Abstract

Background. The main etiology of endodontic treatment failure is caused by bacteries that stay in the root canal. E.faecalis is a bactery that is found as an etiology of endodontic treatment failure. Cell wall of this bacteria is containing Lipoteichoic acid (LTA). LTA can penetrate into the periradicular tissue, act as endotoxin in host and cause periradicular inflammation then lead to bone destruction. Bone destruction occurs due to the inflammation process that is mediated by immune system. The important cell in the process of bone destruction is osteoclast. Bone destruction is marked by the form of osteoclast that is called osteoclastogenesis. NFATc-1 and osteocalcin play important things in osteoclastogenesis. Purpose. The aim of this study is to know about the expression of NFATc-1 and osteocalcin during the periapical bone destruction due to induction of E.faecalis. Method. This study used laboratory experimental with the post test only control group design. A total of 54 male rats were randomly divided into 2 main groups, which each main group had 3 subgroups. Group A (control) : every tooth was induced only by sterile BHIb. Group A had 3 subgroups (A Control day 3, 10, and 21), group B : every tooth was induced by 10 μl BHI-b E.faecalis ATCC212(106 CFU), it was contained 3 sub groups (B day 3,10, and 21). The animals were sacrificed based on their days scheduled group and prepared for histological examination of periapical bone, then we did the immunohistochemistry followed by calculation on the light microscope. Result. The analysis revealed that the expression of NFATc-1 and osteoclast increased significantly in group B when E.faecalis was induced. Conclusion. From this study we know that the expression of NFATc-1 and osteocalcin are increasing during the periapical bone destruction that induced by E.faecalis.

Abstract

Introduction: the esthetic factor should be considered for every patient who seeks a comprehensive dental treatment, especially for women. The anterior teeth deformities will diminish the levels of confidence and decrease self-esteem of the patients. The tooth development disturbances, such as enamel hypoplasia, may present an esthetic problem. Enamel hypoplasia is an alteration of organic matrix formation which characterized by the defect or porosity on the enamel surface. It can be restored with a variety of restorative treatment options including indirect porcelain veneers. Case: a 30 years old female patient was referred with chief complaint of anterior teeth discoloration. Clinical examination on the maxillary left central and lateral incisors revealed the defects on nearly half of the labial surface. Case management: the incisal lapping technique were chosen to prepared teeth #21 and #22 for indirect porcelain veneer fabrications. Conclusion:  indirect porcelain veneers established an excellent esthetic result and restored the size, shape and color of anterior teeth with long term stability.

Abstract

Background. Root canal treatment is a main role in decreasing infection from root canal and pulp. The main cause of periapical damage mostly are bacteries. E.faecalis is a bactery that is found as an etiology of endodontic treatment failure. Cell wall of this bacteria is containing Lipoteichoic acid (LTA). IL-1 is the proinflammation cytokine that is the key of host response bacteria invation and tissue damage. Also IL-1 could cause some indirectly tissue damage through the activation of MMPS. MMPs to stop the collagen formation. Purpose. The aim of this study is to know about the expression of IL- 1 during the periapical tissue damage due to induction of E.faecalis. Method. Abstract

Background: Streptococcus sanguinis is a bacterium that can cause failures in root canal treatments due its ability to penetrate the dentinal tubules to a depth of 400 μm in just two weeks. Irrigation material is needed to stop the growth of this bacteria so that no bacteria can pass through by using chemicals, irrigation materials that are widely used such as Chlorhexidine 0,2% but still lack because it cannot be used as a single irrigation solution because its effectiveness will be reduced if it is related to protein and organic dentine matrix and low Ph saliva.Therefore, research is needed to find natural ingredient that can be an alternative such as Cocoa peel extract was chosen because it contains active compounds, in the form of saponins, tanins, alkaloids, flavonoids, and terpenoids that have been known to have antibacterial properties a concentration of 6,25% is used in accordance with the MKC of Streptococcus sanguinis. Aim: To compare the antibacterial power between cocoa peel extract (Theobroma cacao L.) 6,25% and Chlorhexidine 0,2% against Streptococcus sanguinis Method: This research is an experimental laboratory with a post-test only control group design. The diffusion method was used to determine the susceptibility of bacteria isolated from the material by planting the culture of Streptococcus sanguinis on the agar medium by swabbing the nutrient media which has been divided into 3 parts consisting of negative control, cocoa peel extract and Chlorhexidine, then each nurient media so that it is given a paper disk and 0.01ml liquid on each section. The diameter of the inhibition zone was observed after 2x24 hours using the calipers. Results: The average inhibitory zone that was formed using cocoa peel extract was 20,40 mm against Streptococcus sanguinis and Chlorhexidine was 18,36 against Streptococcus sanguinis. Conclusion: Cocoa peel extract (Theobroma cacao L.) 6,25% had higher anti-bacterial power compared to 0,2% Chlorhexidine against the growth of Streptococcus sanguinis.

Abstract

Background:Enterococcus faecalis (E. faecalis) is a microorganism that is commonly found in endodontic failure treatment, this due to several characteristics of E.faecalis which has the capabillity to living in environments with high salt levels, high temperature, and pH broad spectrum. Bacteria in biofilms form is one of the adaptive process that allows bacteria to survive in an environment with low nutrients in the root canals. Bacteria in biofilms form have different characteristics from planktonic form, resistance to phagocytic cells and drugs, which can effect to persistent infection. Mangosteen (Garcinia mangostana) has many benefits, especially on the pericarp of the fruit contains alkaloids, tannins, phenolics, flavonoids, and triterpenoids. Flavonoids are the largest group of phenolic compounds that have a nature effectively inhibit the growth of viruses, bacteria, and fungi. Purpose:Purpose of this study wasto find out the role of the antibiofilm of the flavonoid in garcinia mangostana pericarp against E. faecalis bacterial biofilm. Methods:Laboratory experimental in-vitro with post test only group design. The method used is microtitter plate biofilm assay and continued with the readings use Elisa reader at a wavelength of 595 nm. Results:Flavonoids mangosteen pericarp effective as antibiofilm E.faecalis bacteria at a concentration of 12.5%. Conclusion:The study showed that flavonoids from mangosteen pericarp has antibiofilm activity against E. faecalis bacterial biofilm.

Abstract

Backround: Smear layer formed during cavity preparation interfere with the adhesion between restoration and tooth structure and is considered a barrier that would degrade the quality of adhesion. Smear layer does not have a stable substrate for adhesion, gradually layer dissolves in the restorative material and cause microleakage, penetration of bacteria and inflammation of the pulp. Adhesion to dentin is more difficult to achieve than enamel, therefore the cleaning procedure on dentin require special treatment. Conditioner form a weak acid is used to remove the smear layer and surface contamination on the email or dentin which can reduce the adhesion of the material and the tooth surface. Dentin conditioner is an acid material containing 10% polyacrylic acid conditioner while Cavity conditioner an acid material containing 20% polyacrylic acid and 3% aluminum chloride. The higher the concentration, the more smear layer is dissolved in order to obtain adhesion of glass ionomer cements better because it is not blocked by the smear layer. Purpose: The purpose of this laboratory research is to study the shear bond strength of glass ionomer cement in dentin after application dentin conditioner and cavity conditioner .Material and Method: Twenty seven bovine cow's teeth were divided into three groups. Each group consisted of nine samples. Group 1 was control (without conditioner). Group 2 was treated with the Dentin conditioner. Group 3 was treated with the Cavity conditioner. Result: The average shear bond strength in group 1 is 3.31 Mpa, group 2 is 7.74 MPa and group 3 is 9.92 Mpa. Conclusion: There is a significant difference between third group and the shear bond strength of glass ionomer cement on dentin with application of the Cavity conditioner is higher than with application of the Dentin conditioner and without application conditioner

Abstract

Background: Pulpitis is an inflammatory pulp that can occur due to mechanical trauma that causes perforation of the pulp. Treatment of pulpitis Emergency frequently using Eugenol. High concentrations of Eugenol have a cytotoxic effect that causes local necrosis and inhibits the recovery process. An alternative consideration with Epigallocatechin 3-gallate (EGCG) which has good antioxidant properties and increases the complexity of inflammation by inhibiting the production of inducible nitric oxide synthase (iNOS) and nitric oxide so that levels migration of inflammatory cells to the area of injury will decrease and inflammation will occur shorter then initiates the proliferation so the recovery process and tissue repair will be more rapidly occurring. One of the cells that support tissue repair is macrophages. Purpose: this study aims to explain the effect of EGCG on the number of macrophage cells in pulp inflammation with mechanical injury. Methods: The study used 24 Wistar rats teeth divided into four groups, namely control (C), and 3 treatment groups T1, T2, and T3. Each group consisted of 6 rats prepared, then T1 was named EGCG hydrogel 60 ppm, T2 was given EGCG hydrogel 90 ppm, while T3 was given EGCG hydrogel 120 ppm. On the 3rd day, Wistar rats were decapitated to continue making HPA preparations. Results:This study showed a significant difference in each group (p< 0.05) using One-Way Anova analysis. Conclusion: EGCG hydrogel 90 ppm  is effectivein increasing the number of macrophage cells.

Abstract

Background: The presence of persistent infections in the root canals by microorganisms causes root canal failure. The most commonly found bacteria that cause persistent infection is Enterococcus faecalis. PDI / photodynamic inactivation is an in vitro approach to inactivation of microorganisms. The combination of light and photosensitivity of chlorophyll in PDI will cause photoinactivation in bacteria. Long radiation of PDT can affect the production of singlet oxygen and ROS (Reactive Oxygen Species) to kill Enterococcus faecalis bacteria. Objective: To prove the effect of 405 nm laser diode  with and without chlorophyll photosensitizer and the irradiation effect of 405 nm laser diode on the number of CFU of Enterococcus faecalis bacteria. Method: This study used the Enterococcus faecalis bacteria cultur  which was divided into 5 groups. Group I as control group, Group II irradiation 30 ', III chlorophyll + irradiation 30', IV irradiation 60 ', V chlorophyll + irradiation 60'. After incubation, the bacteria count was calculated with Quebec Colony Counter and analyzed by Shapiro-Wilk test, Levene test and Anova test. Results: There were significant differences (p <0.05) between the number of colonies of Enterococcus faecalis bacteria in each treatment group. Longer duration of PDT exposure (Group II and IV) with chlorophyll showed less number of Enterococcus faecalis bacteria. Conclusion: The longer the PDT irradiation, the less number of Enterococcus faecalis bacteria. The 60-second radiation with chlorophyll showed the least amount of Enterococcus faecalis bacteria.

Abstract

Background: In medical practice, clinicians come across a lot of obstacle during a treatment , that can lead to a failure, like other dental treatments, endodontics too can fail. Inappropriate mechanical debridement, persistence of bacteria in the canals and apex, poor obturation quality, over and under extension of the root canal filling, and coronal leakage are some of the commonly attributable causes of failure. Despite the high success rate of endodontic treatment, failures do occur in a large number of cases and most of the times can be attributed to the already stated causes. Purpose: Endodontic retreatment of a failure is required by the increased desire to preserve the tooth on the dental arch, preventing the need for dental extraction that may have adverse consequences in terms of functional and psychological effect on patients. Case: This article presents a case report about dental retreatment with 2 common failure, that was under filling obturation and non-fit post. Case management: The failure was corrected with endodontic retreatment, and finally restored with fiber post and porcelain fused to metal crown. Conclusion: Endodontic retreatment was done successfully on underfilling obturation and non-fit post. The patient was satisfied with the result.

Abstract

Background: Preparation is one of the stages in endodontic treatment that always produce debris as a result of intrumentation on root canal walls. Debris consist of organic and inorganic materials. Debris will impede the penetration of disinfection materials and reduce attachment of the root canal filling material, therefore needed root canal irrigant which can remove the debris. Wuluh starfruit leaf extract (Averrhoa bilimbi Linn) contain various active compound, such as saponin. Saponin act as “surfactants” which can remove the debris. Purpose: This study was to determine the cleaning effectiveness concentration of wuluh starfruit leaf extract which can clean the root canal walls from debris. Method: Twenty five first premolar human teeth with straight single roots were randomized into five groups in which each group consist of five teeth. Those teeth instrumented with K-file and irrigated as follows: Group 1 (control) used aquadest as irrigant, group 2 used extract 5%, group 3 used extract 10%, group 4 used extract 15%, and group 5 used extract 20%. Then, those teeth were split horizontally and longitudinally in 1/3 apex. After that, the apical third of root canal walls were observed by using scanning electron microscope (SEM). Result: There were significant differences between each group (p<0,05). Group 4 and 5 had smallest median score that was 1. Saponin in wuluh star fruit leaf extract 15% has been reach critical micelle concentration (CMC). Conclusion: Wuluh starfruit leaf extract 15% effective for cleaning root canal walls from debris.
