Abstract

Objective: The aim of the periodontal treatment is to provide healthy and functional dentition all through a lifetime. The primary objective of this case report is to describe that periodontal disease in the elderly complicated by medical compromise should be treated and the results will be very satisfactory.Methods: Female 68 year old with cardiovascular disease took Amlodipine and Aspirin medication since two months ago. Patients complained of enlarged gingiva, all of the teeth become mobility. Periodontal surgery is performed by previously referring back to the internist to prepare patient condition facing periodontal surgery.Results: After several fase periodontal treatment, the gingival inflamation demonstrated a good control and periodontal status become more stable.Conclusion: Although managing periodontal disease in elderly is complicated, the proper control of the periodontal infection and inclusion of other healthcare professionals will improve the prognosis and get a satisfactory result. In fact, studies demonstrate that elderly individuals whose comprehensive management includes dental care develop fewer co-morbid conditions and require less expenditure of healthcare rupiahs.

Abstract

Objective: This paper aims to review the studies of the saliva analysis for forensic odontology purposes, starting from identifying the saliva stain and extracting the saliva sample.Methods: A literature review was performed using textbook (2016) and online search in scientific databases: PubMed®, ScienceDirect®, JDMFS online, EBSCOhost® (from 2002 to 2017). The studies generated were screened and selected by a single examiner based on the following criteria: full text articles with abstracts reporting on saliva and forensic odontology or personal identification and all articles are written in English.Results: From the articles gained by the author, the author found that saliva stain can be detected through many ways and can be extracted using single swab or double swab technique. Saliva analysis can be used to detect drug and hormone abuse, to detect intoxication, to identify suspects and victims of a crime or disaster, and also to identify bitemark’s perpetrator. Conclusion: Saliva is a very useful diagnostic tool in forensic odontology. Its detection and analysis is very important in verifying the existence of a crime, in identifying suspects and victims of a crime, and in personal identification in mass disaster.  Despite all the usefulness of saliva analysis, the existence of saliva in crime scene is often difficult to be confirmed, therefore it is better to combine more than one saliva stain identification tool to confirm.

Abstract

Objective:Chronic periodontitis lesions include loss of attachment and bone and are regarded as irreversible. Conventional procedures cannot restore the periodontal tissue to its original form. Periodontal tissue engineering is regenerativemedicine novel therapy by either implanting biomaterials. Platelet Rich Fibrin (PRF) as second generation platelet and Carbonate Hydroxyapatite (CHA) alloplastic bone graft can be used in surgical therapy because ease, inexpensive method and does not need any addition of exogenous compounds and a second surgical site and procedure. This case report aims to evaluate the clinical and radiographic effectiveness combination of both for the treatment of chronic periodontitis.Methods:A 21-year-old female was reported with chief complaint tooth mobility. Initial examination revealed grade two mobility of  35 with 9 mm distobuccal and distolingual pocket depth. The Periapical radiograph revealed a large diffused periapical radiolucency in relation to 35. Periodontal surgery was done and the osseous defect was filled with CHA bone graft and PRF.Results:There was a significant change in pocket depth and grade of mobility six months after surgery to 3 mm and grade one mobility.Radiographs showed resolution of an osseous defect.Conclusion:It was concluded that combined PRF with CHA bone graft can be used to treat chronic periodontitis as simple technique periodontal tissue engineering.

Abstract

Objective: To review the classifications of denture patient’s mental attitude in order to expand reader’s knowledge and help practitioners to confront their patient.Methods: Electronic search in internet to find articles which were published in last 20 years with keywords “patient mental attitude” and “house classification”. Articles were then selected by reviewing the title and abstract.Results: Based on data search, there are eight journal articles that suitable with the inclusion criteria.Conclusion: It is important for dentists to recognize patient’s mental attitude before patient’s general condition to help dentist to communicate and deal with patient along their mental attitude, in order to create a bond of trust between patient and practitioner, facilitate the treatment procedure and make a better treatment result.

Abstract

Objective: To explain the management of gingival overgrowth caused by mechanical trauma during tooth preparation.Method: In this case report, a patient experience gingival overgrowth one day after trauma during tooth preparation for bridge. The patient feel severe paint especially when the inflamed gingiva is bitten during occlusion. In intraoral examination overgrowth is localized red to pink white, firm nodule gingiva. Management of the patient is surgical removal of the overgrowth gingivaResults: After one week of operation, the gingiva shows a good healing and no recurrency.Conclusion: Gingival overgrowth may develop as a result from trauma or chronic irritation. The High speed rotary instrument may also make the trauma in the gingiva during tooth preparation. The carefully is needed when using this instrument.

Abstract

Objective: To describe that Wegener’s granulomatosis (WG) is an autoim­mune multisystem disease with unknown etiolo­gy, characterized by the classic triad of necrotizing granulomas affecting the upper and lower respiratory tracts, disseminated vasculitis and glomerulonephritis. WG can affect any organ including kidneys, eyes or other organs. Oral involvement, like strawberry gingivitis, is an important sign of this disease even though it is rare.Methods: A 19 years old female was referred to oral medicine clinic with the chief complaint swelling on the gingiva which was increasing widespread. Since one month before the gingival enlargement did not respond to antibiotic.  Intraoral examination, showed gingival enlargement at the upper anterior, palate, and first left mandibular molar. The swollen tissue was characterized by its granularity, short bulbous projections and bright red colour. The gingival appearance mimicking an overripe strawberry and extra oral examination showed cutaneous vasculitis at the hands and feet.  Laboratory evaluation presented erythrocyte sedimentation rate, CRP and eGFR (modified MORD) were elevated but the urinalysis was within normal ranges. Based on the anamnesis, clinical and laboratory examination, WG diagnosis was made. The patient was treated with oral corticosteroids and chlorhexidine gluconate 0.1% mouthwash.Results: The gingival inflammation improved after one month therapy and the patient still being followed-up at reguler time. Conclusions: Early detection of oral manifestation of WG is important for dentists, especially oral medicine specialist, in order to achieve adequate treatment and preventing systemic tissue or organ damage.

Abstract

Objective: A grade II furcation involvement has been defined as a defect having a horizontal loss attachment of 3 mm or greater but not through the entire furcation. There are several techniques used alone or in combination considered to achieve periodontal regeneration. This case report aims to describe regenerative approach in the treatment of grade II furcation defects in maxillary molars. Method: A-43-years old male reported to the Department of Periodontics with a complaint of pain. At the initial visit, scaling and root planning was done. It was followed by periodontal regenerative therapy using bone graft and resorbable membranes.Results: The treatment for overall seemed success with healthy periodontium and complete closed defect with bone fill, based on the clinical and radiographic development of  the patient at the end of six month follow up.Conclusion: The regenerative approach such as resorbable GTR membrane with bone material was more effective than open debridement alone, in the treatment of furcation defects.

Abstract

Objective: Ossifying fibromas is a benign type of fibro-osseous tumor. These lesions may arise in the maxillary or mandible, but generally have more predilection of the mandible. Women are more often affected.Methods: A female patient, aged 40 years old, came to the Oral and Maxillofacial Polyclinics at Hasan Sadikin Hospital, complaining of a lump in the upper jaw. The lump effect makes the face asymmetrical, painless and has a hard consistency. We diagnosed this patient with ossifying fibroma. Ossifying fibroma is a kind of lesion that can spread to all directions, so it can resemble a sphere shape. Enucleation and curettage are the common treatment. Larger lesions, which have significant expansion or aggressive growth features, may require resection or wide excision Results: In this case our treatment ossifying fibroma is wide excision with ferguson webber incision technique and good results.Conclusion: Ossifying fibromas is a benign type of fibro-osseous tumor. Wide excision is an option for a large size of ossifying fibroma.

Abstract

Objective: Wearing  glove is one form of self protection from blood, saliva and other body fluids secretion exposure or when health worker’s hand in contact with patient’s mucous membrane during treatment/procedure. The type of glove material could affect its ability to prevent bacterial contamination on hand and the most common type of gloves used are nitrile and latex based material.Material and Methods: This is an experimental research with a post-test only design approach. The sampling technique used is Purposive Sampling. In this study, sample were divided into 2 groups consisting of 18 clinical students wearing latex and nitrile gloves while performing dental extraction on either anterior or posterior tooth.Results: The mean results obtained from latex glove are 88.82±168.56 CFU/ml which is higher than the nitrile group 0.00±0.00 CFU/ml. Based on the Mann-Whitney test results, the p value=0.01<0.05 where there are significant difference between latex and nitrile glove.Conclusion: Based on results of research and data analysis, it can be concluded that there are significant differences in the number of bacterial colonies in latex and nitrile gloves where the use of nitrile gloves are more effective than the latex gloves.

Abstract

Objective: To compare post-odontectomy wound healing time using silk and catgut sutures at Pirngadi Hospital from May to June 2018.Material and Methods: This is anon-laboratory experimental study with a “post-test only”design. Wound healing scores was assessed on the first and seventh days post odotectomy by seeing signs of inflammation. The number of patients studied was 30 people which 15 people used silk and 15 people used catgut.Results: The results were statistically analyzed using Mann-Whitney Test. There was no significant relationship (p> 0.05) between the wound healing time to the suture type on the first day post odontectomy. On the seventh day, it was found that there was a significant relationship (p <0.05) which silk had a score of 1.40 ± 0.507 while catgut had a score of 1.07 ± 0.258.Conclusion: Catgut has a better wound healing time than silk.
