Abstract

Introduction: The ABC (Aneurysmal Bone Cysts) lesion has been reported in the orthopedic literature for more than 60 years; it was first described in 1942 by Jaffe and Lichtenstein. Aneurysmal bone cyst (ABC) in pelvic is a benign, expansile osteolytic lesion that typically affects the metaphysis of long bones in the second decade of life and occurring in patients less than 20 years. The Several ABC treatment modalities had been utilized including wide resection, intralesional resection/curettage with or without different adjuvants, radiation, embolization, intralesional sclerotherapy. Case Report: We present a rare case of a huge aneurysmal bone cyst (ABC) in the iliac bone, occurring in the male patient 4th decade of life. The patient presented with a history of pain and huge pelvic mass size 8x6x5 cm extend to abdominal region for 5 years. Plain radiograph and computed tomography showed expansile lytic lesion arising from the all right iliac bone extend to Lumbar 5. A biopsy was performed and histology confirmed diagnosis of aneurysmal bone cyst. The patient was managed surgically with hemipelvectomy type 1 and Autogenous bone graft from fibula and reconstruction with plate and screw for pelvic ring with and re- attach the muscle on it. Follow up 5 months after surgery the patient can squat and stand maximally with no pain, nor recurrence and can return to normal activity. Conclusion: Early diagnosis and complete resection of tumor and treatment of relevant symptoms represent a viable treatment for this rare disorder to achieve increased life expectancy, low recurrence of tumor and improved quality of life.

Abstract

Introduction: Abdominal wound dehiscence (AWD) is a complication of severe postoperative abdominal surgery, with reported death rates ranging from 10% to 45%. Significant mortality, prolonged hospitalization, increased incidence of incisional hernias and reoperations for ruptured stomachs, with costs associated with the community, emphasize the severity of these complications. The VAMC score and KIMS-14 can be used as screening in predicting surgical injury dehiscence Method: This study is a diagnostic test study to assess the sensitivity and specificity of VAMC and KIMS-14 scoring in predicting the occurrence of abdominal wound dehiscence to be performed in surgery outpatient and digestive surgery ward at the General Hospital, Dr. Mohammad Hoesin Palembang in the period March to May 2019. Results: There were 44 subjects that participated in this study. VAMC has a sensitivity value of 87.5% and specificity of 97.2 with an area under curve value of 0.958 with a cut-off of > 10. KIMS 14 has a sensitivity value of 100% and a specificity of 94.4% with an area under curve value of 0.944 with a cut-off of > 5. Conclusion: KIMS-14 is better in sensitivity, but VAMC is more specific to predict dehiscence licensing in patients undergoing intraabdominal surgery.

Abstract

Introduction: Rupture of the Achilles tendon has become an increasingly common case in the last two decades with an incidence of 18% per 100,000 population. The controversy about the best treatment arises because the results are determined by the method of tendon repair and postoperative functional rehabilitation. In recent years, there is a great deal of interest in hyaluronic acid which is found in certain amounts in the soft tissue extracellular matrix and joint fluid. Several experimental studies have shown the application of hyaluronic acid (HA) to heal tendons and stimulate tendon healing and reduce adhesion formation. This research is to find out the effectiveness of injecting hyaluronic acid in behavior changes in mice that have modified Achilles tendon rupture in 21 mice divided into three groups according to their treatment, namely intramuscular HA (IM), Intralesional HA (IL) and NaCl in laboratory animals Sriwijaya University Faculty of Medicine 2019 by seeing changes in behavior every 7 days 3 times. Results: The group with HA-IL treatment had the highest cure success rate, which was 7 (58.3%) good grades, 5 (41.7%) good grades and 2 (100%) very good grades and 5 (33.3%) good and 2 (50%) extraordinary (change behavior 1, 2 and 3, respectively). Then HA-IM 5 (41.7%) Good grades, 7 (58%) Good grades, and 5 (33.3%) and 2 (50%) exceptional grades (change behavior 1, 2 and 3, respectively) and NaCL only have a value of 5 (33.3%) both for behavior change 3. Conclusion: There is a difference in effectiveness between administration of hyaluronic acid and without administration of hyaluronic acid to behavioral changes in repair of the Achilles tendon.

Abstract

Introduction: Pulmonary contusions can cause a progressive inflammatory response. Activation of TNF-α cytokines and reactive oxygen species (ROS) can cause pulmonary cell death. Antioxidants can have the potential to neutralize ROS. The purpose of this study is to determine the effectiveness of antioxidant administration in maintaining pulmonary cell function in wistar rats that have been induced to experience pulmonary contusions through caspase-3 levels. Methods: This study was an in vivo experimental study conducted on thirty male wistar rats and divided into five groups (n = 6): control, pulmonary contusion + asthaxanthine 5 mg/kgBW, pulmonary contusion + vitamin C and E 50 mg/kgBW, pulmonary contusion + vitamin C and E 100 mg/kgBW, pulmonary contusion + vitamin C and E 200 mg/kgBW. The value of Caspase-3 is evaluated by the IHC. All data analyzes used SPSS 18. Results: Low doses of antioxidants have the potential to reduce pulmonary cell death in wistar rats induced by pulmonary contusions. Conclussion: Vitamin C and E effective to reduce polmonary cell death in pulmonary contusion.

Abstract

Introduction: CD8 + CTLs are important components of tumor specific cellular adaptive immunity in breast cancer. Chemotherapy can increase the response of cytotoxic lymphocytes and provide anti-tumor immunity. Therefore, chemotherapy can cause cell death and trigger tumor antigens that are processed by APC and activate CD8 + cells to destroy tumor cells. Evaluation of the immune system before treatment can predict chemotherapy response. The purpose of this study was to assess the effect of neo-adjuvant chemotherapy on serum CD8 + levels in patients with locally advanced breast cancer. Methods: Non-comparative clinical trials were conducted at Dr. Mohammad Hoesin Hospital Palembang from October 2017 to June 2018.The sample of this study was 30 breast cancer patients who met the inclusion criteria. All samples underwent FAC neo-adjuvant chemotherapy and data were analyzed with SPSS version 21. Results: The mean age of advanced breast cancer patients was 45.97 ± 10,526 years with an age range of 30-66 years. Serum CD8 + levels after chemotherapy decreased significantly compared to before chemotherapy (p = 0.000). The value of serum CD8 + before chemotherapy has a sensitivity of 42.86% and specificity of 43.48% with a cut of point of 660.7 cells / mm3. Conclusion: Neo-adjuvant chemotherapy significantly decreases serum CD8 + levels in local advanced breast cancer patients.

Abstract

Background: Pelvic arthroplasty in the elderly is increasingly being performed, along with the increasing life expectancy in Indonesia. However, postoperative mortality must be taken into account. Methods: Prospective cohort study of mortality and morbidity up to 30 days in 38 patients who met the inclusion criteria. The observation of changes in the value of Procalcitonin and C-Reactive Protein was carried out preoperatively, after surgery on day 2 and after surgery on day 7. Results: There was no mortality. There was 18% redness output in the analyzed surgical wound. The cut off of Procalcitonin used Fisher's Exact test (RR = 3.088, p = 0.207), while the cut off of C-Reactive Protein used Fisher's Exact test (RR = 1.647, p = 0.678). Conclusion: There was no significant relationship between procalcitonin levels and C-reactive protein and postoperative redness.

Abstract

Total elbow arthroplasty (TEA) has proven to be a reliable joint replacement procedure that has a high degree of patient satisfaction. Despite these favorable outcomes, few patientswith disabling elbow degenerative conditions have TEA recommendedto them as an alternative procedure by rheumatologists, physiatrists,or orthopedists. The semi-constrained, hinged (linked) prosthesis is the most commonly used prosthesis.This prosthesis is stable postoperatively. Patients are encouraged to do range of motionexercises and use their elbow for activities of daily living as dictated by their pain leveland status of wound healing. Wound management is critical following TEA. A posterior triceps–sparing approach, which preserves the continuity of the triceps, whenpossible, is typically used. Postoperatively the therapist and patient need to respect the integrity of the triceps and posterior incision when performing both active and passiveelbow flexion exercises and functional activities. The surgeon may choose to limitflexion range of motion based on the intraoperative inspection of the triceps tendon. We have study on 5 cases patients with elbow arthrosis and went TEA for management of elbow pain and joint limitation of movement.

Abstract

Background: Hemodialysis has been accepted as a method of treatment in patients with CKD in stage 5. A-V fistula is the closest ideal hemodialysis vascular access, so that it can reduce morbidity and mortality of CKD patients. Methods: is a retrospective descriptive study with data obtained from the A-V fistula surgery form archive in the Vascular and Endovascular Division of the Department of Surgery, FK UNSRI in January 2018-December 2018 and from the patient's medical record. Data processing was carried out with SPSS 16.0. Results: of the study obtained 203 patient data with the use of AV Shunt that met the inclusion criteria. The mean age of patients was 50.53 years. There were 114 (56%) male patients and 89 (44%) female patients. Risk factors such as smoking were found in 9 (4%) patients. History of the disease prior to experiencing chronic renal failure was hypertension in 115 patients (56.6%) and diabetes in 81 (40%). There were 69 patient’s Hb data before AV shunt with Hb <7 (1%) in 2 people, 7-8.9 (33%) 67 people, 9-10.9 (53%) 108 people, > 11 (13%) 26 people. 128 (63%) patients had double lumen catheters installed in the right jugular, 5 (2%) patients were placed in the left jugular, and 70 (34%) patients did not have a double lumen catheter. Conclusion: A total of 169 (83%) patients had a history of using AV Shunt. There were 71 (35%) patients with the wrist region anastomosis, 132 (65%) patients with cubiti region anastomosis. Native AV shunt was the most commonly performed in 202 (99.5%) patients. There were no recorded complications after AV Shunt placement in 99 (48.8%) patients, 51 (25.2%) patients had AVF branches, 23 (11.3%) had AV Shunt rupture, 18 (8.8%) AVF stenosis, 1 (0.5%) central stenosis, 1 (0.5%) thrombus, and 11 (5.4%) infections. A total of 167 (82.2%) patients took AV Shunt after 3 months.

Abstract

Introduction: Lateral ear dogs and loss of anterior axillary fold are side effects of modified radical mastectomy (MRM) operations and are cosmetically unpleasant. Songket technique is expected to reduce the side effects of MRM surgery. This study compared the Modified Radical Mastectomy surgery scar using Songket and standard techniques. Methods: This research is a clinical randomized trial. With anterior chest thoracic data of postoperative patients and assessed by the patient's family and medical professionals using a likert scale on wound length, wound width, wound angle and armpit folding. This study was performed in the Department of Surgery Mohammad Hoesin Hospital Palembang from January 2020 to April 2020. Results: A total of 32 research subjects participated with the average Likert scale score on medical and family assessments between the Songket and standard technique groups with p values ​​of 0.605 and 0.984, respectively. Conclusion: There was no difference in comparison of scar Modified Radical Mastectomy operations using Songket techniques and standard techniques at Mohammad Hoesin Hospital in Palembang.

Abstract

Introduction. Acute appendicitis is the most common case of acute abdomen. Diagnosis of acute appendicitis is still difficult and is one of the problems in the surgical field. The Clinical Scoring System (CSS) has been developed to help doctors classify risk categories. PAS has been widely evaluated in the pediatric population. In another study it was found that RIPASA had better sensitivity, specificity, and accuracy than PAS in pediatric patients. Methods. This study is a cross sectional study to assess the level of concordance between RIPASA and PAS scores in diagnosing acute appendicitis, with the gold standard of Histopathological examination. Samples were taken by consecutive method, in 30 patients aged <18 years for 1 year. Results. This study had an average age of 10.10 ± 3.745 years. Histopathologically early acute appendicitis 3.3%, acute suppurative appendicitis as much as 20%, acute gangrenous appendicitis 73.3% and others 3.3%. Using a cut-off point value of 9.5 for RIPASA and 7 for PAS, the sensitivity, Specificity, Accuracy of 82.75%, 100%, 80% for RIPASA, and 75.8%, 100%, 73.3% for PAS. Conclusion. The RIPASA score on the cut-off treshold 9.5 has better sensitivity and accuracy than PAS in diagnosing acute appendicitis in pediatric patients and can be used as CSS to assist in making decisions regarding the diagnosis of acute appendicitis in children.
