Abstract

Tujuan: Untuk mengetahui manfaat miolisis dengan radiofrequency thermal ablation terhadap mioma uteri dan adenomiosis. Tempat: RSUPN Dr. Cipto Mangunkusumo, Jakarta. Rancangan/rumusan data: Penelitian ini bersifat deskriptif. Bahan dan cara kerja: Delapan orang pasien yang menderita mioma uteri dan atau adenomiosis bergejala menjalani miolisis dengan radiofrequency thermal ablation baik transvaginal maupun per laparoskopik. Satu bulan pascaoperasi, pasien dievaluasi kembali ukuran massa dengan ultrasonografi dan perubahan gejala yang berkaitan dengan kedua patologi uterus tersebut. Hasil: Dari pasien yang diteliti, 5 pasien (62,5%) menderita adenomiosis, dan 3 pasien (37,5%) menderita mioma uteri. Rata-rata diameter dan volume massa paling besar per pasien berturut-turut adalah 4,6 cm (1,4 - 9,0) dan 694,3 cm3 (11,5 - 3061,8). Tujuh pasien (87,5%) mengeluh dismenorea, dan hanya 1 pasien mengeluh menorragia. Tiga pasien (37,5%) menjalani miolisis laparoskopik. Tidak terdapat komplikasi intraoperatif atau pascaoperatif. Rata-rata reduksi volume massa pada follow-up 1 bulan adalah 67,5%; reduksi mioma uteri mencapai 81,5%; sedangkan adenomiosis 59,1%. Pada follow-up tersebut, semua pasien menyatakan keluhan dismenorea atau menorragia menghilang. Kesimpulan: Pada penelitian pendahuluan ini, miolisis dengan radiofrequency thermal ablation telah berhasil mengurangi volume mioma uteri dan adenomiosis serta menghilangkan gejalanya. Diperlukan follow-up serial dan penelitian tambahan untuk menilai efikasi dan keamanan teknik ini. Objectives: To improve skill in making a diagnosis and management of pseudomyxoma peritoneum originating borderline mucinous ovarian tumor during pregnancy. Methods: Case report. Abstract

Objective: To determine the correlation between C‐telopeptide with osteoporosis risk by using Osteoporosis Self Assessment Tools for Asian (OSTA) in postmenopausal women. Method: An analytic cross‐sectional study of 31 postmenopausal women in Prof. Dr. RD Kandou Hospital, Manado. Samples were collected through consecutive sampling. Data was analyzed using Pearson test with significance level of p>0.05. Result: Our sample consisted of 31 postmenopausal women. Mean OSTA score is ‐1.02K2.54. Sixteen women (51.61%) were deemed to have low risk, 8 women (25.81%) deemed to have moderate risk and 7 women (22.58%) deemed high risk based on the OSTA score. The mean C‐telopeptide plasma level of our sample is 0.52K0.25 ????g/l. We found plasma C‐telopeptide level and OSTA score were normally distributed. Correlation analysis using Pearson test identified a significant negative correlation between plasma C‐telopeptide level and OSTA score (r=‐0.768; p=0.001). Conclusion: There is a significant negative correlation between plasma C‐telopeptide level with risk of osteoporosis based on OSTA score in postmenopausal women. Abstract

Objective: To evaluate whether Anti Müllerian Hormone (AMH) can be used as a predictor of metabolic syndrome in Polycystic Ovarian Syndrome (PCOS).Methods: This cross-sectional study was conducted in Yasmin Clinic, Dr. Cipto Mangunkusumo General Hospital Jakarta between June and December 2012. Forty-one patients diagnosed with PCOSbased on Rotterdam Criteria were enrolled. Secondary were was taken from medical record..Results: A total of 22 subjects were involved in this study. Mean AMH level in the metabolic syndrome group is compared to the non-metabolic syndrome group (10.72 ± 6.23 ng/ml vs 7.97 ± 4.50ng/ml, p=0.12). AMH was strongly associated with HDL, triglyceride and insulin resistance (r-value of -0.29, 0.23, and 0.21 respectively, p < 0.05).Conclusion: AMH can be used as a predictor of metabolic syndrome in PCOS.[Indones J Obstet Gynecol 2017; 5-2: 83-86]Keywords: anti müllerian hormone, metabolic syndrome, polycystic ovarian syndrome

Abstract

Objective: To improve diagnostic method to predict ovulation in cycle with clomiphene citrate. Method: This diagnostic research was done in RSUPN Dr. Cipto Mangunkusumo between January 2011 - October 2011 with 31 women who taking CC 50 mg/day and 30 women with normal cycle. Result: Affecting with ovulation in the two groups, it was found that in groups with CC, the follicle diameter is 24.33 ± 3.87 mm with LH 14.21 ± 7.95 IU/l, meanwhile in normal group, follicular diameter is 17.62 ± 3.45 mm with LH 14.42 ± 5.91 IU/l. Cut-off point for follicular diameter simultaneously with ovulation determined by ROC curve was found in 24.33mm (AUC 0.67, sensitivity 0.64, specificity 0.56), meanwhile the cut-off for LH is 14.40 IU/l (AUC 0.61, sensitivity 0.57, specificity 0.43). Conclusion: In group with CC, ovulation occurred in bigger follicular diameter than normal cycle, while LH does not different significantly. Abstract

Tujuan: Mengetahui ada tidaknya penurunan kekuatan otot dasar panggul selama kehamilan, yang dapat menjadi salah satu penyebab terjadinya stres inkontinensia urin. Tempat: Poliklinik Obstetri Rumah Sakit Umum Pusat Nasional Dr. Cipto Mangunkusumo. Bahan dan cara kerja: Subjek penelitian adalah primigravida. Penilaian dilakukan sebanyak dua kali, masing-masing pada kehamilan 20-28 minggu dan 29-36 minggu. Kekuatan otot dasar panggul diukur dengan alat myofeedback, yaitu Myomed 932 (Enraf Nonius, The Nederlands). Pasien melakukan 3 kontraksi maksimal dengan interval istirahat di antaranya. Dari 3 kontraksi tersebut diambil rata-ratanya dalam satuan hPa dan dijadikan sebagai nilai kekuatan otot dasar panggul. Hasil: Selama Oktober 2006 hingga Mei 2007, diperoleh 67 subjek yang memenuhi kriteria penelitian. Karakteristik subjek penelitian: 80,6% berada dalam kelompok umur 18-30 tahun. Hampir berimbang kelompok subjek yang berpendidikan menengah (56,7%) dan tinggi (43,3%). Sebagian besar adalah ibu rumah tangga (61,2%). Tiga kelompok suku bangsa terbanyak yaitu Jawa (29,8%), Betawi (26,9%) dan Sunda (17,9%). Dengan uji t tidak berpasangan, ditemukan bahwa kekuatan otot dasar panggul primigravida pada trimester II dan trimester III tidak terdapat perbedaan bermakna (p = 0,936). Rerata pada trimester II yaitu 30,76 ± 9,60 hPa dan pada trimester III yaitu 30,90 ± 9,67 hPa. Rerata pada seluruh kehamilan yaitu 30,83 ± 9,60 hPa. Kesimpulan: Kecenderungan kekuatan otot dasar panggul yang diukur dengan alat Myomed 932 pada primigravida kehamilan trimester II adalah 30,76 ± 9,60 hPa dan trimester III adalah 30,90 ± 9,67 hPa. Tidak terdapat perbedaan bermakna kekuatan otot dasar panggul pada primigravida kehamilan trimester II dan trimester III. Abstract

Objective: To investigate the cause of postpartum urinary incontinence in primigravida women and to determine the relationship between stress urinary incontinence and kegel’s exercise. Method: This was an observational study with cross sectional method, performed in institutional hospitals in Makassar. Thirty primigravida women who performed kegel’s exercise were compared with 30 primigravida women who did not perform kegel’s exercise. Data were analyzed with independent-t statistical analysis. Result: Urinary incontinence were significantly lower in primigravida women who performed kegel’s exercise (P=0.000). Conclusion: Kegel’s exercise may reduce the incidence of postpartum stress urinary incontinence. Abstract

Objective: To determine the association between BRCA1 gene Q356R (1186A=G) polymorphism and epithelial ovarian cancer incidence. Methods: This study is an observational analytic study with casecontrol study design. All patients diagnosed with epithelial ovarian cancer that were treated in the outpatient clinic and inpatient ward of the Department of Obstetrics and Gynecology, Dr. Mohammad Hoesin Hospital, Palembang who met the inclusion criteria were included in this study. DNA extraction was performed on blood samples, followed by PCR-RFLP process. Results: We obtained the genotype distribution of BRCA1 Q356R (1186A=G) polymorphisms to be QQ genotype (wild-type) on all 50 subjects in the case group (100%) and 50 control subjects (100%). Similarly, all BRCA1 alleles have the Q allele. The results of this study found no polymorphism of the BRCA1 Q356R (1186A=G) in the ovarian cancer and control groups. Conclusion: Polymorphism of BRCA1 gene Q356R (1186A=G) was not significantly associated with epithelial ovarian cancer incidence. Abstract

Objective: Unicornuate uterus with rudimentary cavity is anextremely rare Mullerian duct anomaly. It is frequently misdiagnosedor under treated, since women who suffer from thiscondition tend to be asymptomatic. We aimed to report thisinteresting case and review the available background literature.Methods: Case report.Case: A thirty two-year-old nulligravid woman was presentedwith severe abdominal pain. Laparoscopic surgery demonstratedunicornuate uterus with arudimentary cavity and hematometra.Partial hysterectomy was conducted to the right rudimentaryuterus.Conclusion: Thorough diagnosis is essential to prevent thecomplications of unicornuate uterus. Unicornuate uterus withrudimentary cavity is associated with increased rate of pregnancycomplications such as preterm labor and fetal malpresentation.Pregnancy can also occurred on the rudimentary cavity and leadsto recurrent pregnancy loss. Laparoscopy is essential for confirmatorydiagnosis. Abstract

Objectives: To compare the oocyte count, embryo count, pregnancy rate, and the presence of OHSS in patients undergoing In Vitro Fertilization (IVF) with long and short protocols at Yasmin Clinic, Dr. Cipto Mangunkusumo Hospital, Jakarta. Methods: A cross sectional study was done at Yasmin Reproductive Clinic, Dr. Cipto Mangunkusumo Hospital, Jakarta, Indonesia. Data was collected from the medical records of 200 infertile women undergoing IVF, 100 women with long protocols and the other 100 with short protocols. Variables compared in these 2 groups are the oocyte count, embryo count, the pregnancy rate, and presence OHSS using Mann-Whitney U Test with SPSS 11 Program. Result: From 200 women undergoing IVF procedure, 45.5% of them aged between 35-40 years old, with the youngest is 22 years old and the oldest is 48 years old (average age=35 years, SD=4.7), and almost half of them (45%) have BMI between 18.01-23.00 kg/m2. 185 women has primary infertility. These subjects came to the clinic mostly due to male factor (23.5%), idiopathic cause (19%), and ovulation disturbance (13%), whilst endometriosis, tubal factor, and other factors take just little percentage. When compared between long and short protocols, the number of oocyte retrieved is significantly different (p=0.007, CI=5.84-7.11), whereas embryo numbers (p=0.054, CI=1.80-2.39) and the pregnancy rate (p=0.525, CI=0.21-0.33) found to be not significantly different. There were 2 cases of moderate OHSS developed in the long protocol group. Conclusion: Although the number of embryo produced and the pregnancy rate are found to be not significantly different between the two groups, long protocol of IVF is found to produce more oocyte to be retrieved and develop more OHSS events compared to short protocol. Due to this result, GnRH antagonist can be considered when planning an IVF procedure. However, a longitudinal multicenter study with larger sample size is needed to validate the current data.
