Abstract

Clostridium difficile (C. difficile) infection mostly found in immunocompromised patient. Most of the patients with neutropenia had hematologic malignancy and come with multiple risk factors for C. difficile infection. The aim of this study is to know the prevalence and characteristic of C. difficile infections in cancer neutropenia patients. This cross-sectional study was done in secondary referral hospital in suburban Karawaci, Tangerang, Banten, Indonesia from June 2014 until Mei 2015. Adult cancer patients hospitalized with neutrophil count less than 1500 106 cell/L were included. All patients were checked for antigen and toxin C. difficile in faeces. From 25 patients were evaluated for C. difficile infection in neutropenia. C. difficile antigen only found in one patient or 4% sample.Key words: C. difficile, cancer, neutropenia

Abstract

Introduction: Prostate carcinoma is the second most common tumor in male, 95% in which made up from adenocarcinoma. The diagnosis of prostate adenocarcinoma through a needle biopsy specimen plus the determination of tumor staging are paramount in selecting the therapy and management. This study is done to know the morphologic variation of prostate adenocarcinoma in the needle biopsy as well as to measure the grading compatibility between the needle biopsy and prostatectomy using Gleason scoring system.Materials and Method: This retrospective study is conducted through form and specimen slides compilement. The specimens, consisting of prostate adenocarcinoma’s needle biopsy and prostatectomy, were gathered from the archives of Pathological Anatomy Departement, Faculty of Medicine, University of Indonesia in the year of 2008-2013. The slides were re-read and the morphologic appearance’s variation was valued. Gleason scoring was also executed from the pairing specimen according to International Society of Urological Pathology (ISUP) 2010.Result: Out of 114 needle biopsy cases, the morphologic variation was found to be perineural invation (n=38), mucinous fibroplasia (n=1), glomerulation (n=1), mucinous basophilic (n=25), and eosinophilic crystal (n=5). The amount of patient that was performed both specimen is 11, and there is a compatibility between biopsy score and prostatectomy as much as 63.63% and the median is 7.Conclusion: It is requisite to know the morphological variation in prostate adenocarcinoma in the biopsy needle specimen to get an accurate diagnosis. Undergrading in biopsy specimen is as much as 36.36%, owing to the fact that prostate carcinoma can be manifested as mutifocal lesion, thus the higher grading can only be found in prostatectomy specimen, for the needle biopsy was inadequately taken.Keywords: prostate adenocarcinoma, morphological view, Gleason score

Abstract

Psoas abscess is a rarely found abscess located in the iliopsoas compartment. It characterized by fever, back pain, and limitation of hip movements. Psoas abscess is hard to diagnose due to only 30% of classic symptoms and signs are found. In this study we discussed tuberculous psoas abscess in patient 26 years old male. Anamnesis, physical examination, and supporting medical tests were done to diagnose tuberculous psoas abscess. Therefore we performed abscess incision and drainage, followed by continuous tuberculous therapy.Keywords: tuberculous psoas abscess

Abstract

Nasopharyngeal cancer (NPC) is endemic in Indonesia. The purpose of the review is to describe the risk factors, clinical symptoms, diagnostic procedures and staging, management and treatment options, and follow-up. Multi-disciplinary approach to the NPC not only for treatment, but also has been started from the diagnosis proccess. The multidisciplinary approach also plays role after treatment, especially for the follow-up and rehabilitation of post-treatment toxicity.Keywords: cancer, nasopharynx, radiotherapy.

Abstract

Loss of consciousness (LOC) is a clinical presentation of patients with diabetes mellitus (DM) in emergency department which is caused by complications of the disease. Diabetic ketoacidosis, hyperosmolar hyperglycemic state, lactic acidosis, uremic encephalopathy, and hypoglycemia are the several main causes of LOC in patients with DM.  The comprehension of pathophysiology of LOC in patients with DM will be helpful in making accurate diagnosis. History taking, physical examination, and laboratory tests are essential in diagnosing that particular condition. Abstract

Introduction: Previous studies showed that personality have significant association with the outcome of performance of the students in clerkship. There are three dominance factors of personality such as Ego strength (Es), Responsibility (Re), and Dominance (Do) associated with the length of study of students. This three personality factors will be measured by using MMPI-2 Test. Aim: This study wants to find the mean difference of three factors of personality among the length of study in Faculty of Medicine of University of Pelita Harapan.Methods: This was a cross-sectional study by using the secondary data of MMPI-2 test of student from batch 2005-2006. Personality factors of Ego Strength, Responsibility, and Dominance are measure by mean and standard deviation.Results: Eighty one students were 27 males and 54 female.  From 81 students, 79 persent has finished their medical study within 5.5 years (on time) and 21 persent more than 5.5 years (not on time).Corellation by using spearman test show the significant association of Es with Do (p=0.001 and r=0.357); Es with Re (p=0.001 and r=0.3589). However the Do has no show the significant correlation with Re (p=0.525; r=0.0716). Analysis of t-test show that students who has graduate on time has higher Es (mean=44.12 ± 9.71) than student graduate not on time (mean 42.82 ± 7.97). Score of Do of students has graduate on time (mean 45.85 ± 10)  also higher than late graduate group (mean 41.05 ± 8.45) with p=0.0738. However the Responsibility of graduate on time (mean 48.06 ± 11.21) is lower than late graduated students (mean 49.7 ± 9.66).Conclusion: Ego strength and Dominance has positive association with the length of study.Keywords: ego strength, responsibility, dominance, MMPI-2, medical education

Abstract

Introduction: Colorectal cancer (CRC) is generally a disease of persons aged above 40 year-old. Controversies still exist regarding clinical characteristics of CRC in young persons. The aim of this study is to know the clinical characteristics of young CRC patients in Siloam General Hospital, Karawaci, Banten.Material And Methods: A total of 14 patients with diagnosis of CRC from December 2013 to January 2014 in Siloam General Hospital, Karawaci, Banten were studied cross - sectionally.  The symptoms and signs, family history of cancer, diabetes, location of mass were collected and evaluated.Results: There were 5 (35.7%) cases of CRC patients aged below 40 year-old (young patients) and 9 (64.3%) above 40 years-old (old patients). The most common symptom was constipation followed by bloody diarrhea i.e. 57.1% and 42.9% consecutively. Constipation occurred in 2 and 6 young and old patients consecutively and in 3 and 3 young and old patients consecutively. In young patients, 2 (14.2%) had a history of cancer in the family compared to none in the old patients. According to the location of tumor in the colon 4 (28.5%) were in ascending colon with 2 and 2 in young and old patients consecutively, 3 (21.7%) were in descending colon with 2 and 1 in young and old patients consecutively, 8 (57.1%) were in rectum with 2 and 6 in young and old patients consecutively. One young patient had diabetes, and one other young patient had positive Clostridium difficile antigen and toxin.Conclusion: Approximately 35.7% cases CRC patient in our hospital was of young patients. Two patients were having family history of cancer. The location of mass was not specific for young CRC patients.Key words: colorectal cancer, young patients

Abstract

Guillain-Barre syndrome is an acute poliradiculopathy that characterizes as flaccid limb weakness subsequently after an infection disease. It is scarcely found to be triggered by the dengue virus infection. Here in a case report of 48-year-old male, presented as an ascending weakness of both limbs, following 2 days fever. Abstract

Third-degree atrioventricular (AV) block also termed complete heart block is present when there is complete absence of conduction between atria and ventricles. In adults the most common causes are acute myocardial infarction and age-related degeneration of conduction system. Incidence of conduction block is reported to be 25-30 % in the setting of acute myocardial infarction (AMI) and is 2 to 3 times as commonly associated with inferior than anterior infarction. Proper diagnosis involves noninvasive diagnostic tests (12 lead electrocardiogram (ECG), Holter ECG, or stress/exercise ECG).Keywords: third-degree AV block, complete av block, acute myocardial infarction

Abstract

Therapy of hemorrhoids ranges from dietary and lifestyle modification to  surgery, depending on degree and severity of symtoms. While debate continues as to which is the best surgical method for the therapy of hemorrhoids, none of the currently available surgical methods approach the ideal surgical option, which is one that effective while being safe and painless. In reality, the less painful the procedure, the more likely it is to be associated with recurrence post-op. A fuller understanding of the pathophysiology will help to make choices.Keywords: hemorrhoids, pathophysiology, therapy.
