The aim of this research is to determine the influence of increasing access to health for children on increasing the role of family doctors in improving counseling so that children can improve their growth and development. The existing research method is to use a quantitative descriptive approach using analytical methods. From the results of existing research, it can be explained that the average age of children in the Medan city area whose health is disturbed is in the age range of 11-14 years and who do not have access to adequate health services. The results of the research also state that improving poor access to health which is related to the lack of knowledge and skills of family doctors will worsen children's health conditions, and the lack of the role of doctors, especially the lack of counseling, will actually make children's health conditions worse. The conclusion are increasing access to health services and increasing counseling can speed up children's recovery in a comprehensive manner, where access to health services and counseling provided by family doctors will increase children's enthusiasm to recover and be healthy again, so that they will easily increase their immune system and avoid existing diseases.

Medan is a city in the Sumatra region, where the number of children in Medan City is 776,345 people, while the number of mothers in Medan City is 530,988 people, where more than 50% of the population in Medan City is affected by infectious diseases due to environmental conditions. densely populated residential areas, poor sanitation levels, worrying air and water pollution conditions, poor air circulation conditions and inadequate air ventilation availability cause the health of the community, especially mothers and children, to be disturbed, which will result in medical staff are not providing the service properly, fearing that there will be a high transmission of the disease, thus making access to health services less than optimal, even disrupted which makes the services provided inadequate, thus causing the maternal and child mortality rate to increase by 40-50 %. This research aims to understand and analyze the influence of the environmental conditions where residents in Medan City live on access to maternal and child health services which is less than optimal. The method used in this research is the analytical descriptive method. The population in this study is that the number of children in Medan City is 776,345 people, while the number of mothers in Medan City is 530,988 people, while the sample size is 88 respondents consisting of of 44 mothers and children in the city of Medan who were affected by disease due to poor environmental conditions. The data collection techniques were carried out through observation and documentation studies. The results of the research show that the population with the most illnesses due to poor environmental conditions are mothers aged 25-29 years and children aged 10-14 years, while the number of mothers and children who tend to have bad illnesses due to inadequate sanitation conditions, thereby increasing the number of viruses or diseases that can be transmitted from sanitation that is not clean and good, which will cause disease conditions to worsen, thereby disrupting the health and activities of the population in a society so that they do not have appropriate ways to deal with them. deteriorating quality of the condition of facilities in the house and around it.

Medical Record Document (DRM) is a tool to record, recording the transactions, Quality of service can be enhanced when supported by the safety and confidentiality of medical record patient in the storage room. Security and confidentiality of medical records in RS Vina Estetica were still not going well. Still found leaking roof space of filing, shelving humid, dusty, absence of camphor, the door is often not locked, still found the patient requests their own medical record and bring the document it self, and document medical records piled up on the floor. The main problems were the lack of procedures of security and confidentiality of medical records document. Therefore, reseacher wanted to describe aspects of security and confidentiality the medical record documents on filing unit in RS Vina Estetica. This study was a descriptive, cross-sectional approach and methods were observation and interviews. Subjects were filing officer as many as five people, and object was the security and confidentiality of medical records in Filing. The research instrument used were guidelines for observation, interview and gauge the temperature of the moisture. Data processing is editing, tabulation, and data presentation. Based on the observation of security medical records processing in filing RS Vina Estetica can be seen from the physical aspect was still not safe from the dangers of fire, and flood, in terms of biological not use camphor on each shelf, and in terms of chemical officers were still doing activities such as eating and drinking in the filing room. From the aspect of confidentiality, unlocked door and patients or staff other than medical record personel enter the filing room. There was no policies or standard operating procedures governing the security and confidentiality of medical records document. To maintain the security and confidentiality of medical records document, it needs facilities and infrastructure and the rooms were quite safe and good for maintaining confidentiality medical records document. Policies and procedures need to be made that contains rules in providing good service.

Sundari hospital Medan is a type C hospital, that has been used as guidelines ICD-10 for coding, the hospital had not conducted a study to determine the accuracy of the determination of primary diagnosis code based on specification writing primary diagnosis in the medical record document in the inpatient permata medika hospital 2020th periode.Rom the results of the initial survey using interviews with officers, inaccuracies in the coding of diagnosis were found on the medical resume of inpatients.This study used an observational method with a cross-sectional approach and the type of analytic research, while the population of this study amounted to 2,280 files of inpatient medical records for the 2020th periode so that a sample of 99 that are retrieved by using a random sample of sampling techniques.The results of the observations concluded that the main diagnosis code that was accurate from the specific diagnosis (Thypoid Fever) 54 (85,7%) was the main diagnosis code that was accurate from the non-specific diagnosis (Fever) 9 (14,3%), and the main diagnosis code was not accurate from specific diagnosis (Pneumonia) 14 (42.4%) the main diagnostic code was inaccurate from the non specific diagnosis (Effusion) 19 (57.6%).Conclusion obtained that is, to get the accuration of disease code, do not only influenced by writing diagnosed just specific especial, but influenced also by correctness of officer coding and also other factor which influencing in consequence officer koding better be active in searching information if finding diagnosed especial which is not specific and also need the existence of the make-up of knowledge of officer koding by joining in training of Coding ICD-10.

Background: Returning medical records is the process of taking medical records from the borrowing service unit back to the medical records unit. Based on a preliminary study carried out at the medical records installation at RSU Sundari Medan, it is known that there are still medical record files whose returns are not on time or within 2x24 hours. Objective: To analyse the inaccurate time for returning medical record files from the inpatient room to the medical records installation and to determine the factors causing the inaccurate time for returning inpatient medical record files at RSU Sundari Medan. Research Method: The research used is descriptive research, namely describing the analysis of data collection on inaccurate times for returning inpatient record files to the medical records installation at Sundari RSU Medan. Results: Based on the results of the analysis of 2280 patient medical record files at RSU Sundari Medan, the percentage of timely return of medical record files was 46%, and the inaccurate time of return of medical record files was 54%. Factors that cause inaccurate returns of inpatient medical record files are incomplete filling out of medical record files, especially in medical resume forms, such as incomplete diagnoses and Dr. signatures, and a and a lack of communication and training regarding the correct time for returning medical record files.

Returning medical records is the initial activity of processing patient medical record files before proceeding to the next stage. This study aims to determine the quality standards for returning medical record files for inpatients at RSU Sundari Medan, return time, delay time and factors causing delays in returning medical records. The research was conducted at RSU Sundari Medan using a mixed methods approach. Data collection was carried out from January to March 2021, using secondary data originating from 985 medical record files based on documentation of the return of inpatient medical record files in February 2021 and primary data originating from interviews. Data were processed using programs, spreadsheets, with univariate analysis. The research result of returning medical records is a process of taking medical records from the borrowing service unit back to the medical records unit. Return of medical record documents is declared late if it exceeds the return time limit, which is a maximum of 2 x 24 hours. After the patient is discharged from the hospital. The rate of delays in returning medical record files from January to March 2021 reached 58%. The causes of delays are influenced by the doctor's discipline in filling out the patient's resume, incomplete documents in the patient's status, and human resources.

Hospital, what is meant by hospital is a health service institution that provides comprehensive individual health services that provide inpatient, outpatient and emergency services. A hospital is a place that provides short-term and long-term medical services which include observation, diagnostic, therapeutic and rehabilitation activities for all people suffering from illness or injury and giving birth. The aim of this research is to analyze the depreciation activities of medical record archives at Setio Husodo Kisaran Hospital. This research uses a qualitative research design with a case study approach. The data collection techniques used were observation, interviews and literature study. Data analysis in this research is data reduction, data presentation, and drawing conclusions. The results of this research explain that the House Setio Husodo Kisaran Hospital last carried out depreciation of medical record archives in 2008. Currently, depreciation is carried out by moving inactive medical record archives after a storage period of 5 (five) years for patients seeking treatment. Limited human resources and a lack of understanding by officers about the Archive Retention Schedule (JRA) are the main obstacles to the depreciation of medical record archives.

Background: A method of writing progress notes that explains three things, namely what has happened to the patient, what is planned for the patient, and how the patient reacted to therapy. Based on a preliminary study conducted by researchers at the medical records installation at H. Adam Malik General Hospital, Medan, it was discovered that there were still incomplete records in the outpatient integrated service development record form, the signature and clear name of the doctor. Objective: To find out the percentage of completeness and incompleteness in filling out the outpatient integrated service progress note form and to find out the factors that cause incompleteness in filling out the outpatient integrated service progress note form. Research Method: This type of research is descriptive research with a qualitative approach. The research design used is a case study. The subjects in this study were 2 medical records unit officers. The objects in this research were 398 outpatient medical record files. Data collection techniques using interviews and documentation studies. This research was conducted via telephone due to Covid-19. Results: Based on the results of the analysis of 398 outpatient medical record files which contained an outpatient integrated service development record form, the results obtained were the percentage of completeness of recording of 398 samples of medical record files, there were 34 medical record files completely filled in and 351 incompletely filled out and 13 of them were missing. at all. The factor that causes incomplete filling is the doctor's busyness in treating patients because of the patient's condition.

Background: Health service facilities are places where health services are carried out which can be used for medical or dental practice, one of the health service facilities is a hospital. In supporting health services in the form of conveying/providing information about the patient's condition, it is provided by the doctor who carries out medical procedures, whether requested or not requested by the patient. Objective: To describe the completeness of informed consent at North Sumatra University Hospital in March 2018. Method: in this research is descriptive with a qualitative approach. The research design used in this study was cross sectional, namely data on the completeness of approval for medical procedures in the surgical ward in March and the research object was 70 sheets providing information on medical procedures. Meanwhile, the object of this research is a sheet providing information on medical procedures. Data collection using observation, interviews and documentation studies. Results: The patient's right to receive a complete detailed explanation includes: diagnosis and procedures for medical procedures, the purpose of the medical action being carried out, other alternative actions and their risks, risks and complications that may occur, prognosis for the action being carried out. The completeness of filling out the sheet providing information on medical procedures is known to be 100% complete in filling out identification items, 78.57% in complete information giving items, 37.14% in information recipients, 82.1% in alternatives, and 84.2% in doctor authentication and 55.71% in patient authentication. as for factors that influence the completeness of Human Resources

Background: Assembling officers must make efforts to maintain the completeness of medical record files. By making these efforts, it is hoped that they can complete incomplete medical record files. at the University of North Sumatra Hospital. The results of interviews with the Head of Medical Records and Documentation Studies for inpatient medical record files revealed that this goal had not yet been achieved because incomplete inpatient medical record files were still found. Research Objective: to determine the efforts that can be made to maintain the completeness of medical record files. Research method: This type of research is descriptive research with a qualitative approach and a cross-sectional research design. The subjects in this research were assembling officers, two doctors, two nurses, and the head of the Medical Records agency. Data collection techniques are observation, interviews and documentation studies. Research Results: the causes of incomplete medical record files are a) Man: nurses prioritize service or treatment of patients; nurses still don't remind doctors of the parts of the medical record that must be filled in by the doctor, the doctor visits once a week; Doctors must immediately treat patients who need help. b) methods; the procedure for filling out medical record files has not been socialized to doctors or nurses. c) machines; There is no date stamp facility. e) Money; There is no incentive system if filling in medical records is still incomplete.
