Abstract

Increased access to health services after universal health coverage has not been matched by the fulfillment of the ratio of doctors will disrupt the resilience of national health insurance. The role of doctors in first-level health facilities as health service providers and referral screeners (gate keepers). This gap in the ratio of doctors can lead to high rates of non-specialistic referrals. This study aims to analyze the relationship between the ratio of doctors to Non-Specialistic Referrals (RNS) at BPJS Kesehatan Pamekasan Branch Office. The research method used was quantitative with an observational analytic approach using cross sectional and secondary data derived from all Status in Social Security Agency on Health, Pamekasan and Sumenep districts, and analyzed with the chi-square test (α=0.05). The results of this study indicate the existence of a doctor's ratio on the status of non-specialistic referrals, namely there is a relationship with a p-value of 0.001<0.05; OR=2.381 CI= 1.099-3.574. The conclusion of this study shows that there is a relationship between the ratio of doctors to the status of non-specialistic referrals. Local governments are expected to develop strategies to fulfill the ratio of doctors, especially in first-level facilities to achieve quality and standardized health services

Abstract

Refill Drinking Water Depot workers rely on physical strength to work, so they are at risk of experiencing Musculoskeletal Disorders (MSDs) complaints. Based on the initial survey, 13 out of 15 workers (86.7%) experienced MSDs. This research aims to determine the relationship between individual and work factors and MSD complaints among refill drinking water depot workers in Padang City. The study used a cross-sectional design. We collected data from November 2022 to July 2023 with a sample of 83 people, selected using proportional random sampling techniques. The dependent variable in this study is complaints of Musculoskeletal Disorders (high risk if NBM score > 20). The independent variables are age (at risk ³ 35 years), work period (at risk ³ five years), smoking habits (at risk if IB values ³ 200), Working posture (at stake if REBA score ³ 7), physical workload (at risk if ³ 100 beats/minute) and BMI (at risk if < 17 and > 25). Data analysis uses univariate and bivariate analysis (chi-square test). The results of the study showed that 71.1% of workers experienced MSD complaints, 34.9% of workers with a risky age, 19.3% of workers with a problematic work period, 32.5% of workers had a smoking habit, 9.6% of workers with a risky BMI, all workers are not at risk of physical workload and 56.6% of workers work with dangerous work postures. The bivariate test results showed that there was no significant relationship between the variables age (p=0.954), years of service (p=0.192), smoking habits (p=0.233), BMI (p=0.330), and work posture (p=0.499) with complaints of MSD. There was no relationship between age, length of service, smoking habits, BMI, and work posture with MSD complaints. We recommended that business owners collaborate with the Community Health Center to provide education about ergonomics when working for refill drinking water depot workers.

The lack of community participation in posbindu activities is due to lack of public knowledge, community awareness of non-communicable diseases, poor attitudes and behavior. It is necessary to create a posbindu model based on community needs. The aim of this study is to develop a Posbindu (Integrated Service Post) model using a local cultural approach. Qualitative approach using in-depth interviews with 4 informants aimed at obtaining more in-depth information and Focus Group Discussion (FGD) were attended by 10 participants, consist of 4 males and 6 females, whose ranged from 38-55 years. FGD analysis resulted in several themes, namely: treatment compliance, health education, treatment seeking behavior, family support, policies, and the posbindu model with a local cultural approach. Obtained a posbindu development model with a local cultural approach, according to the needs of the community. The Posbindu model developed using a local cultural approach, specifically Sundanese culture, aligns with the cultural background of the informants. By creating a Posbindu model tailored to the community's needs, it is expected to enhance community participation in Posbindu activities, thereby improving compliance with hypertension treatment. Health promotion programs using local culture, such as during botram activities, have proven effective in increasing public knowledge about hypertension

Abstract

Excess weight in children and adolescents is a global health problem that can result in various serious complications. One of the main factors that trigger obesity is lack of physical activity, which is largely influenced by the length of time using screen devices. Screen use of more than two hours a day has been linked to increased sedentary behavior, decreased physical activity, disrupted sleep patterns, and unhealthy eating patterns, all of which contribute significantly to the risk of obesity. This study aims to assess the relationship between duration of screen use and nutritional status among students at SDN 060838 in Medan City. With a cross-sectional analytical research design, the sample used was 50 students selected from a population of 198 students. Data were analyzed using the Mann-Whitney test, which showed a statistically significant relationship between screen time duration and nutritional status (p < 0.001). These results emphasize the importance of managing screen time as a key element in preventing obesity in children. Appropriate policies and guidelines are needed to support their health and well-being, especially amidst the increasing use of screen devices for social and educational needs.
