Abstract

Background: Malocclusion can affect the aesthetics and function of the oral cavity, thus requiring orthodontic treatment. In removable orthodontic appliances, treatment success is greatly influenced by the retention components. The Adams clasp is one of the retention components commonly used because it has good retention and can be modified. In cases requiring additional retention in the anterior region, a modified long span Adams clasp with an interproximal arrowhead can be used on the incisors. Objective: To describe the fabrication procedure of a modified long span Adams clasp with an interproximal arrowhead on the central incisors. Case: The patient was in the mixed dentition phase with Angle Class I malocclusion, maxillary and mandibular anterior crowding, 4 mm overbite, 2 mm overjet, rotation of teeth 12 and 22, and linguoversion of tooth 42. Treatment was initiated in the maxillary arch with a treatment plan involving extraction of teeth 53 and 63, followed by derotation of teeth 12 and 22. To support appliance stability, a long span Adams clasp was fabricated on teeth 11 and 21. Conclusion: The fabrication of the long span Adams clasp was performed by bending 0.7 mm wire, forming interproximal arrowheads on teeth 11 and 21, creating the clasp bridge, arrowheads on the distolabial undercuts, clasp arms, and tags. The acrylic plate was fabricated using the sprinkle-on method, followed by finishing and polishing to produce a stable, smooth, and comfortable appliance. Keywords : Adams clasp, long span Adams clasp, removable orthodontic appliance, retention component

Abstract

Background: Malocclusion is the misalignment of teeth that affects oral health and facial aesthetics. The prevalence of malocclusion in South Kalimantan is high, but the number of people undergoing orthodontic treatment is still low. The need for memengaruhi dalam menjalani perawatanc treatment is particularly high among adolescents, as malocclusion affects their psychosocial well-being and appearance. Many adolescents with malocclusion do not undergo treatment because they are unaware of the need for it. This indicates that, in addition to clinical need, interest also influences the decision to undergo treatment. Objective: This study aims to analyze the relationship between orthodontic treatment needs and interest in fixed orthodontic treatment among high school students in Banjarmasin, a non-urban area. Method: This study was an analytical observational study with a cross-sectional design. The sample consisted of 175 adolescents aged 15-18 years. A questionnaire on interest in fixed orthodontic treatment was used to measure interest in fixed orthodontic treatment. The Index of Complexity, Outcome, and Need (ICON) was used to assess the need for orthodontic treatment. Results: The Spearman test results showed a significance of 0.041 (p<0.05), indicating that there is a relationship between orthodontic treatment needs and interest in fixed orthodontic treatment. A correlation value of 0.155 indicates that there is a weak relationship between the need for orthodontic treatment and interest in fixed orthodontic treatment. Conclusion: There is a positive correlation between the need for orthodontic treatment and interest in fixed orthodontic treatment, with a weak coefficient. The greater the need for orthodontic treatment, the greater the interest in treatmentKeywords: High school students, ICON, Interest, Orthodontic treatment needs

Abstract

Background: Dental caries and malocclusion remain common oral health problems among school-aged children. Irregular tooth arrangement may hinder effective oral hygiene practices, resulting in greater plaque retention and an increased likelihood of dental caries. However, evidence regarding the association between these two conditions in Banjar ethnic children is still limited. Objective: To determine the relationship between the severity of malocclusion and caries status among Banjar ethnic students aged 11–12 years at SDN Sungai Andai 4, Banjarmasin City. Methods: An observational analytic study with a cross-sectional design was conducted involving 45 students selected through purposive sampling. Malocclusion severity was assessed using the Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN), while caries status was evaluated using the Decayed, Missing, and Filled Teeth (DMF-T) index. The association between the variables was analyzed using Spearman’s rank correlation test. Results: The most frequently observed level of malocclusion severity was IOTN-DHC Grade 2, identified in 16 participants (35.6%). Displacement was the predominant malocclusion component, occurring in 28 cases (62.2%). The population DMF-T score was 0.69, indicating a very low caries category. Spearman’s correlation analysis demonstrated a statistically significant association between malocclusion severity and caries status (p = 0.001), with a correlation coefficient of r = 0.571, indicating a moderate positive relationship. Conclusion: The findings demonstrated a significant association between malocclusion severity and caries status among Banjar ethnic children aged 11–12 years at SDN Sungai Andai 4. Children with more severe malocclusion tended to exhibit higher caries status. Keywords: Banjar ethnic group, caries status, DMF-T, IOTN-DHC, malocclusion

Abstract

Background: Malocclusion is one of the oral health problems with a relatively high prevalence in Indonesia. This condition not only affects oral cavity function but also has an impact on facial aesthetics and the psychosocial aspects of adolescents. The severity of malocclusion can be assessed using the Index of Complexity, Outcome, and Need (ICON), while the perception of oral aesthetics can be measured through a self-perception questionnaire. Objective: To determine the effect of malocclusion severity on oral aesthetic perception among senior high school/equivalent students in the non-urban area of Banjarmasin. Methods: This study used an analytical observational design with a cross-sectional approach. The study sample consisted of 175 senior high school/equivalent students selected by simple random sampling. Data on malocclusion severity were obtained from jaw impressions measured using the ICON index, while data on oral aesthetic perception were collected through a questionnaire. Data analysis was performed using the Spearman correlation test with a statistical application program. Results: The majority of students had mild (31.4%) and very mild (28.6%) levels of malocclusion severity, while severe and very severe categories were found in (11.4%) and (13.7%) of students, respectively. Most respondents had a positive oral aesthetic perception (57.1%). The correlation coefficient value was 0.571, indicating that the malocclusion severity variable affected oral aesthetic perception by 57.1% with a moderate strength of effect, while the remaining percentage was influenced by other factors. The positive direction of the effect means that the higher the level of malocclusion severity, the more negative the students’ oral aesthetic perception. Conclusion: There is a significant effect between malocclusion severity and oral aesthetic perception among senior high school/equivalent students in the non-urban area of Banjarmasin. This indicates the importance of paying attention to malocclusion not only from functional aspects but also from the aesthetic and psychosocial aspects of adolescents. Keywords: ICON, Malocclusion, Oral Aesthetic Perception

Abstract

Background: In the field of orthodontics, anterior dental crowding in the mandibular arch is a common problem that affects aesthetic appearance, masticatory function, and periodontal tissue health. This study focused on the development of a fixed Williams Expander appliance as a solution to overcome dental arch misalignment in the mandible. Objective: To determine the proper fabrication technique of the Williams Expander in cases of mandibular anterior dental crowding. Case: The laboratory received a working model with a case of anterior dental crowding in the early permanent dentition phase, and the dentist provided instructions for the fabrication of a fixed orthodontic appliance, namely the Williams Expander, to correct dental crowding and dental arch misalignment. The fabrication process of this fixed Williams Expander appliance utilized various instruments and materials, such as molar band s, which were used for anchorage and as a strong connection between the arms of the Hyrax screw and the molar band s. This fabrication process was carried out in stages, resulting in a stable and effective fixed Williams Expander appliance for creating space for overlapping teeth. Conclusion: The fabrication stages of this fixed Williams Expander appliance began with drawing the design, relieving the interdental area to a depth of 2–3 mm, adjusting the molar band s by cutting the lingual part of the molar band s and performing welding, fitting the screw according to the mandibular arch, soldering with the application of flux and antiflux, followed by finishing and polishing. Keywords : Anterior dental crowding, Fixed orthodontic appliance, Williams Expander.
