Abstract

Peritoneal Malignant Mesothelioma (PMM) is uncommon disease, but increasing in frequency nowadays. This highly aggressive malignancy occurs most commonly in older men and has a strong association with asbestos exposure. It manifests most often as diffuse sheet-like or nodular thickening of the peritoneal surfaces, but it may occasionally be a localized mass. The very large variations of its clinical features and its histological appearance mimicking adenocarcinoma make this tumor is difficult to diagnose.We report a case of PMM that previously diagnose as adenocarcinoma of the ovary. A 29 year-old female came to gynecology clinic with para-ovarian mass. She had no history of asbestos exposure. The mass was oval 9x6x6 cm in size, whitish and firm. Microscopic features showed papillary dense structure with desmoplastic stroma, covered by a layer of cuboidal to columnar cells. The cells with mild pleomorphism and hyperchromatic nuclei, mitotic figures were minimal. The immunohistochemistry tests revealed positive for D2-40, Calretinin, CK8 and CK 18, weakly positive for Inhibin and EMA, and negative for CEA and AFP. Patient had been received chemotherapy, there were no metastasis.

Abstract

Astrositoma merupakan glioma tersering. Tumor ini bisa mengenai  dewasa dan anak-anak.World Health Organization (WHO) mengelompokkan astrositoma menjadi 4 grade berdasarkan karakteristik histologik. Astrositoma high grade terdiri atas astrositoma anaplastik (grade III) dan glioblastoma (grade IV).Data Departemen Patologi Anatomik Rumah Sakit Cipto Mangunkusumo (RSCM) tahun 2001-2010 melaporkan kejadian astrositoma sebanyak 179 kasus atau sekitar 20% dari seluruh tumor intrakranial, astrositoma anaplastik ditemukan sebanyak 12 kasus, dan  glioblastoma 42 kasus.Seperti pada tumor otak lain, astrositoma high grade mengakibatkan gejala dan tanda gangguan neurologik fokal dan umum. Pemeriksaan radiologik pilihan adalah dengan Magnetic Resonance Imaging (MRI). Astrositoma anaplastik memberikan gambaran hypointense pada T1 dan hyperintense pada T2 dengan efek massa yang bervariasi. Karakteristik glioblastoma pada MRI berupa lesi iregular menyangat kontras di sekeliling nekrosis sentral (ring enhancement) dan edema vasogenik luas di sekitar tumor.Astrositoma anaplastik secara histopatologik dicirikan dengan atipia inti, peningkatan selularitas, serta aktivitas proliferasi yang nyata. Glioblastoma secara histopatologik serupa dengan astrositoma anaplastik, disertai adanya proliferasi vaskular dan/atau nekrosis. Astrositoma anaplastik dan khususnya glioblastoma mempunyai variasi gambaran histologik yang beragam, antara lain varian small cell, granular cell, giant cell, dan gliosarcoma.

Abstract

Background: Central nervous system (CNS) tumours affect the quality of life of patients since its neurological deficits. Data from Globocan 2012 reveals that there are 256,000 cases of CNS tumour. Epidemiology of the CNS tumours is very important for diagnosis and treatment, but data in Indonesia is still not fully reported.Objective: The aim of this study was to determine the exact amount of the incidence, histologic type of the tumour and the characteristic of patient in our institution.Methods: This is an observational study, all pathology report of CNS tumours that underwent surgery at Siloam Hospitals Lippo Village from 2011 until 2015. We classified based on gender, age, location of the tumour, and the histologic type according to WHO Classification of CNS tumour 2007.Results and Discussion: There were 913 patients of CNS tumours from 2011 until 2015. The most common tumours were meningioma (32.96%) followed by glioma (21.35%) and pituitary adenoma (16.10%). In meningioma, most occur in women, 41 - 50 years old, located in the frontal region and the most common subtype is transitional meningioma. In glioma, most occur in men, 31 - 40 years old, located in the frontal region and the most common subtype is glioblastoma. In adenoma hipofisis, most occur in men, 41 - 50 years old.Conclusion: The result of this study was accordance with the literature so this data could be a reference for further research.

Abstract

Introduction: Overseas studies have shown various distributions of breast cancer primary tumor locations based on stages and histopathologic types. Based on them, breast cancer was consistently reported to be found on the left breast more than the right. Breast cancer laterality was estimated to be one of the determinant factors in prognosis. In Indonesia, there are only a few studies on the laterality of breast cancer, as well as the characteristics that accompany it. The aim of this study is to know the distributions of primary breast cancer tumor locations in Indonesian patients.Material and Methods: A cross-sectional design study was done and the data was collected from Siloam Hospital Lippo Village radiology and pathology anatomy department within the 2014-2018 period. The results were presented using descriptive statistics.Results: A total of 74 breast cancer patients from various ages and stages were enrolled in this study. Based on the histopathological types, 84.2% of patients left breast tumors and 72.2% of patients with right breast tumors had invasive ductal carcinoma of no special type. Based on quadrants, 50% of tumors on the left breast are in the superolateral quadrant, followed by 31.6% in superomedial, 13.2% in inferomedial, and the rest in inferolateral. Conclusion: Most tumors are located in the superolateral quadrant and the most histopathological types are non-specific invasive carcinomas.

Abstract

Introduction: Prostate carcinoma is the second most common tumor in male, 95% in which made up from adenocarcinoma. The diagnosis of prostate adenocarcinoma through a needle biopsy specimen plus the determination of tumor staging are paramount in selecting the therapy and management. This study is done to know the morphologic variation of prostate adenocarcinoma in the needle biopsy as well as to measure the grading compatibility between the needle biopsy and prostatectomy using Gleason scoring system.Materials and Method: This retrospective study is conducted through form and specimen slides compilement. The specimens, consisting of prostate adenocarcinoma’s needle biopsy and prostatectomy, were gathered from the archives of Pathological Anatomy Departement, Faculty of Medicine, University of Indonesia in the year of 2008-2013. The slides were re-read and the morphologic appearance’s variation was valued. Gleason scoring was also executed from the pairing specimen according to International Society of Urological Pathology (ISUP) 2010.Result: Out of 114 needle biopsy cases, the morphologic variation was found to be perineural invation (n=38), mucinous fibroplasia (n=1), glomerulation (n=1), mucinous basophilic (n=25), and eosinophilic crystal (n=5). The amount of patient that was performed both specimen is 11, and there is a compatibility between biopsy score and prostatectomy as much as 63.63% and the median is 7.Conclusion: It is requisite to know the morphological variation in prostate adenocarcinoma in the biopsy needle specimen to get an accurate diagnosis. Undergrading in biopsy specimen is as much as 36.36%, owing to the fact that prostate carcinoma can be manifested as mutifocal lesion, thus the higher grading can only be found in prostatectomy specimen, for the needle biopsy was inadequately taken.Keywords: prostate adenocarcinoma, morphological view, Gleason score

Abstract

Background: Meningiomas are the most common intracranial tumor of central nervous system tumors. Although the prevalence is lower, the WHO grade II and III meningiomas are more aggressive, with higher mitosis rates, are more likely to recur after surgery, and have lower survival rates. The ability to differentiate between WHO I and WHO II/ III meningiomas before surgery can contribute to a significant clinical benefit in helping the neurosurgeon doing the best management planning.Methods: This is a retrospective cross-sectional study of meningioma patients in Siloam Hospital Lippo Village between 2014 – 2018. The sample will be recruited using consecutive sampling. The relationship between analyzed variables and meningioma grades will be investigated using a chi-square test if the data was eligible; otherwise, the Fisher-exact test will be performed.Result: Ninety eight (69%) patients diagnosed as low grade meningioma, and 44 (31%) as high grade meningioma. Tumor location, size, edema, necrosis, age, and gender had significant results with p £0.05. Multivariate results also show that all six variables have a significant relationship with each other.Conclusions: Tumor location, size, edema, necrosis, age, and gender have a significant relationship to histopathological meningioma grade in patients at Siloam Hospital Lippo Village in 2014-2018.

Abstract

Background: Sexual violence can take many forms to anyone. The increase in the number of sexual violence against women in Indonesia is quite significant. These incidents are reported to occur frequently in women in young adulthood. These experiences can have long-term effects, one of which is depression. There are several studies that show an association between sexual violence and the incidence of depression. However, there has not been much quantitative research to look at the relationship on this matter in Indonesia, especially on female students. This study aims to understand the relationship between sexual violence and the incidence of depression in female students of the Faculty of Medicine, Universitas Pelita Harapan.Methods: This study use a cross-sectional study design. The data has been collected using questionnaires filled in by the respondents. This study involved 149 female students of the Faculty of Medicine, Pelita Harapan University. The results of the study were processed and statistically analyzed using SPSS 25.Result: Based on research data, there are 149 respondents from female students of the Faculty of Medicine, Universitas Pelita Harapan. with a mean age of 19.29 (SD = 1.086). The youngest age of the respondent is 17 years old and the oldest is 24 years old. A total of 55 respondents had experienced sexual violence. Based on the results of the Beck's Depression Inventory (BDI) questionnaire, 16% had mild depression, 9% had moderate depression, and 2% had severe depression. Chi-square test showed a significant relationship between experiences of sexual violence and the incidence of depression, with p <0.001.Conclusions: Sexual violence is related to the incidence of depression in female students of the Faculty of Medicine, Pelita Harapan University

Abstract

Aim: to provide additional information regarding the clinicopathological characteristics of Papillary Thyroid Carcinoma (PTC). Methods: Fifty patient with PTC were reviewed to determine prognostic factors such as age, gender, size of tumor and histologic variant. BRAF V600E mutation was detected by immunohistochemical staining and assessed with H score. Result: BRAF V600E mutations were detected in 17 (34%) cases. There were seven cases with extrathyroidal extension (ETE) p 0,04, 11 cases with lymph node metastasis (LNM) p < 0,001, and 8 cases with tall cell variant p 0,047.The cases with positive BRAF V600E mutation had mean age of 44.71 years, and the size of the tumor between 0.1-4cm. Six cases of them are male and 11 female.Conclusion: There were significant relationships between BRAF V600E mutation with ETE, LNM, and tall cell variant. There was no significant relationship between BRAF V600E mutation, either with age, gender, or size of the tumor. BRAF V600E immunohistochemical examination can be performed as additional investigation for PTC patients.

Abstract

Background: Non-Hodgkin large cell B-lymphoma (DLBCL: Diffuse Large B-cell Lymphoma) were classified into two subtypes of Germinal Center B-Cell Like (GCB) and non-Germinal Center B-Cell Like (non-GCB), which GCB has a better prognosis. The Ki-67 antigen is one of the most reliable markers of cell proliferation. This research aims to establish the relationship between proliferation index Ki-67 with DLBCL subtypes and to evaluate the utility of Ki-67 proliferative index as a predictive marker for predicting stages of lymphoma.Methods: We obtained 60 cases of patient samples DLBCL in Siloam Hospital Lippo Karawaci in 2014-2018. Clinical and pathological data were obtained from medical records. Chi-square methods were used to analyze data.Result: There were more ≤60 years compared to >60 years. In this study, there were more male patients than female patients. lymphoma provided in extranodal were higher than lymphoma in nodal form.Conclusions: There is a relationship between Ki-67 proliferation index with lymphoma GCB and non-GCB subtypes and stage of lymphoma, which can be used as a predictive factor in predicting the stage of lymphoma.
