Background.Chlorhexidinegluconate is one of endodontic irrigants potential due to its antibacterial activity. Although it is an effective antibacterial agent, chlorhexidinegluconate cannotdissolve organic substances and necrotic tissue present in the root canal. In addition, same as other chemicals, chlorhexidinegluconate also cause a cytotoxic effect. Tannin extracts of mangosteenpericarp (Garciniamangostana Linn.) demonstrated various biological activities including antibacterial, antioxidant, anticancer and anti-inflammatory. Tannin extracts of mangosteenpericarp can be considered as an alternative endodontic irrigation for dental application.Purpose.The aim of this study was to compare the biocompatibility between 0,78% tannin extracts of mangosteen pericarp and 0,2% chlorhexidinegluconate to BHK-21 fibroblass cell. Method.Tannins obtained from extracts of mangosteenpericarp. Preliminary test was conducted in the biocompatibility of tannin extracts of mangosteenpericarp in various concentrations of the BHK-21 fibroblass. Tannins extracts of mangosteenpericarp was at concentrations of 0,78% less toxic than the other concentrations. Then biocompatibility of 0,78% tannin extracts of mangosteenpericarp compare with 0.2% chlorhexidinegluconate using cytotoxicity test on BHK-21 fibroblass cells with MTT method and observed by ELISA reader. Results.0,78% tannins extracts of mangosteenpericarp showed good biocompatibility with fibroblass BHK-21 than 0,2% chlorhexidinegluconate. There was a significant differences between the results of treatment with 0,78% tannin extracts of mangosteenpericarp and 0.2% chlorhexidinegluconate. Conclusion. 0,78% tannin extracts of mangosteenpericarp has better biocompatibility than 0,2% chlorhexidinegluconate as an endodontic irrigants.

Introduction: Dental implants gained popularity as the treatment to replace grossly decayed teeth. However, with the increasing evidence and the difficulty to manage complications associated with dental implants, clinicians are pushed to opt for a more conservative approach. Case report: A male patient with a chief complaint of frequent food impaction in a severely decayed mandibular molar wished to retain his tooth. Upon thorough examination, the carious lesion extended to furcal area that rendered the distal root unsalvageable. However the mesial root can be retained; thus, hemisection was proposed. Management: Root canal treatment was carried out in the mesial canals. Then, the tooth was split mesio-distally.The distal root was extracted, and the mesial root was retained. Subsequently, the tooth was restored with PFM crown.Conclusion: Hemisection with subsequent prosthetic rehabilitation can be a viable alternative to retain severely decayed mandibular molar.

Background: Preparation, one of the stages in root canal treatment, always producesdebrisas aresultof instrumentation onroot canalwalls. Debrisconsistsoforganicand inorganicmaterials. Irrigation need to be performed in every preparation so that debrisandmicroorganisms can be removed from root canal wallsthrough flushingmechanism. NaOCl 2.5% is the most popular irrigant used in root canal treatment. However, NaOCl2.5% onlyworks onorganic tissue. Mangosteenpeel extractcontainsvariousactive compounds, such as saponin. Saponin acts assurfactantso as tolower thesurface tension and remove debris from the root canalwalls. Purpose: to compare the cleanliness of root canal walls following irrigation with NaOCl 2.5% and mangosteen peel extract. Method: Eighteen mandible premolar extracted for orthodontics necessity were used in this study. The teeth were divided into three groups, in which each group consists of six teeth. All of them were instrumented with ProTaper for Hand Use and irrigated. Group 1 used aquadest as irrigant, group 2 used NaOCl 2,5%, and group 3 used mangosteen peel extract 400 ug/ml. The roots were split longitudinally into halves and in 1/3 of apex. The surface of the canal walls were examined using scanning electron microscope. Photomicrographs were scored by three independent observers and statistically tested. Result: There were significant differences between three groups (p<0.05). Group 3, irrigated with mangosteen peel extract 400 ug/ml, had the smallest median score that was 1. Conclusion: Mangosteen peel extract is moreeffective thanNaOCl 2.5% incleaningroot canalwall from debris.

Introduction: Dental caries is a multifactorial disease caused by the interaction of pathogenic bacteria, especially Streptococcus mutans. Various caries prevention chemically and mechanically can help reduce the number of bacteria. However, this approach does not cover the entire bacterial population with a photodynamic therapy approach with the addition of photosensitizers and low-intensity light radiation with the right wavelength able to eliminate the number of S. mutans bacteria. Purpose:To determinethe duration of effective irradiation time on a 405 nm diode laser with erythrosine photosensitizer in killing S. mutans bacteria.Methods: This research was an experimental laboratory type with 25 S. mutans samples divided into 5 groups treated with erythrosine photosensitizer 0.1 mg / ml and 405 nm diode laser irradiation with 45 seconds, 60 seconds, 75 seconds duration and no erythrosine photosensitizers and without irradiation. The growth of S. mutans bacteria was calculated then the data were analyzed statistically.Results:the research found that S. mutans bacteria decreased gradually in each group; erythrosine without photosensitizer and without irradiation had the highest average growth rate of 71.6 CFU / ml; without photosensitizer with irradiation of 40.6 CFU / ml; irradiation with photosensitizer 45 seconds at 20.6 CFU / ml; irradiation with 60 seconds photosensitizer at 11 CFU / ml; and irradiation with 75 seconds photosensitizer at 0 CFU / ml. In statistical tests, the data are normally distributed and homogeneous. There are significant differences between groups.Conclusion: The effective duration in this study was found at 75 seconds.

Background. Necrosis teeth need a procedure called the root canal treatment to eliminate microorganisms in root canal, so the inflammation process did not continue to the periapical.One of the microbes that exist in thenecrosis teethis Porphyromonas gingivalis, that capable of forming biofilm because it has the ability to communicate with bacteria signaling (quorum sensing). It is necessary to discover another antibiofilm agent as an irrigation material in root canal preparation deriving from nature. Flavonoidmangosteen pericarp extract can disrupt acyl-homoserine lactones (AHLs) thus quorum sensing bacteria will be disrupted and causing permanent attachment and the establishment of colonies on the surface of the teeth any barrier. Purpose. The aim of this study found out the antibiofilmactivity of flavonoid mangosteen pericarp extract against Porphyromonas gingivalis bacteria. Method. This studyhasa laboratory experimental with post-test only group design. Porphyromonas gingivalis ATCC 33277 was diluted according to Mc. Farland standard 106CFU/ml in Tryptic Soy Broth (TSB) medium and inserted into microtitterplate flexible U bottom. Porphyromonas gingivalis was incubatedfor 6x24 hour and checked with a simple staining to see the formation of biofilm. Flavonoid mangosteen pericarp extract was added in various concentrations and OD (optical density) readings done with a wavelength of 595 nm. Result. Flavonoid mangosteen pericarp extractshowed a decrease of OD value from control in all concentrations.Conclusion.Flavonoid mangosteen pericarp extract has antibiofilm activity against Porphyromonas gingivalis bacteria.

Background: Most soft drinks contain citric acid / phosphoric acid with pH <3.5. Strong acids in the oral cavity cause tooth erosion. Tooth erosion leads to demineralization enamel. Remineralization can prevent demineralization process. One of the commercial remineralization agents is Casein Phosphopeptide-amorphous Calcium Phosphate(CPP-ACP) but it has low solubility and less affordable price. Therefore, a natural ingredients such as theobromine from cocoa extract need to be developed as remineralization agent. Objective: To determine enamel hardness differences due to topical application of theobromine gel and CPP-ACP. Method: incisive crowns were cut off (1x1) cm then planted in cylindrical acrylic resin. All samples were immersed with citric acid for 90 minutes then divided into 3 groups. Group I (control) was immersed with aquades (96 min), group II was applied with theobromin gel 200 mg/l (96 min), group III was applied with CPP-ACP (96 min). By using vickers hardness tester, enamel hardness be measured (15 sec) at 3 points. Results: There were significant differences between the control group, CPP-ACP group, and theobromine gel group. Conclusion: Enamel hardnesse with theobromine gel application is higher than CPP-ACP application..

Background. Use of cavity cleanser is important before restoration the teeth to clean debris, residue of prepared dentine, blood, bacteria, collagen denaturized by teeth preparation. Nowadays, the cavity cleanser that people used still having shortcoming, one of which is the lack of ability to clean the root canal of the smear layer. Purpose. The purpose of this study examines the difference of cavity cleaner between saponin of mangoosteen peel (GarciniaMangostana L.) and chlorhexidinegluconate 2%. Method. Eighteen upper first premolar divided into 3 groups, each of them consist three tooth. Forming the preparation tooth cavity then group 1 using aquadest for control group, group 2 using chlorhexidinegluconate 2%, and group 3 using saponin of mangoosteen peel (GarciniaMangostana L.). For rating cleanliness of the tooth cavity using a scale of cleanliness conducted under Scanning Electron Microscope. Result. There was a significant difference (p < 0.05) in One-Way ANOVA parametric test andpos hoc test between chlorhexidinegluconate 2 % and saponin of mangoosteen peel to the cleanliness of the tooth cavity. Conclusion.According to the result of the study, it can be concluded that saponin of mangoosteen peel (GarciniaMangostana L.) less effective for cleaning the tooth cavity than chlorhexidinegluconate 2%.

Background: Pulpitis is an inflammatory pulp that can caused by pulp perforation by mechanical injury. Emergency treatment of pulpitis is using Eugenol. High concentrations of Eugenol have a cytotoxic effect that causes local necrosis and inhibits the healing process. Because of negative effects from eugenol, then it’s necessary to consider a new ingredient with minimal side effects, and it’s epigallocatechin-3-gallate (EGCG) in green tea. As a polyphenol, it has good antioxidant effect and plays a role in shortening the duration of inflammation by radical scavenging against Nitric Oxide so that NO levels rapidly decrease which causes migration of neutrophil cells to the area of injury will decrease and the inflammatory process faster so that the healing process become faster. Lymphocyte is plays a role in tissue repair. Purpose: to explain the effect of EGCG hydrogel on the number of lymphocyte cells in pulp inflammation with mechanical injury. Methods: The study used 24 Wistar rats divided into four groups, namely control (C), and 3 groups of treatment (T1, T2, T3). Each group consisted of 6 rats prepared, then EGCG hydrogel 60 ppm was named T1, EGCG hydrogel 90 ppm was named T2, EGCG hydrogel 120 ppm was named P3. On the 3rd day, Wistar rats were decapitated for HPA preparations. Results: This study showed a significant difference in each group (p < 0.05) using One-Way Anova analysis. Conclusion: EGCG hydrogel 90 ppm is effective in increasing the number of lymphocyte cells in inflammation of the pulp with mechanical injury.

Background: Pulpitis is an inflammatory pulp that can occur due to mechanical trauma that causes perforation of the pulp. Treatment of pulpitis Emergency frequently using Eugenol. High concentrations of Eugenol have a cytotoxic effect that causes local necrosis and inhibits the recovery process. An alternative consideration with Epigallocatechin 3-gallate (EGCG) which has good antioxidant properties and increases the complexity of inflammation by inhibiting the production of inducible nitric oxide synthase (iNOS) and nitric oxide so that levels migration of inflammatory cells to the area of injury will decrease and inflammation will occur shorter then initiates the proliferation so the recovery process and tissue repair will be more rapidly occurring. One of the cells that support tissue repair is macrophages. Purpose: this study aims to explain the effect of EGCG on the number of macrophage cells in pulp inflammation with mechanical injury. Methods: The study used 24 Wistar rats teeth divided into four groups, namely control (C), and 3 treatment groups T1, T2, and T3. Each group consisted of 6 rats prepared, then T1 was named EGCG hydrogel 60 ppm, T2 was given EGCG hydrogel 90 ppm, while T3 was given EGCG hydrogel 120 ppm. On the 3rd day, Wistar rats were decapitated to continue making HPA preparations. Results:This study showed a significant difference in each group (p< 0.05) using One-Way Anova analysis. Conclusion: EGCG hydrogel 90 ppm is effectivein increasing the number of macrophage cells.

Background: The vitality of the pulp must be protected because the pulp contains fibers, cells, and various structures. Pulp vitality can be maintained by pulp capping to stimulate the formation of reparative dentin. The material commonly used for pulp capping is calcium hydroxide. Calcium hydroxide has the disadvantage of causing inflammation and necrosis of the pulp surface. To compensate that, propolis is added because it has the ability as an anti-inflammatory. Purpose: To analyze the increase in the number of fibroblast cells and angiogenesis in the pulp after the application of combination of Calcium hydroxide-propolis compared to the application of Calcium hydroxide.. Methods: This study used 30 samples of wistar rats which were preparated until perforation and then divided into 3 groups. The first group continued with filling with cention, the second group continued with application of calcium hydroxide and filled with cention, and the third group continued with the application of combination of calcium hydroxide and propolis and then being filledwith cention. fibroblast cells and angiogenesis is calculated histopathologically by using compound light microscopeon the 7th day with 400x magnification.Results:Based on the results of the study found a significant difference between the calcium hydroxide group and the calcium hydroxide-propolis group. Increased numbers of fibroblasts and angiogenesis indicate a faster inflammatory process that affects the acceleration of wound healingConclusion: More number of fibroblasts and angiognesis cells found in the pulp after the application of a combination of Calcium hydroxide-propolis compared to the application of Calcium hydroxide
