Abstract

Critical congenital heart disease (CCHD) represents a severe subset of congenital heart defects (CHD) with high neonatal mortality rates if undiagnosed or untreated. This review highlights the prevalence, diagnosis, and management strategies for CCHD. In Asia, CHD prevalence is among the highest globally, contributing significantly to neonatal mortality. Early detection, such as prenatal ultrasound and pulse oximetry screening, plays a crucial role in improving patient outcomes. While fetal echocardiography provides diagnostic insights, it has limitations, emphasizing the value of postnatal pulse oximetry as a cost-effective, non-invasive screening tool. Management involves early interventions, including hemodynamic stabilization, pharmacological therapies like prostaglandin infusion, and surgical or catheter-based procedures. Definitive corrective surgeries, although complex, are increasingly performed in neonates to minimize long-term complications. Early diagnosis and timely referral to advanced facilities remain vital to reducing mortality and enhancing the quality of life for affected neonates.

Abstract

A foreign body, especially an organic type, causes eye injuries due to the infectious reactions it may provoke. Therefore, it is crucial to promptly identify and locate the foreign body to facilitate its removal and prevent undesirable complications. This case report highlights the importance of establishing a diagnosis and providing appropriate management for a case of conjunctival foreign body in the palpebral area. This case involved a 50-year-old male patient presenting with complaints of discomfort and pain in the left eye, particularly when closing the eye. The patient reported a history of being struck in the left eye by a rice stalk two days ago. The visual acuity of the right and left eyes was recorded as 6/6, with intraocular pressure of 18 mmHg using a Schiotz tonometer. A fragment of rice stalk was found embedded in the inferior palpebral conjunctiva. In the eyelid eversion examination, there was no foreign body. The foreign body was only visible when the patient gazed to the left while we pulled down the lower eyelid. In conclusion, in this case, foreign body extraction was performed, followed by topical antibiotic therapy and analgesia. The patient was advised to return for a follow-up in one month.

Abstract

The first year of college is a difficult time for new students. New students are required to have good endurance because it is one of the important factors that must be possessed by individuals to adapt well. Agarwood is known for its fragrant aroma, so it is widely used in aromatherapy. Special compounds contained in agarwood essential oil have a relaxing effect that can reduce fatigue levels. This study aims to examine the effect of Gyrinops versteegii essential oil aromatherapy on the work endurance in first-year students at the Faculty of Medicine, University of Muhammadiyah Surabaya. An experimental crossover design involving 32 first-year students. Data was collected through Inhalation of Gyrinops versteegii essential oil vapor from a diffuser, vital sign measurement, and Kraepelin test results. The data obtained will be processed using SPSS version 27 for Windows. Wilcoxon test results showed a significant difference in work endurance (p = 0.025). Kendall's Tau-b correlation test revealed a non-significant and weak correlation between work endurance (p = 0.071; r = 0.284). In conclusion, aromatherapy with Gyrinops versteegii essential oil has an impact on work endurance, with a significant decrease in work endurance in first-year medical students.

Abstract

Bloodstream infections (BSIs) caused by extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae represent a growing concern in healthcare settings, particularly in tertiary hospitals. These infections are associated with limited therapeutic options, increased mortality, and rising healthcare costs. This study aimed to describe the clinical, microbiological, and economic characteristics of adult inpatients diagnosed with hospital-acquired (HA) BSIs due to ESBL-producing Enterobacteriaceae at Dr. Soetomo General Academic Hospital from January 1 to December 31, 2024. A cross-sectional study was conducted using retrospective data from adult inpatients (≥18 years) with confirmed HA-BSIs caused by ESBL-producing Escherichia coli, Klebsiella pneumoniae, or Proteus mirabilis. Only the first positive blood culture per patient was analyzed. Community-acquired infections and non-Enterobacteriaceae organisms were excluded. The most frequent pathogen was Escherichia coli ESBL, especially among patients from medical wards. BSIs occurred predominantly in medical and intensive care units. Notably, microbiological confirmation of ESBL-producing organisms led to higher claim reimbursements through the national health insurance system, yielding a positive cost margin despite longer hospital stays and expensive treatment regimens. In conclusion, ESBL-producing Enterobacteriaceae remain a major challenge in managing HA-BSIs at Dr. Soetomo General Academic Hospital. While clinically burdensome, accurate microbiological diagnosis also supports optimal resource utilization and insurance claim efficiency in the hospital setting.

Abstract

Patent Ductus Arteriosus (PDA) is a disease often found in premature babies characterized by failure or delay in the closure of the Ductus Arteriosus (DA). PDA frequently occurs in babies born prematurely and is caused by high levels of prostaglandin E2 (PGE2) due to incomplete closure of the ductus and lung function. This condition can disrupt hemodynamics, so it needs to be closed. Closure can be done using cyclooxygenase (COX) inhibitor drugs, such as Indomethacin and Ibuprofen. Apart from that, paracetamol can also be used as an alternative to trigger the closure of the PDA. This study aims to establish the correlation between the medical intervention cycle and the efficacy of closing Patent Ductus Arteriosus (PDA) in preterm infants. The investigation employed an observational, retrospective, analytical approach along with a cross-sectional layout. The sampling technique in this research was total sampling. The sample obtained was 24 newborns, which was secondary data in the form of medical records obtained at the medical records installation of RSUP Haji Adam Malik Medan and Mother and Child Stella Maris Hospital Medan. All data were then processed and analyzed, and statistical tests were carried out using Spearman correlation tests with the SPSS application. The outcomes of this investigation, utilizing the Spearman correlation analysis, revealed a significant value of p = 0.001 (p<0.05) concerning the association between the duration of treatment and treatment efficacy and a significant value of p = <0.001 (p<0.05) concerning the treatment cycle and treatment efficacy. Thus, there was no relationship between the treatment cycle and the success of the treatment.

Abstract

Severe dengue virus infection (DVI) is characterized by thrombocytopenia and plasma leakage (PL) due to the release of vasoactive cytokines by immune cells such as monocytes, lymphocytes, neutrophils, and platelets in response to dengue virus (DENV). This immune response triggers systemic inflammation, altering monocyte, lymphocyte, and neutrophil kinetics, which in turn affects leukocyte differentials. PL is indicated by an increase in hematocrit (Hct). This study aims to evaluate the potential use of the Aggregate Index of Systemic Inflammation (AISI) as an indicator of plasma leakage. A correlative analytical study with a cross-sectional approach was conducted on 70 pediatric DVI patients diagnosed according to the 2009 WHO criteria and confirmed by anti-DENV IgM/IgG tests. Patients with a history of malignancy, autoimmune thrombocytopenic purpura, or secondary DVI were excluded. Absolute neutrophil, monocyte, platelet, and lymphocyte counts, AISI, and Hct levels were obtained from complete blood count results using a hematology analyzer. The results showed that AISI and lymphocytes correlated with Hct, with correlation values of (r=0.410, p<0.001) and (r=0.446, p<0.001), respectively. In contrast, monocytes, neutrophils, and platelets did not correlate with Hct, with correlation values of (r=-0.009, p=0.942), (r=-0.059, p=0.628), and (r=-0.175, p=0.147), respectively. In conclusion, AISI levels in DVI were low and negatively correlated with Hct, a known indicator of PL. These findings suggest that AISI has potential as a marker for PL and disease severity in DVI.

Abstract

Pneumonia was a communicable respiratory infection associated with substantial morbidity and mortality across diverse age groups globally. Transmission occurred via direct exposure to respiratory secretions or aerosolized droplets expelled by infected individuals through coughing or sneezing. Previous investigations on antibiotic susceptibility patterns of pneumonia-causing bacterial pathogens were conducted at RSUD Dr. Soetomo Surabaya (2020) and RSUD Dr. Saiful Anwar Malang (2022). This study aimed to analyze and compare the antimicrobial susceptibility profiles of bacterial isolates from hospitalized pneumonia patients at RSUD Mardi Waluyo, Blitar City. A quantitative methodology was employed, using an observational analytic design with a cross-sectional approach to assess antibiotic resistance patterns. Statistical analysis yielded a p-value of 0.002 (p < 0.05), indicating a significant difference in antimicrobial effectiveness between Gram-positive and Gram-negative bacterial infections. The majority of patients were males over 60 years old. Staphylococcus aureus was identified as the predominant Gram-positive pathogen, while Klebsiella pneumoniae was the most prevalent Gram-negative species. Azithromycin, Erythromycin, and Levofloxacin showed the highest susceptibility among Gram-positive bacteria, whereas Amikacin, Ceftazidime, and Gentamicin were most effective against Gram-negative isolates.

Abstract

This study focuses on assessing the effectiveness of digital marketing applications at X Hospital Mojokerto by reviewing patient demographics, online behavior, and perceptions of digital media. A quantitative approach using a survey method was used, with a questionnaire consisting of several questions, to collect data from new patients who access health information through the hospital's digital media. The results indicate that patients aged 26–35 years dominate, and TikTok and Instagram are shown as the primary platforms used for accessing health information. Nonetheless, only Facebook indicated a statistically meaningful result on patients' choices in seeking hospital care. The remaining variables, such as demographic factors and digital media perception, demonstrated no significant impact. The study implies that digital platforms' usage behavior and content strategy bring a greater impact than demographic characteristics. Recommendations cover maximizing Facebook performance, improving content interaction on Instagram and TikTok, and implementing communication strategies tailored to user behavior to strengthen communication outcomes and conversion.

Abstract

Subdural Hematoma (SDH) is a common pathological condition characterized by venous bleeding in the subdural space between the dura mater and the arachnoid membrane. Computed tomography (CT) scan plays a crucial role in diagnosing SDH and assessing the severity of the hemorrhage by estimating the hematoma volume.  Various measurement techniques, including the ABC/2 formula and volumetric analysis, are used to determine SDH volume. This study aimed to compare the SDH volume results obtained from the ABC/2 technique and the volumetric technique using Philips Access CT V2.0 computer software. A cross-sectional study was conducted involving 30 patients diagnosed with SDH based on CT scan findings at Siti Khodijah Muhammadiyah Hospital, Sepanjang, Sidoarjo. Volume measurements were performed by a radiologist using the hospital’s radiology computer. The ABC/2 technique involved manual measurement using linear dimensions, while the volumetric technique utilized Philips Access CT V2.0 computer software to obtain three-dimensional volume estimates. Data were analyzed using the Wilcoxon signed-rank test to determine statistical differences between the two techniques. The analysis showed no significant difference between the volumes measured by the ABC/2 and the volumetric techniques (p = 0.382; p > 0.05). In conclusion, the ABC/2 technique remains a practical and time-efficient method for estimating SDH volume, especially in clinical settings where rapid assessment is required.

Abstract

Employee job satisfaction is a crucial aspect in the health sector, which directly impacts the quality of patient care. In hospitals, job satisfaction is often affected by key factors such as leadership style and workload. According to data from the Central Statistics Agency of 2021, job satisfaction in the health sector is lower than in other sectors, with only 65% employees expressing satisfaction with their work. This study aims to analyze the effect of participative leadership style and workload on employee job satisfaction in the outpatient unit of Gresik Petrokimia Hospital. This current research employed a quantitative method with a cross-sectional design. Data were collected using a questionnaire distributed to 41 respondents, and then analyzed using multivariate multiple linear regression with SPSS version 25. The findings revealed that the participative leadership style had no significant effect on job satisfaction. However, workload partially had a significant effect. When assessed together, both participative leadership style and workload demonstrate a statistically significant influence on job satisfaction.
