Free radicals contribute to oxidative stress, premature aging, and degenerative diseases, while extracts from plants of the Annonaceae family are rich in flavonoids, polyphenols, and alkaloids with strong antioxidant properties. This study aimed to systematically review the formulation, physicochemical characteristics, stability, and activity of topical cream preparations containing Annonaceae plant extracts based on literature published between 2020 and 2025, as well as to identify the most optimal formulation. A systematic literature review following PRISMA guidelines was conducted using Google Scholar and SINTA databases, yielding 10 eligible articles from an initial 100 after duplicate removal and screening, with inclusion criteria covering ethanol maceration extraction, oil-in-water formulations, evaluation of pH, viscosity, spreadability, and DPPH antioxidant testing. The results showed that the creams were stable (pH 5.2–7.0, viscosity 18,000–50,000 cPs, spreadability 4.9–6.4 cm, oil-in-water type, non-irritating), with dose-dependent IC₅₀ values ranging from 10.91 to 88.73 ppm, indicating strong to very strong antioxidant activity (p < 0.05), leading to the conclusion that Annonaceae plant extracts have promising potential as stable active ingredients in optimal topical cream formulations.
