 30 postpartum mothers (EPDS ≥10) at the public health center. Data were analyzed descriptively and using paired t-tests. The analysis successfully mapped each NURSE concept to the stages of Beck's “Theory of Self-Loss.” The intervention showed a significant decrease in postpartum depression scores (p<0.001). The average EPDS score decreased from 17.2 (moderate-severe) to 8.6 (mild-non-depressed), with 83.3% of respondents experiencing a decrease in category. The NURSE Program is a coherent conceptual construct that integrates theory and is effectively applied. The results of this concept analysis can be used as standard operational guidelines for nurses in theory-based maternal mental health nursing care.

Abstract

The equitable distribution of nurses is a foundational element of health system equity and effectiveness. Disparities in nurse allocation between urban and rural areas have long been recognized as barriers to achieving universal health coverage and patient-centered care. Despite policy efforts, the concept of equity in nurse distribution often remains undefined and inconsistently applied in practice. This paper applies Walker and Avant’s (2010) concept analysis approach to clarify the meaning, attributes, antecedents, and consequences of equitable distribution of nurses as a central concept in nursing administration and workforce planning. Through theoretical analysis and synthesis of literature, five defining attributes were identified: fairness, accessibility, competence alignment, sustainability, and policy responsiveness. The findings highlight that equitable nurse distribution extends beyond numerical balance, reflecting ethical, structural, and reflective dimensions of the nursing discipline. This conceptual clarity contributes to a stronger theoretical framework for nursing management and supports evidence-based policymaking to ensure justice and quality in healthcare delivery.
