Abstract

Stroke is the number one cause of disability in Indonesia. Stroke is a condition of loss of brain function caused by stopping the blood supply to the brain. The main risk factor for stroke is hypertension. Hypertension sufferers who have poor preventive behavior have the potential to experience a stroke. This study aimed to identify the dominant factors related to stroke prevention behavior based on the Health Promotion Model (HPM) theory in the Gubeng District area of ​​Surabaya. This research used cross-sectional research with a sample of 108 respondents using purposive sampling. Data were collected using a questionnaire and analyzed using the logistic regression test α < 0.05. Independent factors related to stroke prevention behavior are perceived self-efficacy (0.000), activity-related affect (0.002), personal influences (0.000), and situational influences (0.023). Meanwhile, factors that have no relationship are prior related behavior (0.360), personal factor (motivation) (0.319), perceived benefits to action (0.200), perceived barriers to action (0.243), education level (0.708), socio-economic status (0.105). In conclusion, this research can be used as material for developing health workers' knowledge in intervening related to stroke prevention behavior in hypertension sufferers.

Abstract

First-year medical students tend to experience stress, which can affect how they maintain focus on their work. One alternative treatment that can be done is through relaxation techniques using aromatherapy in the form of essential oils. Crocodile agarwood (Aetoxylon sympetalum) is one of the plants that can provide anxiolytic and antidepressant effects that can be associated with these problems. This study aims to determine the effect of giving Aetoxylon sympetalum essential oil on the level of ability to maintain the focus of first-year students of the medical faculty. This quasi-experiment research method with a nonequivalent control group design was conducted on 34 samples which were divided into control and treatment groups with the same number to see the effect of essential oil on both groups. The research data were analyzed using Spearman's rho test and showed a significance value of 0.004 and a correlation coefficient value of 0.661, meaning that a significant correlation was obtained, the level of correlation between variables was strong and unidirectional. So it can be concluded that the more often inhaling Aetoxylon sympetalum essential oil, the ability to maintain focus will increase.

Abstract

We report a case of a 50-year-old female who presented with rhinosinusitis-like symptoms, which she had been experiencing intermittently for over two decades. Her medical history revealed a nasal surgery performed approximately 22 years ago, after which she continued to experience unresolved symptoms despite multiple treatments. A Computed Tomography (CT) scan of the paranasal sinuses revealed findings with ethmoiditis, prompting the decision for endoscopic sinus surgery to address the underlying pathology. During surgery, a 3 cm gauze was discovered after opening the posterior ethmoid sinus. We highlight an unexpected foreign body inside the ethmoid sinus based on an unidentified picture from the CT scan, which we accidentally discovered during endoscopic surgery. This case underscores the importance of considering a retained foreign body as a potential cause of chronic rhinosinusitis, even many years after the initial surgical intervention. The unexpected identification of the gauze, which was not depicted in the preoperative CT scan, highlights the challenges in diagnosing retained foreign bodies within the complex anatomy of the sinuses. The report discusses the clinical presentation, diagnostic challenges, and management of this unusual case, contributing valuable insights to the literature on chronic rhinosinusitis and retained foreign bodies in the sinonasal region.

Abstract

Maternal and fetal mortality is 31.15% due to undetected preeclampsia leading to eclampsia. Pre-occurrence of eclampsia is often characterized by severe preeclampsia symptoms such as increased blood pressure, and proteinuria, furthermore, preeclampsia is also divided into 2 types, based on the time of symptom onset, in early-onset type preeclampsia appears at <34 weeks of gestation while for late-onset appears at >34 weeks of gestation. Extreme age and multigravida parity are risk factors for severe preeclampsia. This study aims to determine the relationship between age and parity with late-onset severe preeclampsia. This study used a cross-sectional study design, sampling method used nonprobability sampling techniques with observational analysis on pregnant women diagnosed with preeclampsia at PKU Muhammadiyah Surabaya Hospital for January 2022 - November 2023. The data collection instrument used medical record data, which was analyzed using Chi-Square. In the analysis obtained for the relationship between age and preeclampsia, the large p-value is 0.16 <0.05. The relationship of parity with preeclampsia is 0.004 < 0.05, thus proving a relationship between age and parity with late-onset severe preeclampsia at PKU Muhammadiyah Surabaya Hospital. In conclusion, there is a relationship between age and parity with late-onset severe preeclampsia at PKU Muhammadiyah Surabaya Hospital.

Abstract

Nutritional status is of particular concern in both developed and developing countries. The prevalence of overnutrition and obesity, especially in adolescents, continues to increase. Various parameters can be used to assess a person's physical fitness. Hand grip strength is one of the parameters used to evaluate nutritional status and physical fitness and predict various metabolic diseases. This study aims to determine the relationship between Body Mass Index (BMI) and hand grip strength in adolescents aged 13-16. The research design was analytic observation with a cross-sectional approach at SMP Negeri 7 Denpasar—sampling technique with simple random sampling of as many as 100 respondents based on inclusion and exclusion criteria. The nutritional status criteria are guided by the Indonesian Ministry of Health, namely underweight (-3SD to -2SD), normal (-2SD to +1SD), overweight (+1SD to +2SD), and obesity >+2SD. The classification of hand grip strength that we use includes Weak (< 26.6 kg), normal (26.6 kg), and strong (> 26.6 kg) for men, while for women, they are weak (< 27.1 kg), normal (27.1 kg), strong (> 27.1 kg). Data were analyzed using the Chi-Square Test. The results obtained in this study include the majority of 13-year-olds (55%), male gender (56%), weak hand grip strength (54%), and normal BMI (43%). The mean BMI was 20.49±4.24, and the mean hand grip strength was 26.74±9.33. Based on the Chi-Square test, there was a significant relationship (p<0.001) between BMI and hand grip strength. Thus, this study can conclude that a substantial relationship exists between BMI and hand grip strength in adolescents aged 13-16.

Abstract

Based on WHO data, almost 20%-30% of hospital patients in developing countries experience mental emotional disorders such as depression. Depression occurs from mild to severe, thus interfering with daily functioning. The most effective therapy for severe depression is electroconvulsant therapy (ECT). But in some cases it cannot be done, so there needs to be alternative therapies. Acupuncture can trigger endorphins and lower cortisol levels that can affect a person's mood. In this case, we present A 50-year-old woman with a diagnosis of recurrent depressive disorder now severe episodes with psychotic symptoms was planned to receive ECT therapy, but the patient was not eligible. Then the patient is given drugs Fluoxetine 1x20 mg and Clozapine 1x50 mg. The patient is still silent, neither eating nor answering questions. Based on several journals found, acupuncture can be given as an additional therapy given with psychopharmaceuticals. Acupuncture is beneficial in faster improvement of depressive symptoms when compared to patients who only get antidepressants. After several acupuncture sessions, patient’s symptomps was improved. So we conclude that acupuncture can be used as an additional therapeutic option given together with psychopharmaceuticals to depressed patient.

Abstract

Osteoporosis is a global health issue. Fragility fractures, which occur due to minor trauma, are one of the serious consequences of osteoporosis. Aims: This study investigates the correlation between demographic characteristics and the level of knowledge about osteoporosis in patients with fragility fractures. This research used a descriptive design with a cross-sectional approach. The data was gathered through validated and reliable questionnaires. Respondents were patients with fragility fractures treated at Dr. Saiful Anwar General Hospital, Malang. Data analysis was performed using statistical tests to evaluate the correlation between age, gender, and knowledge about osteoporosis. The analysis revealed significant gender differences in osteoporosis knowledge (p-value = 0.037) but no substantial age-related disparities (p-value = 0.194). There were no significant differences in nutritional status based on gender (p-value = 0.881) or age (p-value = 0.521). Similarly, medication adherence showed no significant variance by gender (p-value = 0.883) or age (p-value = 0.589). Lastly, the understanding of osteoporosis did not significantly differ by gender (p-value = 0.467) or age (p-value = 0.782). In conclusion, the study reveals a significant gender disparity in osteoporosis knowledge, highlighting the need for tailored educational interventions and physician initiatives to ensure equitable awareness across diverse demographic groups.

Abstract

Nasopharyngeal carcinoma is endemic in Southeast Asia. In Indonesia, the recorded mean prevalence is 6.2/100.000, with 13.000 yearly new Nasopharyngeal carcinoma cases. Observational data suggest that cancer-specific mortality was higher during the COVID-19 pandemic when compared with pre-pandemic levels. This study aims to compare the treatment outcome of nasopharyngeal carcinoma cases before and during the COVID-19 pandemic at Dr Soetomo Hospital in 2017-2022. This study employed a retrospective cross-sectional approach with an observational analytic method. Data is taken secondary from medical records for nasopharyngeal carcinoma patients at Dr. Soetomo Hospital in Surabaya and primary by telephone interview for additional information on the patient’s risk factors. The majority of nasopharyngeal carcinoma patients at Dr. Soetomo Hospital were male, elderly,>50 years old, and referral patients (outside Surabaya). In both eras, patients arrived already in stage 4 (advanced stage). However, before COVID-19, more patients were cured and survived before the COVID-19 period in comparison to during the COVID-19 pandemic period. This may be related to delayed treatment caused by COVID-19 policy on social distancing, healthcare regulation during the pandemic, and immunocompromised status.

Abstract

Infertility is a condition in which a couple fails to achieve pregnancy after 12 months of unprotected sex. In Indonesia, the prevalence of infertility is estimated to be around 10-15%. Primary infertility occurs when a woman has never had a pregnancy. Hysterosalpingography (HSG) examination is a radiological modality that is often used to evaluate the condition of the uterus and fallopian tubes in infertile women. The case series features data from five female patients diagnosed with primary infertility, where each patient underwent an HSG examination, and the results of the analysis were based on radiological findings that included structural abnormalities in the uterus and fallopian tubes. The HSG findings showed various abnormalities such as hydrosalpinx, tubal occlusion, uterine arcuatus, uterine bicornuate, and salpingitis isthmica nodosa. Uterus arcuatus was found in two cases, while hydrosalpinx was detected in three cases. In addition, two cases showed the presence of bilateral occlusion of the fallopian tubes, which correlated with primary infertility. HSG examination is an effective modality for detecting various abnormalities in the female reproductive tract that contribute to primary infertility. Findings such as hydrosalpinx, tubal occlusion, and uterine anomalies play an important role in determining the right treatment strategy for patients.

Abstract

The high morbidity and mortality rates due to pneumonia in developing countries indicate that a more affordable marker is needed to determine the degree of inflammation in cases of pneumonia infection. The neutrophil-to-lymphocyte ratio (NLR) is a simple indicator used extensively in evaluating the systemic inflammatory response. An observational cross-sectional study of 297 pneumonia patients was conducted using medical records as the source document. The patients’ blood was taken during the hospital admission to obtain neutrophil and lymphocyte values. Meanwhile, the PSI score was used during the hospital admission to assess the severity of the pneumonia. Bivariate analysis using Spearman correlation was used to determine the correlation between NLR and pneumonia severity. The ROC curve was used to determine the cut-off value of NLR to distinguish mild from severe patients. The results of the Spearman correlation test analysis showed that the correlation between the NLR and the pneumonia severity was significantly positive (r = 0.130; p = 0.025). Furthermore, the median NLR value was higher in patients with severe pneumonia than in those with milder pneumonia severity (13.01 vs. 11.79 vs. 6.75). The NLR cut-off value of 9.07 was obtained to differentiate patients with mild severity and those with more severe disease. In conclusion, there is a correlation between the neutrophil-to-lymphocyte ratio and the severity of patients’ pneumonia.
