Abstract

The most common worm infections are those caused by soil-transmitted helminths (STH) infection. Soil contamination is one of the causes of helminth transmission to humans. Poor environmental sanitation and personal hygiene facilitate the spread and transmission of STH. Jember is a district where the majority of the population lives in rural areas with poor sanitation access. This study aims to determine the correlation between soil contamination and incident of STH infection in Sukokerto Village, Sukowono District, Jember Regency. This cross-sectional analytic observational study was conducted from August 2022 to February 2023. The total of samples in this study was 44 fecal and soil around the house. The data obtained from the laboratory examination showed that the total fecal positive of STH was 11 or 25%, and soil was 13 or 29.5%. The results of the Chi-Square test analysis obtained ρ<0.05 (ρ=0.036, OR=4,457). The results showed a significant relationship between soil contamination and the incidence of STH infection in Sukokerto Village, Sukowo District, Jember Regency

Abstract

Hypertension, often known as high blood pressure, is characterized by systolic blood pressure of at least 140 mmHg and/or diastolic blood pressure of at least 90 mmHg. Lack of regular physical activity is one of the risk factors for hypertension. This study aims to analyze the relationship between physical activity and the incidence of hypertension in adulthood. This study employs a cross-sectional design and analytical observational research methodology. The number of subjects in the study were 62 hypertension subjects and 62 non-hypertension subjects. Data was collected with the IPAQ-SF questionnaire to determine the level of physical activity. Data analysis was performed using the Chi-square statistical test. The results of the univariate analysis showed that the majority of research subjects were aged 41-60 years (53.2%), female (66.9%), and 91.5% of subjects with hypertension were classified as having low physical activity. There was a strong correlation between physical activity and the incidence of hypertension, as shown by the bivariate analysis, which yielded a value of p = 0,00 (p 0,05). The conclusion of this study was that there is a relationship between physical activity and the incidence of hypertension in adulthood in the working area of Tampaksiring I public health center.

Abstract

Noise Induced Hearing Loss (NIHL) is sensorineural deafness caused by hearing threshold values in the workplace. According to the Regulation of the Minister of Manpower and Transmigration Number 13 of 2011 it regulates the threshold and chemical factors. The threshold value is 85 dB with 8 hours/day noise exposure time. This study aimed to determine the relationship between work period and the incidence of NIHL in PT. Kayu Perkasa Raya. The type of research used was quantitative with observational analytic methods. Data was obtained from questionnaires and audiometric results. The research design used cross-sectional, a study between measurement and observation variables that were carried out simultaneously. The sampling technique was purposive sampling. The analysis used using chi-square. The distribution was based on a working period. During the working period of ≥ 5 years, there were 10 people (33.3%) who experienced NIHL, and 20 people (66.7%) who did not experience NIHL. During the working period < 5 years, two people (11.1%) experienced NIHL, and 16 people (88.9%) did not experience NIHL (p=0.081). In conclusion, no significant relationship exists between the work period and NIHL at PT.Kayu Perkasa Raya.

Abstract

Stroke is a metabolic illness that causes significant impairment in the working-age population. Disability develops due to the consequences of neurological deficiencies and the failure of the medical rehabilitation process. Post-stroke pain is one of the causes of this failure. In a post-stroke patient, pain is an unpleasant physical and emotional experience. In contrast, range of motion barriers might cause the medical rehabilitation procedure to fail. This paper aims to review the various methods of post-stroke pain management that can be used as an alternative therapy that helps post-stroke patients' rehabilitation. The PubMed database was used to search for different kinds of literature. The study includes clinical studies, pilot studies, and randomized control trials published between January 2015 and June 2023. The authors omit several publications to ensure that the final selection of papers includes only the most relevant and reputable sources of information on post-stroke pain, post-stroke pain management, and pain as a measure of outcome. There are 28 publications to be reviewed. The most prevalent cause of post-stroke pain was hemiplegic shoulder pain in numerous studies that have an impact on the post-stroke recovery process. In conclusion, the options for post-stroke therapy range from conservative rehabilitation to interventional therapy. Several innovative experimental rehabilitation treatment approaches have been studied. However, the findings do not outperform conventional rehabilitation treatment.

Abstract

Congenital pseudarthrosis of the tibia (CPT) is an infrequently observed condition. However, it is considered to be one of the most complex ailments in the field of pediatric orthopedics. In cases where a tibial fracture is absent, the conventional treatment approach involves using bracing as a preventive measure against fractures. Various surgical modalities have been proposed for the management of CPT, including intramedullary (IM) rod fixation, external fixators, on-lay grafts, cancellous grafts, periosteal grafts, Paley cross-union, amputation, vascularized fibula grafts, and induced membrane techniques. These modalities can be used in different combinations and permutations. The need for repetitive surgical interventions due to unsuccessful therapeutic outcomes or modifications in secondary treatments can significantly disrupt the childhood experience, resulting in prolonged and recurrent functional limitations. In certain instances, amputation may be required as a primary or secondary treatment modality. This literature review will clarify the current surgical interventions for CPT disease based on a summary of research data. The surgery the patient receives significantly impacts their general health and capacity to heal.

Abstract

Skin can reflect systemic conditions due to abnormalities in internal organs, including kidney disorders. Cutaneous manifestations are common in patients with end-stage renal disease (ESRD). It can be severe and negatively impact a patient's quality of life. Clinicians can effectively manage ESRD by closely examining the patient's skin and nails, improving the patient's quality of life, and reducing mortality and morbidity. Clinicians can be more aware of ESRD by examining the patient's skin and nails closely so proper management can be performed and the patient's quality of life can be improved. This review aims to increase understanding of common cutaneous manifestations in ESRD for early recognition and better management.  Cutaneous manifestations in ESRD are divided into specific and nonspecific manifestations. Specific manifestations include acquired perforating dermatosis (APD), bullous disease (porphyria cutanea tarda and pseudoporphyria), metastatic calcification (calcinosis cutis and calcific uremic arteriolopathy), and nephrogenic systemic fibrosis. Nonspecific manifestations include pruritus, xerosis, skin pigmentation changes, nail disorders, purpura, hair disorders, oral mucosal changes, skin infections, and other skin manifestations. These manifestations range from benign and asymptomatic to serious conditions that negatively impact life quality. In conclusion, Early detection and treatment of cutaneous manifestations in patients with ESRD are crucial for reducing morbidity and mortality and also improving patients' quality of life.

Abstract

Stress, anxiety, and depression in tourism workers have increased during the COVID-19 pandemic, which are mental health problems that need to be watched out for. Mental health has become one of the most significant problems in the world, including Indonesia, especially during the COVID-19 pandemic. Seeing the significant role of mental health disorders in tourism workers accompanied by stress, anxiety, and depression can have an impact on the health of human resources in the tourism sector. The purpose of this study was to determine the level of stress, anxiety, and depression in tourism workers in the province of Bali during the COVID-19 pandemic. The sample of this study was 103 people aged 18-60 years who are active tourism workers in the Province of Bali, which was selected by convenience sampling using Google Forms. The research method was cross-sectional and analyzed using SPSS version 27. The study results showed that most of the research samples experienced severe stress, as many as 38 (36.9%). The majority of the study sample experienced very severe anxiety with 74 people (71.8%). Most of the research samples experienced very severe depression, with 65 respondents (63.1%). In conclusion, tourism sector workers suffer severe stress, anxiety, and depression during the COVID-19 pandemic.

Abstract

The immune-complex mediated inflammation known as post-streptococcal glomerulonephritis (PSGN) was once thought to be one of the most common causes of acute nephritis in children. In this case, we present a 12-year-old girl experiencing symptoms of cough phlegm, congested breath, change of urine color, decreased urine production, as well swelling in the area face and legs after experiencing fever, pain swallowing, cough, and runny nose 3 weeks before admission. On physical examination, the pressure blood was 130/80 mmHg. In addition, the palpebral and extremity were edema bilateral, hyperemia of the tonsil and pharynx, enlarged tonsils T2/T3, and detritus were found. Laboratory and Imaging Tests: hematology; leukocytes 12,930, albumin 2.4 g/dl, on urinalysis, obtained urine cloudy, brownish, leukocyturia, hematuria, and proteinuria, there was also an increase in ASTO titers, abdominal ultrasound found bilateral renal artery stenosis and nephritis bilateral acute. The patient in this case presented with bilateral renal artery stenosis, which is a rare finding in PSGN cases. Management of PSGN include bed rest, low salt diet, fluid balance, as well supportive treatment with IVFD D5 ½ NS 15 drops/min (micro), Antibiotics injection of Ceftriaxone 1 gram/12 hours on the first day and continued with oral Erythromycin 4x500mg, injection prednisolone 3x1, injection Furosemide 1x1 amp, oral spironolactone and sublingual nifedipine, and hypoalbuminemia correction. Prognosis in patients with the given GNAPS governance optimally will give good results.

Abstract

Anxiety is a global psychiatric phenomenon, while poor sleep quality remains a pressing public health issue. The arduous medical education journey exposes students to prolonged stress, potentially exacerbating anxiety and sleep disturbances. This study investigates the correlation between anxiety and sleep quality in Hang Tuah Faculty of Medicine students, bridging the pre-clinical and clinical phases. Employing an analytical observational approach with a cross-sectional design, this study enrolled students from pre-clinical (batches 2020, 2021, 2022) and clinical years (batches DM45, DM46, DM47). Among the 409 approached participants, 296 met the inclusion criteria after excluding 113 samples. Data collection utilized online questionnaires with acquired informed consent. Sleep quality was evaluated via the Pittsburgh Sleep Quality Index, generating scores from 0 to 21. This study showed significantly robust evidence of an association between anxiety and sleep quality (p-value: 0.0001; r: 0.342) across both pre-clinical and clinical students. This study underscores a direct relationship: elevated anxiety levels correspond to diminished sleep quality. Such findings resonate with the prevailing challenges in medical education and their impact on students' mental and physical well-being. Implementing targeted interventions is crucial to address these concerns. In conclusion, this study underscores the undeniable link between anxiety and sleep quality among Hang Tuah Faculty of Medicine students, encompassing the years 2017 to 2022. The implications are far-reaching, emphasizing the necessity of interventions that enhance both psychological well-being and sleep quality. By recognizing and addressing these challenges, medical institutions can foster healthier academic environments and better equip students for their medical careers.

Abstract

Transient synovitis (TS), an abrupt, non-specific synovium inflammation, is the most prevalent cause of pediatric hip discomfort. Transient synovitis affects 3–10-year-olds. This disease is benign and self-limiting, but clinicians must distinguish it from an acute infection. After ruling out other causes of hip discomfort and limping, clinicians detect transient synovitis. Based on the patient's history, physical examination, and hip radiography, septic arthritis (SA) and TS care are the most likely causes. Clinicians must consider clinical factors and order appropriate lab and imaging tests to rule out alternative illnesses. This review aims to determine how to propose TS hip disease. The databases used were Scopus and PubMed. We included seven articles in our review. Clinical symptoms, lab testing, and radiographs determine the diagnosis. Ultrasound is a useful TS diagnostic tool since it is widely available, and children are not directly exposed to ionizing radiation. If TS and SA illnesses, which are commonly linked, are unclear, detecting bone marrow abnormalities, soft tissue lesions, synovial enhancement, and joint effusion by MRI can be beneficial. Laboratory tests help distinguish TS from SA because there are more extensive evaluations. Additionally, the healthcare provider performs a hip joint aspiration examination under ultrasound or fluoroscopy.
