Clinical and Research Journal in Internal Medicine is the official open access journal of Internal Medicine Research Center, Department of Internal Medicine, Faculty of Medicine, Universitas Brawijaya, Malang, Indonesia. It publishes articles two times per year. It is a peer reviewed publication of Indonesian Internal Medicine journals and accepting articles for publication from around the world. CRJIM only publishes articles in the English version. The objective of this journal is to publish the selected clinical and basic research relevant to Internal Medicine. Abstract

Background: Peritoneal dialysis (PD) is one of the modalities of renal replacement therapy (RRT) in end-stage renal disease (ESRD). One of the complications that are often found in the use of PD is peritonitis. Presence of the culture from the peritoneal fluid was known to be associated with the incidence of peritonitis relapse among ESRD patients with PD. Aim: Comparing the incidence of relapse in patients with continuous ambulatory peritoneal dialysis (CAPD) with culture-negative and culture-positive in RSUD Dr.  Saiful Anwar Malang. Methods: This study measured the incidence of relapse after administration of CAPD peritonitis therapy. Descriptive data are presented in percentages, and numerical data is presented in Mean ± SD. The analysis test was carried out using the Mann-Whitney with p<0.05, significant. Results: According to the culture results, it was found that 25.4% (18 of 71 PD peritonitis cultures) were culture-negative in patients with CAPD peritonitis. This result is slightly higher than the ISPD recommendation, with 20% culture-negative results. There were 3 out of 53 relapses in culture-positive subjects and 1 in 18 relapses in culture-negative subjects. There was no significant difference in the two groups, with an OR of 0.721 in the culture-positive group lead the culture-positive tended to experience no relapse by 0.721 times less than the patient with culture-negative. Conclusion:  In peritoneal dialysis peritonitis, there was no significant difference in the incidence of relapse in the positive and culture-negatives.

Abstract

Multiple myelomas (MM) is the proliferation of malignant plasma cells in the bone marrow, characterized by the production of monoclonal immunoglobulins that are secreted in the blood and urine. However, in 1 - 5% of cases, the protein is undetectable and defined as non-secretory type of myeloma. A 36-year-old woman presented with complaints of weakness in both legs so she could not walk. She also felt pain in the head, back, and thighs since one year before. Physical examination showed anemic conjunctiva, gibus as high as T4, hypoesthesia as high as T6 segment. She had anemia (Hb 10.2 g/dL), decreased kidney function (eGFR: 30 mL/min/1.73m2), and multiple lytic lesions in the calvaria, ribs, superior-inferior pubic ramus, and left humeral os in the 1/3 proximal to 1/3 distal. Protein electrophoresis and Bence Jonce protein were negative. There was no hypercalcemia, and bone marrow aspiration showed myelodysplasia syndrome. A thoracolumbar MRI showed an extradural solid mass and the histopathology examination result was plasmacytoma. Non-secretory MM was defined into two groups, non-producers and multiple non-secretory myeloma patients who produce tumor proteins but cannot be excreted. This patients was categorized as non-producers non-secretory MM because she did not show a protein that can be measured in blood nor urine, but has significant plasma cells in tissue biopsy. The absence of paraprotein in the blood does not rule out multiple myeloma. Suspicion of multiple myeloma needs to be increased in patients who have symptoms of CRAB (calcium, renal impairment, anemia, bone lytic).

Abstract

Hyperthyroid can cause thyroid crisis that a rare endocrinal emergency. Jaundice in thyroid heart disease and hyperthyroid patient was very rare. This case we present a patient with thyroid heart disease presenting thyroid crisis and jaundice. A 49-year-old with complaints of shortness of breath at night accompanied by a feeling of weakness. The patient felt tremor in her upper extremity, lost weight, difficulty sleeping and easily tired in the last 2 months. She said that her eyer become yellow in color 10 days ago. On admission the vital signs are blood pressure 130/80 mm Hg, pulse 120x/min, respiration rate 24x/min, O2 saturation 80% on room air, temperature 36.8°C. Physical examination showed icteric, jugular vein pressure was +3 cm and from palpation examination found nodule ±4cm in diameter, well-defined, chewy consistency. Extremities showed tremor and edema. Electrocardiography showed atrial tachycardia and chest x-ray imaging showed cardiomegaly and minimal pleural effusion. Thyroid hormone evaluation showed increase Free T4 and decrease TSH level. Liver function test showed increase bilirubin level. Patient received Propylthiouracil 4x300 mg, Propranolol 4x20mg, normal saline 0.9% Infusion 1000ml/24 hours, Pantoprazole Injection 2x40 mg, Furosemide 10 mg/hour, Spironolactone 1x100mg, Albuforce 2x1 tab, Injection of Ceftriaxone 2x1 gr and referred to RS Soetomo, Surabaya for further treatment Percutaneous Transhepatic Biliary Drainage.

Abstract

The thyroid storm is an emergency characterized by multiple organ failure due to severe thyrotoxicosis that is often triggered by other diseases. The diagnosis of thyroid storm is made clinically in a severe thyrotoxic patient with evidence of systemic decompensation. The Burch–Wartofsky Point Scale (BWPS) is a score that is widely used to diagnose thyroid storm, introduced by Burch and Wartofsky in 1993. In Burch–Wartofsky Point Scale, the thyroid storm diagnosis is made if there is clinically severe thyrotoxicosis with a score of 45 in a patient that has a previous history of elevated free fT3  (triiodothyronine) or fT4 (free thyroxine). But in a condition it cannot be distinguished whether a condition is caused by the effects of thyrotoxicosis or due to other diseases, then these symptoms are considered as a result of the effects of thyrotoxicosis. In the Burch–Wartofsky Point Scale, there is a pitfall. If we are accustomed to assuming a worsening of the condition in thyroid patients as a result of the effects of thyrotoxicosis, without trying to exclude the condition due to other conditions can lead to a misdiagnosis of thyroid storm. This article aims to remind us to be careful in applying diagnostic tools in diagnosing a thyroid storm.

Abstract

Background: Obesity is a prominent avoidable cause of mortality and a growing health concern in both adults and children throughout the world. The study's goal is to measure leucocyte telomere length and its relationship with leptin throughout the BMI spectrum. Material & Method: This cross sectional study was conducted among patients attending tertiary care hospital fulfilling the inclusion criteria. The serum taken from these samples was numbered and sent to the lab in an ice box for Leptin and Telomere length analysis. Telomere length was measured in leukocytes using the qPCR method. Result: Total of 360 patients with mean age of the patients 47.84±15.84yrs, equally distributed in age group of 25-39yrs, 40-54yrs and >=55yrs of age. A significant negative strength of association of BMI with telomere length (r=-0.21, p<0.05) and a significant association of waist to hip ratio with telomere length (r=0.24, p<0.05). No significant relation between telomere length with the serum level of Leptin (r=0.09, p=0.386). Conclusion: The mean Telomere length correlated negatively with BMI and age. In the current investigation, leptin had no significant connection with telomere length.

Abstract

Background: There is age related increase in the incidence of ovarian cancer. Most Indian population-based cancer registries have reported a gradual rise in the ovarian cancer incidence over the years. Since ovary is composed of totipotent gametocytes and multipotent mesenchymal tissues, almost any type of tumor can arise from these tissues. Therefore, their proper recognition and classification is important for appropriate therapy and better prognosis.Aim: to look at the histopathological profile of various ovarian lesions in the local female subjects from central India.Methods: A prospective observational study was carried out on the surgically resected ovarian samples that were referred to the Pathology department over two and half year. A total of 100 ovarian cases were included. Histopathological examination of the specimen was done using standard protocols.Results: On gross examination, majority of non-neoplastic specimen showed unremarkable findings. Microscopic examination of non-neoplastic cases revealed that most of the cases were of follicular cyst followed by corpus luteum cyst. On the contrary, majority of neoplastic specimen showed cystic external surface on gross examination. Surface epithelial tumor was the most common histologic type amongst neoplastic cases. Most common histologic subtype amongst the surface epithelial tumors was found to be benign simple serous cystadenoma. Conclusion: In women from central India, commonest non-neoplastic lesion of ovary is follicular cyst whereas most common neoplastic lesion is surface epithelial tumor (benign simple serous cystadenoma).

Abstract

Background: Hepatitis B is the most common chronic viral infection and a significant contributor to morbidity and death globally. Based on the mother's hepatitis B e antigen (HBeAg) status, the probability of perinatal HBV infection in children delivered to mothers with HBV ranges from 10% to 85%.Aim: to determine the prevalence of hepatitis B virus infection among women of childbearing age in the eastern region of Algeria and investigate risk factors for infection to recommend ways to reduce the disease's impact on neonatal morbidity and mortality.Methods: We conducted a cross-sectional study in Sétif, Algeria, from 2005 to 2007 to assess the prevalence of Hepatitis B Surface Antigen among women of childbearing age. This study is the first and only one in Algeria. Data on risk factors, obstetrics, and sociodemographic were gathered using structured questionnaire; they were subsequently tested using an enzymelinked immunosorbent assay for HBsAg. The data collected were entered and processed using Epi info 3.3.2 software. Infection prevalence, sociodemographic, clinical, obstetric and risk factors variable frequency distributions were calculated. The student's t-test and Fisher's exact test were applied, at a significance level of 5%.Results: There are834 women of childbearing age's medical records were examined for this study. 1% of HBsAg test findings were positive. Positive HBsAg didn't significantly correlate with any other variables, including age, place of residence, municipality, marital status, occupation, parity, current pregnancy, reason for current consultation, transmission risk factors (blood transfusion, recent piercing, dental care, shared personal hygiene equipment, injection with multiple use equipment, tattoo, scarification, partner characteristics). However, history of jaundice has a significant protective effect against HBsAg positive.Conclusion: Although our results classify the two municipalities studied as low prevalence areas (< 2%). Prenatal HBsAg screening is strongly advised.

Abstract

Tuberculosis is a high-incidence public health problem in Peru. In our context, Tuberculosis is seen commonly as a pulmonary infection. Extrapulmonary tuberculosis as gastric is rare, both as a primary or secondary infection, with just a few case reports published about these kinds of presentations. This case report presents a 44-year-old female patient with weight loss, dyspepsia, and abdominal pain. Upper endoscopy reveals antral polypoid masses and a gastric ulcer. Abdominal CT shows lymph nodes and stomach wall thickening. Biopsies results were negative for malignancy and positive for granulomas. Sputum acid-fast bacilli smear on microscopic examination was positive. Coexistent pulmonary, gastric tuberculosis, and human immunodeficiency virus infection mimicking gastric cancer were confirmed. We will present this case in detail below.

Abstract

Alzheimer’s disease is a common neurodegenerative disease that affect elderly and it became a serious global burden for years. Despite many studies have been performed, an effective cure for Alzheimer’s was still not be found. Therefore, it is necessary to not only focus on curative methods but also preventive methods with modifiable risk factors and underlying disease, for example lifestyle and metabolic disease, as potential approach in Alzheimer’s disease prevention in the future. The excessive dietary sugar intake has been suggested as a risk factor of many diseases such as metabolic disease and cognitive impairment disease. It may promote cognitive declines through some pathways such as induce alteration of gut microbiomes, metabolism and immune system. These abnormal processes may evoke amyloid β plaque formation and neurofibrillary tangles in brain cells as the effect of chronic inflammation. Chronic inflammation has been widely studied as the hallmark of Alzheimer’s disease pathology. Therefore, the correlation between dietary sugar and impairment cognitive, specifically Alzheimer’s disease will be summarized.

Abstract

Background: Some personal and health-related factors may influence hypertensive patients' level of self-management. Aim: to determine clinical factors acting as barriers to the eleven (11) healthy lifestyles (self-management) practices amongst hypertensives in communities of Idoma tribe, Benue state Nigeria. Methods: A multi-stage, multi-communities, cross-sectional simple random sampling method for quantitative and qualitative data using Self-reported/administered questionnaire and Semi- structured focus group discussion/interviews was used. Data analyzed using descriptive statistics-frequency and percentage presented in tables, while thematic analysis was used for qualitative information. Results: Clinical factors barrier to medication adherent are; not having any symptom 85.1%, fear of side effect of drugs 84.4%, physical exercise barriers are Physician did not prescribe physical exercise 86.1%, Not have any symptoms 85.7%; Respondent stated that feel better 84.9% and when you do not have any symptom 79.9%, are barriers to self- BP monitoring clinically. Feel better 88%, poor commitment between patients and health care personnel 76% are barriers to DASH. 86% and 74.2%, feel better, being overweight were weight management barriers respectively. Moderation in Alcohol clinical factor barriers are; when you feel well 75.7% and feel better 72.6%. Barrier factor to non-smoking adherence is feel better 58.4%. Stress control clinical barriers are; feel worse 73.5% and multiple medical treatment regimen 71.8% amongst others. Follow prescribed treatment plan; Feel better 84%, not having symptom 81% amongst others were clinical barriers. Conclusion: The result calls for intervention to improve the population clinical factor barriers to the eleven healthy lifestyles variables.
