The Social Security Organizing Agency (BPJS) is an institution formed to organize a Social Security Program in Indonesia based on Law Number 40 of 2004 and Law Number 24 of 2011. Indonesian General Imelda Worker Hospital is a private hospital in the city of Medan who have collaborated with BPJS Based on the initial survey of researchers on 30 medical records documents 13 files (43%) are in accordance with claim requirements and 17 files (56%) are incomplete. Data collection in this study is a questionnaire method. This research was conducted using quantitative research methods with cross sectional approach. The population used in this study were all medical record officers in the medical record unit in the IPI General Hospital Medan as many as 31 officers. Based on the results of a statistical test research conducted using the Chi Square (X2) computer test application with a significance of 95% (p <0.05) it can be concluded that the Hospital conducts an evaluation for medical records officers so that the performance of officers is as expected. Increase evaluation for the completeness of medical record documents used in BPJS claims.

Queue is an occurrence where someone has to wait their turn to get service. Queuing processes in hospitals when processing outpatient registration medical records at the hospital often occur especially during peak hours. To overcome this problem, a solution is needed to improve service performance at the hospital. The queuing model used is the Multi Channel-single phase queuing model which has one or more services flowing by a single queue. The variables to be observed are time between arrivals, service time data and number of services assuming a Poisson distribution pattern. The results of this study will show the operator's busy time is 83.33%, the number of queues in a certain period (Lq) is 13 patients, the number of registrants in the system (L) is 14 patients, the waiting time in the queue (Wq) is 24 minutes, and waiting time in the system (W) is 30.06 minutes.

Every health service provided to patients must recorded in the medical records completely because the completeness of filling the medical record is used as an indicator of the quality of health services in the hospital. The purpose of this study is to determine the incomplete picture of filling out an anesthetic case report form in a surgical case at the Imelda Indonesian Workers Hospital in Medan. The type of research is descriptive. The populations of this study are all forms of anesthesia patient case reports for the first quarter of 2020 with a total of 728 forms. The samples are 88 forms taken using systematic random sampling. Data collection was carried out using the checklist sheet. Data were analyzed univariate to describe the characteristics of each research variable by producing a frequency distribution and percentage of each variable. The results showed that there were 24 anesthetized report forms that were completely filled and 64 anesthetic report forms that were not completely filled. The highest percentage of incompleteness of filling in the patient identity component on the item date of birth was 10 forms (11%), on the anesthesia time component on the old surgical item by 39 forms (44%), on the diagnostic component on the post-surgical diagnostic item by 28 forms (32%), the examination component on the preoperative state item was 63 forms (72%) and the signature component was 2 forms (2%). It is recommended that every health worker who provides health services to surgical case patients fill out the anesthetic report form in accordance with the SPO authorizing the medical record filling, and to the medical record officer to conduct an assembling or complete analysis of medical record documents.

Health education is one of the techniques to increase someone’s knowledge level due to the aim of improving knowledge and well understanding regarding microplastic and microplastic pollution. This research aimed to analyze the effect of health education towards the knowledge regarding macroplastic and microplastic pollution among students at the medical record and health major in the college of health sciences Imelda in terms of health education. This research used a quantitative approach with a Quasi-Experimental Study Design of a non-equivalent control group. The population was all the students with the medical record and health information major. The sampling technique used was purposive sampling, all the first-year students. The group of the sample consisted of two groups- the intervention and the control group. The intervention group consisted of 73 people who have given health education. Control group consisted of 73 people who have not given any health education. The statistic test used was the Wilcoxon test. The statistic result on the knowledge variable among the experiment group who had a good knowledge level refer to the pre-test score only 4.1 %. After given health education, the increasing number of people who had a good knowledge level (76.1%). The good knowledge level among the control group only showed 1.4% (pre-test and post-test). The result showed there was a significant increase after given health education among the experiment group (p=0.00). However, another result showed there was no significant difference among the control group (p=1.00). Health education towards macroplastic and microplastic pollution had a significant influence to the knowledge level for students. It is recommended for further research to examine the role of knowledge towards attitudes and behaviors or actions in the management of macro and microplastic waste pollution. Based on the results of the pre-test where respondents still have less and moderate knowledge while the issue of plastic waste in Indonesia requires full attention and more participation from the community from society, it is necessary to get the attention of educational institutions to further elaborate and educate students in pollution macro-plastic and microplastic waste.

The outpatient registration place is the place where patients first met with the officer, ideally the average waiting time for record services is 10 minutes. The purpose of this study was conducted to determine patient satisfaction with the waiting time for the provision of medical records in RSU Imelda Workers Indonesia Medan. Methodology This research uses descriptive research method with a quantitative approach. The tools used were questionnaire and Likert scale. The population in this study were all outpatients. The sample size was 30 patients and the medical record file. Data analysis uses univariate analysis. The sampling technique uses accidental sampling. This research was conducted at the Imelda Indonesian Workers Hospital in Medan. From this study the results obtained by the time required by officers to provide medical record files are 11 medical record files (36.7%) right and 19 medical record files (63.3%) incorrect. The level of outpatient satisfaction based on 5 dimensions, namely the Responsiveness dimension, said 12 patients were satisfied (40%), the Reability dimension stated 19 patients were dissatisfied (63.3%), the Assurance dimension was satisfied as many as 15 patients (50%), the Emphaty dimension stated as many as 14 patients (46.7%), and Tangible dimensions stated that they were quite satisfied as many as 17 patients (56.7%). Conclusion, The level of patient satisfaction is mostly stated quite satisfied. Suggestions, It is necessary to improve the performance of health services from the Reability dimension through work culture efforts, such as the formation of a compact working group because they have received training in accordance with technological developments and community expectations, so that health care workers are responsive, timely and accurate in serving patients.

Quality of service is the overall characteristics of the service that shows its ability to satisfy the needs of consumers, and the Health BPJS is a legal entity formed to carry out a health insurance program, and patient satisfaction is the level of patient feelings that arise as a result of the performance of health services obtained after the patient compares it with what he hoped for. The purpose of this study was to determine the quality of medical record officer services to the level of satisfaction of outpatient BPJS patients in General Hospital. IPI Medan 2018. This research uses Analytical research design. The independent variable is BPJS Health service quality and the dependent variable is patient satisfaction. Samples taken using the Slovin Formula found 33 patients in the Outpatient Installation at the General Hospital. IPI. Medan in 2018. The research instrument used a questionnaire. The results showed that 28 respondents (84.8%) stated that the quality of BPJS Health services was good, and 32% of respondents (97.0%) expressed satisfaction with BPJS Health services. Chi Square statistical test results state there is a relationship between the quality of BPJS Health services with patient satisfaction Outpatient Installation at the General Hospital. IPI. Medan Year 2018 (pValue = 0,000). The implication of the results of the study shows that the quality of BPJS Health services has an important role in influencing patient satisfaction.

Patient registration is the foremost part of Hospital service, because the patient registration records the identity and requirements of his visit to the hospital, and at the time of registration the patient gets the first impression of the hospital's services. The purpose of this study was to determine the stages of admission of inpatients at Mitra Sejati Hospital Medan and to find out the obstacles in the implementation of inpatient admissions at Mitra Sejati Hospital in Medan in 2017. The type of research used is descriptive using a quantitative approach, the research sample is 5 inpatient registration officers, data collection was done by interviewing the inpatient registration officer. The results showed that the registration stages of inpatients at Mitra Sejati Hospital Medan had not met the regulations of the PERMENKES (2008) on the naming and use of KIB and KIUP. It can be concluded that Mitra Sejati Hospital Medan has not implemented the naming rules according to the PERMENKES and has not implemented the use of KIB and KIUP. It can be suggested that registration officers should have implemented a naming system according to the PERMENKES rules and already have to implement the use of KIB and KIUP to make it easier for officers to register patients and find patient medical record files. One form of service at the hospital is the distribution of medical record files. Distribution is a process of delivering goods or services from producers to consumers and users, when the goods or services are needed. The purpose of this study was to determine the distribution of outpatient medical record files at the UPT Special Paru Hospital in 2019. This study used a descriptive research method with the aim of making an objective description or description of a situation with a total population of all medical record staff employees, namely 5 people and the sampling technique in this study is total sampling with the characteristics of all medical record officers. So the researchers analyzed the data starting from editing, coding, sorting, data entry, cleaning. The results of the study based on the majority of high school education were 3 people (60%) minority DIII medical records as many as 1 person (20%) and S1 education as many as 1 person (20%), based on the majority of 1-5 years working duration as many as 4 people ( 80%), and a minority of 6-10 years of work as many as 1 person (20%). So it can be concluded that the implementation of the distribution of outpatient medical record files is said to be sufficient, the length of the process of distributing medical record files and there are still a few medical record officers who are educated in DIII medical records and can be expected to improve the process of sending medical record files and the addition of officers with DIII medical record education.

Completeness of the Inpatient Examination Form is a problem that often causes complaints in several hospitals. The purpose of this study was to conduct a literature review related to the incomplete analysis of filling in form A, MPP Initial Evaluation and Form B Implementation Notes. The collection method is secondary data, the steps are determining keywords, exploration and title selection, reading articles that have not been eliminated. At that stage, the reference list of selected articles is reviewed to determine other related studies. The articles contained in the summary list The results of the literature review, namely, still found incompleteness in filling out medical record forms, the responsibilities and attitudes of officers in filling out medical record files can be said to be still not optimal. The human resources involved are still lacking, for example the assembling officer also serves the file preparation so that the assembly function is not optimal. Based on the literature review of several related articles, further research is needed on other causes of incompleteness in filling out medical record file forms and the obstacles experienced by doctors in filling out medical record files.

The medical record is also a documentation of the patient's condition and the contents of the medical record are medical secrets, especially in the storage of status in the filling room, which must be kept confidential, secure, and the medical record layout, whether it is appropriate or not by every health worker. The purpose of this study was to determine the Knowledge Review and Attitudes of Officers and Patients on the Confidentiality of Medical Record Files Primary Health Center at Kuranji Padang. This type of research is descriptive with a quantitative approach, the research sample is 90 patients and 4 health center officers using accidental sampling technique with data collection using questionnaires for patients and table cklis for officers. The results showed that there are still officers whose knowledge and attitude in maintaining the confidentiality of medical records (50%) or 2 people are still lacking and more than half of the knowledge (51.1%) and attitudes (52.2%) of patients towards maintaining the confidentiality of record files medical who do not understand and know. It can be concluded that Kuranji Primary Health Center, both patients and officers, is still lacking in maintaining the confidentiality of medical records. It is hoped that the head of the Primary Health Center will prepare Standard Operating Procedures (SOP) on medical records so that officers and patients know about medical records and about the confidentiality of medical records. In implementing the confidentiality of medical records, competent and professional medical record officers should know and understand more about medical records, participate in training and provide information to patients so that the confidentiality of medical records is maintained and can create good service.
