Jurnal Plastik Rekonstruksi is an open access, peer reviewed, international journal focusing on global plastic and reconstructive surgery. JPR publishes on all areas of plastic and reconstructive surgery, including basic science/experimental studies pertinent to the field and also clinical articles on such topics as: breast reconstruction, head and neck surgery, pediatric and craniofacial surgery, hand and microsurgery, wound healing, and cosmetic and aesthetic surgery. Clinical studies, experimental articles, ideas and innovations, and techniques and case reports are all welcome article types.

Abstract

Background: Cleft lip and palate is one of the most common congenital anomalies with the prevalence of 1 case for every 1000 birth. Nulliparity has been associated with increased of many birth defects. In contrast, multiparity has also been associated with several birth defects. Methods: This case-control study compares data from 74 cleft lip patients who was enrolled in Gentur Cleft Foundation to those of 87 normal children. The dependent variable of this study was the incidence of CL/P (cleft lip with or without cleft palate) and the independent variables were maternal parity. Binary logistic regression was used to analyze crude and adjusted OR. Nulliparous mothers were treated as the reference category. Result: Compared to nulliparous mothers, primiparous mothers has OR 1.57 (95%CI=0.78-3.2). While multiparous mothers has OR 27.54 (95%CI=3.5-216.32). P-value was insignificant for the association of primiparous mothers with CL/P Conclusion: We observe higher incidence of multiparous mothers with the incidence of CL/P compared to nulliparous mothers.

Abstract

Background: In the last decade, there are various methods of bone grafting in plastic surgery. Cancellous bone is more often used as bone graft than cortical bone due to its high osteoconductivity and osteogenic capability. The synthetic materials for bone graft such as tricalcium phospate (TCP) and hydroxy-apatite can be use as new alternative for bone grafting surgery. Methods: The PubMed® databases were searched using the terms: ‘bone graft OR bone grafting’ AND ‘cancellous bone OR iliac crest OR tibia’ AND ‘tricalcium phospate OR tricalciumphospate OR tri calcium phospate’, restricted to English language, and to a publication period from January 2010 to January 2017. The inclusion criteria were any kind of research paper that compared the utility of TCP synthetic graft and autograft as a bone graft. Results: Out of 139 selected studies, only 8 met the inclusion criteria. In 6 publications TCP graft has similar outcome and in 1 publication have a better result in reducing post operative complication. Conclusion: TCP graft can be used as substitution for autograft in particular clinical conditions and surgical methods.

Abstract

Background : Algorithm for vulvar reconstructions in defects after oncologic resection of vulvar tumors has been established. It goals are to restores form and function for the purpose of coitus, micturition, and defecation. The purpose of this article is to present our experience in vulvar and anal reconstruction. Method : A 43-year-old female with extensive vulvar, perineal and anal squamous cell carcinoma in-situ and groin lymph nodes metastasis requiring defect coverage after tumor resection. The defects involving exposed soft tissue around vulvar, perineal and anal areas. Result : Vulvar and anal reconstruction was done in stages utilizing triple VY advancement flap. The reconstruction was successful with preservation of sexual and anal functions; and minimal scarring. Conclution: Extensive vulvar and anal defects can be reconstructed by recruiting local tissue. VY advancement flap is one of the best options in these reconstructions.

Abstract

Background : Burns result in severe injuries that cause damage or loss of tissue due to contact with sources of heat resulting in injuries to all body systems. Injuries of the skin, which functions as a barrier to protect internal organs, may cause patients to experience damage to one's physical appearance and body image causing negative feelings that may lead to other problems such as psychopathology and symptoms of mental illness. Method : A cross sectional study with consecutive sampling method of burn patients who were treated at the Plastic Surgery Outpatient Clinic and Burn Unit of RSCM was conducted between April-May 2017. Subjects were asked to fill in self-report questionnaires including patient identity form, SRQ-20 (cutoff point ?6) for presence of psychopathology, and WHOQoL-BREF to obtain mean scores of quality of life that include four domains of physical, psychological, social, and environment assessment. Data collected was analyzed using correlation analysis. Result : 56 burn patients were included in the study. 30.4% did not work and 48.2% had very low income per month. 67.9% patients experienced burns due to fire and 44.6% had burns 10-30% of the TBSA with a majority of patients (80.4%) experiencing a combination of second & third degree burns. Based on the analysis, 57.1% of patients had a form of psychopathology and low mean scores of quality life (physical domain 48.1, psychological domain 51.5). Significant negative correlations (p ? 0.05) were obtained between the psychological domain and symptoms of depression, anxiety, low energy; physical domain and low energy; and social domain with anxiety. Conclusion: This study obtained significant results to identify the correlation between psychopathology and various domains of quality of life affected.

Abstract

To achieve an optimal result in cleft surgery, executing the proper designs and precise techniques are mandatory. Several techniques are proposed for the repair of unilateral and bilateral cleft lip. In 1955, Millard !rst described the techniques of advancing a lateral upper lip "ap combined with downward rotation of the medial segment.1 From experience, the above- mentioned technique may be modi!ed to facilitate better lip rotation and symmetry. Technically, it also provides a more ef!cient work"ow and more accurate results. All efforts in lip repairs have one ultimate goal, which is to achieve corrected lips with minimal scar and as much resemblance to that of the normal lips: by anatomy, symmetry, and muscle function.

Abstract

Background: Acute kidney injury is important factor that can increase mortality in patient with severe burn injury. Knowledge about incidence and characteristic of acute kidney injury among severe burned patients will help medical practitioner to establish appropriate medical decision and health care protocol. Method: PubMed, Cochrane Library, and GoogleTM search yielded 12 articles. The articles were screened by inclusion criteria, exclusion criteria, and full text reading. Five remaining articles appraised in regard of validity, importance, and applicability. Result: Incidence of acute kidney injury among severely burned patients varies from 24.4% to 53.3%. According to the study with best level of evidence, a systematic review, the incidence of AKI is 28.8% (95% CI 26.8%-30.7%). AKI can occur early or late, and may progress. Progressive AKI has highest mortality rate. Conclusion: AKI is common complication among patients with severe burn injury. Approximately one- third to half of patients with severe burn injury will develop AKI in the course of the disease. Early clinical assessment and re-evaluation are important in severe burn injury management.

Abstract

Scar is a reminding mark left by surgeons on patients who presented with wounds or required surgery. In healing process, due to some circumstances and individual traits, wounds could become bad scars that would definitely affect the patient’s appearance and cause many complaints. In the first years after treatment, surgeons will be commemorated as heroes who once helped the patients to overcome and cure the disease. After a few years, the memory of the surgeon’s heroic action will gradually dissipate with the formation of a bad scar. Such good memory will be replaced with persistent complaints arising from the scar such as deformity, pain, itchiness and enlargement of the lesion. Several efforts have been made to manage disturbing scars however they often do not yield satisfactory result and moreover sometimes the scars will enlarge, become unpleasant and more disturbing. This will frustrate the patient as well as the surgeon. The good memory of the surgeon as a hero has shifted into one whose surgical procedure was regretted. Presently, there are about 25 modalities using various approaches for scar treatment. These manifold choices indicate that the available modalities have been generally inadequate and the therapeutic response very variable. Furthermore, treatment selection is also based on recurrence. Most of the available therapeutic modalities in this stage are nonsurgical. There is also topical treatment mostly containing herbal extract, which is considered inadequate and lacking of evidence base. Corticosteroid injection and silicone gel is a nonsurgical method recommended and supported by evidence based. This technique is largely chosen by doctors. Combined therapy is the method of choice believed to yield optimal result. Let us remain heroes by paying attention to scars.

Abstract

Background: To evaluate two pediatric patients with syndromic craniofacial anomaly that underwent craniofacial reconstructive surgery using resorbable plate-screw systems which have been claimed as biodegradable fixation materials and used in craniofacial reconstructive procedures owing to their advantages such as adequate biomechanical resistance, longer dwelling time, elimination through physiological routes without causing any foreign body reaction and/or significant sequale. Patient and Method: Resorbable plate-screw systems used in 2 patiens for craniofacial reconstructive procedures such as bilateral fronto-orbital advancement and segmental right orbita (four wall box) osteotomy were evaluated as for their efficacy. Result: Adequate fixation was obtained in both patient, but Infection complication was seen in segmental right orbita osteotomy patient that appear localized abcess formation on subciliary incision and fronto medial insicion. After drainage insicion and antibiotic administration for 1 week, the infection was relieved. Summary: Owing to resorbable copolymer which contain a polyester derivate of L- lactidc and glycolic acid are ideal fixation materials used favourably in pediatric craniofacial reconstructive surgery and have further advantages such as adequate biomechanical resistance against distraction and compression forces in the early postoperative period, longer dwelling time and elimination from the body through physiological routes without causing any foreign body reaction.

Abstract

Background: Deep burn wound with exposed underlying structures such as muscle or moreover in this case, bone, may increase morbidity in the terms of infection. Therefore careful planning of reconstruction is needed. Patient and Method: We report one case of neglected burn wound with exposed tibia and muscle contracture on bilateral knee joint that was reconstructed in our Plastic Surgery Division of Prof RD Kandou Hospital. Data was taken from medical and surgery record. Result: We managed the muscle contracture of the left knee joint by sedated gradual stretching and splinting. We also utilized hyperbaric treatment in order to maximize granulation process. A medial gastrocnemius muscle flap followed by split thickness skin graft is used to reconstruct the missing tissue at upper tibia, and another split thickness skin graft to cover another wound at opposite upper leg. The result was satisfying without any complication. Conclusion: In managing a neglected burn wound with bone exposed and muscle contracture, it is compulsory to have a careful reconstruction planning. Our objectives are to release the muscle contracture and to provide an adequate coverage to the bone-exposed wound by using many modalities we have.

For more than 5 decades, microvascular surgery has been growing and found its place in reconstructive surgery. In Indonesia, microsurgery became popular since 30 years ago. In the past, this subspecialty was developed for soft tissue transfer and replantation of amputated extremities.
