Abstract

Introduction: to determine the correlation between duration of ethambutol and copper levels in serum with color vision disorder examined with Farnsworth Munsell 100 Hue (FM100) in patients with tuberculosis. Methods: This study was an explorative observation of 20 samples during November 2017 - January 2018 period. Comparative analysis using independent t-test, paired t-test, Wilcoxon test and Mann-Whitney test, while correlation analysis using Spearman's test and Pearson's test. Results: There was a significant difference of FM100 value before and after in category I (p=0.000) with increase of total error value 11.80+5.31 or category II (p=0.000) with increase of total error value 15.00+7.41. There was significant difference of Cu2+ level before and after in category I (p=0.005) with decrease of Cu2+ level 15.57+5.04 mg/dL or category II (p=0.005) with decrease of Cu2+ level 31.66+7.35 mg/dL. There was significant correlation between color vision and duration of ethambutol (p=0.000), but there was no significant correlation of Cu2+ with color vision (p>0.05). Conclusion: The duration of Ethambutol was related to changes in color vision examined with FM100.

Abstract

Abstract Background: Basal cell carcinoma (BCC) is a malignant tumor originating from non- keratinizing cells originating from the basal layer of the epidermis. Basal cell carcinoma is the most common malignant tumor, nearly 90% of all non-melanoma skin cancers. Clinically, BCC is difficult to distinguish, final diagnosis determined based on histopathological examination of complete tumor tissue. Based on histological growth patterns, BCC is divided into two, non- aggressive type (nodular and superficial) and aggressive type (infiltration, micronodular, fibroepithelial, basoskuamosa, keratotic, adnexal differentiation). The examination technique by analyzing the proteins or genes involved is related to the aggressiveness of basal cell carcinoma. Objective: This study was conducted to determine the comparison of expression of Bcl-2 in periocular basal type aggressive and non-aggressive carcinomas in dr. Mohammad Hoesin Palembang. Methodology: A cross sectional analytic study from 26 periocular basal cell carcinoma patients in RSUP dr. Mohammad Hoesin Palembang, January 2014- June 2018. Histopathologic features were examined with hematoxylin-eosine staining and imunohistochemical were examined with Bcl-2 monoclonal antibody. Data were analyzed by bivariate analysis, using chi-square test and fisher exact test. Results: Positive Bcl-2 expression was more common in periocular basal cell carcinoma (93.8%) than aggressive (50%), and nodular type (93.8%) compared to other types of histological differentiation. Positive Bcl-2 expression was also higher in female patients (90.9%) than males (66.7%). There is a significant relationship between expression of Bcl-2 and growth patterns, differentiation of cell histology and also sex of patients with basal cell carcinoma. Conclusion: In this study, there was a significant correlation between expression of Bcl-2 and aggressive and non-aggressive periocular basal cell carcinoma in RSUP dr. Mohammad Hoesin Palembang.

Abstract

Abstract Background: Phacoemulsification is a technique of modern cataract surgery with smaller corneal incision than conventional techniques but uses various effects on springs that cause dry eye syndrome (DES). DES is a multifactorial defect of tears and ocular surfaces characterized by eye discomfort. Examination of DES can be done by examining Schirmer 1 and Tear Break Up Time (TBUT). Purpose: To determine the accuracy, sensitivity and specificity of the Schirmer test dan TBUT in postoperative patients with phacoemulsification techniques at the Rumah Sakit Khusus Mata Province of South Sumatra. Method: Diagnostic test has been done in order to the accuracy, sensitivity and specificity of the Schirmer test dan TBUT on DES in post operative cataract surgery with phacoemulsification techniques. This study has been conduct Schirmer 1 test and TBUT was done periodically from 1 week, 2 weeks and 1 month after cataract surgery. The data analysis has been done with diagnostic research by SPSS version 20.0 (SPSS, Inc Chicago, Illinois). Result: 40 study samples of post operative cataract surgery with phacoemulsification technique obtained by female sex with 22 out of samples were (55%), with an average age of patients between of 60-70 years.The accuracy of Shirmer 1 and TBUT test was 0,425 and the sensitivity value was 39.5% and also the specificity value was 100%. Conclusion: Schirmer 1 test and TBUT in patients of post cataract surgery with phacoemulsification techniques are specific (100%) but less sensitive (39,5%)

Abstract

Abstract Background: Convergence Insufficiency (CI) is an inability to perform convergence movements when doing near activity. The prevalence of CI was estimated varying between 0.1%-8%. One of the easiest therapeutic attempts from some research have showed the effectiveness to overcome CI is by pencil push up therapy. This study is to determine the improvement of CI symptoms and signs in patients after performing the pencil push up exercise for 8 weeks. Methods: This clinical trial was conducted on October 2016-March 2017. Nineteen patients who met the inclusion criteria was obtained. The frequency and distribution of data is described in tabular form and the effectiveness of pencil push up therapy is analyzed with Paired T Test and Wilcoxon test. Result: Patients with CI had an average age of approximately 22 years, with the sex ratio of females and males being 3:2. After 8 weeks of pencil push up therapy, there was a significant decrease of Convergence Insufficiency Symptom Survey (CISS) value from 25.2±3.72 to 15.7±5.64 (p=0.000), significantly decreased Near Point Convergence (NPC) value from 13.7±1.88 cm to 9.05±2.99 cm (p=0.000), significantly decreased Red Blue Test (RBT) value from 14.2±1.84 to 9,36±3.22 cm (p=0.000), and significantly decreased Penlight Red/Green Test (PLT) from 16.7±2.69 to 10.7±4.13 cm (p=0.000). A number of Alternate Cover Test (ACT) positives also decreased from 4 to 2 samples but not significant (p=0,500). Symptoms and signs of CI were improved in 14 samples (73.7%). Conclusion: Pencil push up therapy for 8 weeks is effective to improve symptoms and signs of convergence insufficiency.

Abstract

Introduction: Laser-assisted in situ keratomileusis (LASIK) is the most common refractive surgery to treat myopia. One of the most common complain after this procedure is blurry near-vision. This study was aimed to evaluate the role of accommodation amplitude (AA) in myopic patients before and after LASIK and its effect to blurry near-vision in myopic patients after LASIK. Methods: Patients with myopia who underwent LASIK procedure in Sriwijaya Eye Center Hospital from January to February 2018 studied prospectively. AA was assessed before and after LASIK with 1 day, 1 week and 1 month. Myopia degree, intraocular pressure (IOP), age, gender, ablation and corneal profile were also assessed. Results: A total 52 eyes from 32 patients were included. Visual acuity (VA) of all sample were significantly improve in 1 day after LASIK (p 0,001). Mean AA in myopic patients before LASIK 9,25 D and AA 1 day after LASIK were all significantly decrease into 9,00D (p 0,012) which can cause in blurry near- vision after LASIK. In 1 month followed- up, mean AA is significantly improved into 11,00 D (p 0,000) with no complain. Other factor that affect the changing AA were corneal cell density (CD), IOP and AA before LASIK. Conclusions: There is significant AA difference in myopic patients before and after LASIK. Blurry near-vision after LASIK is caused by AA adaptation mechanism after LASIK.

Abstract

Introduction: In general, diabetic retinopathy is classified into early stage, namely non proliferative diabetic retinopathy (NPDR), and advanced stage, proliferative diabetic retinopathy (PDR). Diabetic macular edema is the most common cause of visual impairment in cases of early stage or NPDR. Purpose: To describe Diabetic Macular Edema (DME) with intravitreal injection of anti-VEGF as a treatment. Case presentation: 43 old female, came with chief complaint of blurred vision on her right eye since six months ago. Blurring is felt slowly. Patient has a history of uncontrolled diabetes since 10 years and a history of hypercholesterolemia since 1 year ago. visual acuity of the right eye is 4/60. On posterior segment examination, neovascularization of the papilla was found. Decreased foveal reflex (+), hard exudates (+) within 500 µm from the central macula. Microaneurism (+), dot hemorrhage (+), blot (+) in 4 quadrants, hard exudates (+) in 2 quadrants in her right eye. The patient was planned for intravitreal injection of anti-VEGF on her right eye. Conclusion: Intravitreal injection of anti-VEGF can improve visual acuity and reduce exudate and hemorrhage in retina from ophthalmoscope and fundus photography examination. In addition, the investigation with OCT was found to improve with reduced macular thickness.

Abstract

Introduction: Fungal keratitis is less common than bacterial keratitis, generally representing less than 5%–10% of corneal infections in reported clinical series in the United States. But in developing countries (Ghana, India, China) it accounts for more than 90% of the cases. Corneal trauma by plant or vegetative material is the leading risk factor for fungal keratitis. One or more topical antifungals are usually administered with systemic support of oral antifungals. The most common classes of antifungal used for medical therapy include the polyenes, azoles, and the echinocandins. Case presentation: A 47-year-old male presented with a widening white patch on his left eye since 4 days before coming to our hospital. The left eye was injured by wood flakes 3 weeks ago. He felt a sore, red, watery left eye and blurred vision. He washed his eyes with betel leaf water. His left eye visual acuity was 1/60 and not improved with pinhole. There were blepharospasm, ciliary and conjunctival injection. Corneal examination showed a cloudy cornea, a central corneal defect sized 8x6 mm with an irregular margin, 2/3 to stromal depth, infiltrate, satellite lesion, and positive fluorescein staining at the defect margin. KOH examination demonstrated hyphae. The culture test of left corneal discharge identified a fungi species (Curvularia sp). We prescribed fortified fluconazole eye drops in combination with supportive therapy for 2.5 months. This regimen showed a significant increase in visual acuity to 6/30 with pinhole improvement of 6/21 in the left eye. Discussion: Imidazole acts by inhibiting ergosterol biosynthesis of the fungal cell wall, through action on the cytochrome P450-dependent enzyme. This leads to cell membrane destabilization and leakage. Curvularia sp belongs to the family of dematiaceous fungi. Its clinical manifestations include raised lesions and feathery edges; hypopyon rarely occurs, and the prognosis is quite good. Fluconazole (2 mg/ml) is available for injection and is a well-tolerated eye drop. The intravenous injection can be used as a topical treatment and can be given subconjunctivally at the same concentration. Conclusions: Fortified fluconazole eyedrops (2 mg/ml) can become an alternative and additional therapy for deep fungal keratitis, especially for Curvularia sp.

Abstract

Introduction. Diabetic retinopathy is a progressive microangiopathy characterized by damage and occlusion of small blood vessels. The earliest pathologic changes are thickening of the capillary endothelial basement membrane and a reduction in the number of pericits. Diabetic retinopathy is the main cause of vision loss in type 1 of DM patients and has various risk factors such as chronic hyperglycemia, hypertension, hypercholesterolemia, and elevated HbA1C levels. Methods. This research was conducted using a descriptive observational analytic method with a cross sectional approach at The Eye Polyclinic Dr. Mohammad Hoesin General Hospital Palembang used secondary data on diabetic retinopathy patients. The sample consisted of 64 patients with a total sampling technique, there were 50 patients who met the inclusion criteria. Results. There was a significant relationship between HbA1C levels (p value = 0.050) with a PR value = 1.463 and total cholesterol (p value = 0.038) with a PR value = 1.667 for diabetic retinopathy. Conclusion. HbA1C levels and total cholesterol are significant risk factors for diabetic retinopathy.

Abstract

Introduction: Ocular myasthenia gravis (OMG) is an autoimmune disease which is characterized by weakness of extraocular muscles, levator palpebrae and orbicularis oculi, resulting in ptosis and binocular diplopia. Nearly all patients present with eyelid and extra ocular muscles involvement. Approximately 30% to 80% of patients with OMG experience a conversion to generalized myasthenia gravis (GMG) within 2 years. There are not only have ptosis and diplopia but also limb weakness,bulbar symptoms, or even respiratory failure. Objective: To observe the clinical features of OMG to GMG and risk factors and median time to conversion of OMG to GMG of myasthenia gravis patients in Dr. Mohammad Hoesin General Hospital Palembang. Methods: This study is a cohort retrospective study and the data were collected from the medical records of 91 patients who were registered as myasthenia gravis patients during September 2018 to March 2020. Sosiodemographic and clinical characteristics, including onset of OMG to GMG, history of smoking, presence of thymic abnormalities, and medications received were reviewed retrospectively. Results: A total of 91 OMG patients were observed in this study with 32 (35,2%) patients converted from ocular myastenia gravis to general myastenia gravis. Median conversion time to GMG was 34 months. Risk factor for convertion cases of OMG to MGG was receiving immunosupressive agents (Risk: 14.7, 95% CI 4.83, 44.7), thymus hyperplasia (Risk: 3.36, CI 95% 0.33, 33.6), Female (Risk: 2.41, 95% CI 0.94, 6.17), Smoking (Risk: 1.56, 95% CI 0.31, 7.81). Conclusion: Ptosis was the definitive sign for OMG in this study, with all patients had ptosis, thus it needs the colaboration from neuroophthalmologist and neurologist to diagnose and manage this case. Most of converted case was female and those who receive an immunosupressive agent therapy.

Abstract

Anxiety disorders has been termed a social epidemic especially by people with disabilities, one of them is visual impairment because age related macular degenaration. The present study compared the qualitative dimensions of loneliness of the blind and visually impaired with the generall population not on the frequency or intensity of their loneliness. This study compared the qualitative dimensions of loneliness between blind and visually impaired persons and the general population, rather than the frequency or intensity of their loneliness., no studies have specifically evaluated anxiety disorder in young and middle-aged people with visual impairment (VI). High prevalence of loneliness among the visually impaired elderly persons, that reports a prevalence of 54% in the visually impaired elderly aged ≥55 years. In addition, the risk of loneliness was higher for those who were aged 36 to 50 years, exposed to bullying or physical or sexual abuse, had blindness, other impairments, or were unemployed. The mean score for loneliness was 4.83 (SD 1.82), 4.88 for women and 4.78 for men (p = 0.46). The majority of participants reported missing somebody to be with sometimes (39.0%) or often (21.1%). The prevalence of anxiety among older adults with AMD is also higher than among both general populations of older adults and those with other common age-related medical conditions. The aim of this study to estimate the prevalence of loneliness and associated factors in adults with age related macular degeneration.
