Abstract

Background: Genu osteoarthritis (OA) is the most common arthritis disease caused by a degenerative joint process that causes knee pain and functional disorders. One of the recommended therapies for the treatment of OA genu from several studies is hydrotherapy exercises. Hydrotherapy exercises can reduce pain and improve the patient's quality of life. This study aims to determine the effect of hydrotherapy exercises on pain intensity and functional ability in OA genu patients. Methods: This study is a quasi-experimental study with a one-group pretest-posttest design conducted by the Medical Rehabilitation Installation of dr. Mohammad Hoesin Palembang during the month of October-November 2019. Primary data was collected using interviews to assess pain intensity based on the Numerical Paon Rating Scale (NPRS) score and functional ability based on the WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) questionnaire before and after training. hydrotherapy for 4 weeks for one therapy a week. The data was carried out by the Shapiro-Wilk normality test, then analyzed using the paired T-test or Wilcoxon test using the SPSS tool. Results: In as many as 31 study subjects, the results showed that there was a significant effect before and after hydrotherapy exercises on reducing pain intensity (p = 0.000) and improving functional ability (p = 0.000) in patients with OA genu for 4 weeks. Conclusion: There is an effect of hydrotherapy exercise on pain intensity and functional ability in genu osteoarthritis (OA) patients.

Abstract

Osteoarthritis (OA) is the most common disease in joints that affects people in their middle until late years. In Indonesia the prevalence of OA is relatively high and disturbs their daily activity. Diclofenac is one of the drug of choice in treating OA. To avoid multiple side effects from Diclofenac use, the usage must be in accordance to rationality indicators which are correct dose, correct frequency, and correct length of use. This study is aimed to know the rationality of Diclofenac use in Osteoarthritis outpatient cases at RSUP Mohammad Hoesin Palembang. This study is a descriptive observational with a cross-sectional approach to know the rationality of Diclofenac use in outpatient cases of osteoarthritis at RSUP Mohammad Hoesin Palembang. Samples were medical records of OA patients in outpatient setting from January to March 2018 which fulfilled the inclusion and exclusion criteria. Sampling technique used was total sampling. The amount of samples fulfilling the inclusion criteria were 201 patients, with the most were aged 46-65 years (60.2%), female (55.7%), and has a history of comorbidity which includes low back pain (22.8%). The result of this study shows pattern of Diclofenac use with dosage of 2 x 25mg (73.6%), length of use about <7 days (57.2%). In combination with other drugs there were no interaction to be found (84.4%), or synergistic interaction (8.5%) and antagonistic interaction (7.1%). The use of diclofenac in osteoarthritis cases at outpatient setting in RSUP Dr Mohammad Hoesin Palembang is rational and needs to be maintained.

Abstract

Scurvy is a rare case found in many countries, but it is often found in refugee areas, especially in Africa. Vitamin C deficiency varies based on season and occurs more in men with age. The purpose of this research is to avoid misdiagnosis so that it can be a reference in the field of medicine to diagnose and provide management on scurvy. This prospective research was conducted with one sample of research. Observation had been carried out for two months, starting at one month after the patient was suspected with the diagnosis of scurvy. Computer Tomography (CT) Scan lumbosacral to pelvic results were normal. Laboratory test of HB result was 9.5 g/dl, in which MCV was 63.2 fL, MCH was 20.3 pg, and MCHC was 32.1 g/dl. Scurvy treatment in the first visit was 3x1 tablets vitamin C, 3x5 ml ibuprofen Syr, and physiotherapy. The results were that the patient still suffered swollen and bleeding gums, but the pain no longer existed, pale, behavioural disorders, unable to walk, and pain in both knees. After the second visit, the child got therapy of 4x50 mg vitamin C, 1x1 tablets vitamin B12, 1x150 IU vitamin E, 1x1 tablets cavit D3, and physiotherapy. After two weeks of treatment, there were no complaints of swelling, painful or bleeding gums. The child could straighten her legs, but she was still unable to walk due to the trauma of feeling great pain while walking. Special attention is required to diagnose appropriately so the doctor can minimize and prevent complications.

Abstract

Prostate cancer is the most common cancer in men worldwide and ranks second in the Laboratory of Anatomical Pathology, Dr. Mohammad Hoesin with the highest incidence, especially at the age of more than 60 years. Factors that play a role as a prognostic factor and therapy in prostate carcinoma, including VEGF. The role of VEGF expression in prostate carcinoma as a prognostic and histopathological factor which is an important predictor for the progression of prostate carcinoma. This study aims to determine the relationship between VEGF expression and Gleason score in prostate carcinoma. This study was a cross-sectional observational study. Thirty samples diagnosed with prostate denocarcinoma were derived from the results of the tranurethral resection of the prostate (TRUP) and prostatectomy. Samples were taken from the archives in the Anatomic Pathology section of Dr. Mohammad Hoesin Palembang (period January 1, 2011 to December 31, 2013. Then the sample was stained with VEGF antibody, identified and analyzed the VEGF relationship with the Gleason.z score. The positivity of VEGF expression in prostate adenocarcinoma tended to be more prevalent in the Gleason score group ≥ 7 (43.3%) than in the Gleason score group <(10%). There was no significant relationship between VEGF expression and high Gleason score (p> 0.05). There was no significant relationship between VEGF expression and Gleason score in prostate adenocarcinoma

Abstract

Diabetes mellitus is a condition in which blood glucose levels increase in a person's body which can result in chronic disorder. The effect of diabetes on a male reproductive system can affect sperm quality and disrupt the spermatogenesis process. Moringa oleifera leaves are rich in antioxidants such as flavonoids, vitamin A, vitamin E, and vitamin C, and contain selenium which helps lower bloodssugar levels. This study was conductedtto determine the effect of administering Moringa oleifera leaf extract on the quality andiquantity ofssperms in Wistar rats with diabetes mellitus. This research is an experimental studyiusing pre-andipost-randomized,controlled group design. This study examinediblood sugarllevels, density, motility, andiviability ofssperm cells. This experimental test was dividediinto 4 groups: one as a control group andtthree groups as treatment groups induced with alloxan and given ethanol extract of Moringa leaves with doses of 250, 500, and,750mg/kgBB. The results showed that the ethanol extract of Moringa leaves showedssignificant improvements in various doses (P-value ≤ 0.05).

Abstract

Tuberculosis is an infectious disease caused by bacteria in the form of rods (bacilli) known as Mycobacterium tuberculosis. This disease is included as a chronic infectious disease. The causes of the increase in TB cases each year are still various. One of them is smoking behavior. A person who smokes will have twice the risk of contracting TB. This study aims to determine the relationship between smoking behavior and the incidence of pulmonary tuberculosis in the work area of the Medan Sunggal Puskesmas in 2019. This study is an analytical study with a cross sectional design. The population of this study were patients with pulmonary TB in the work area of the Medan Sunggal Public Health Center in 2019. The sample size used was 104 people who had met the inclusion and exclusion criteria. In the second study, the data collection tool was through the Puskesmas medical records. Data analysis was performed using the statistical test is chi square. the result showed that there was a relationship between smoking behavior and the incidence of pulmonary tuberculosis in Medan Sunggal Public Health Center with an OR (95% CI) value of 105,125 (13,280-832,161), which means that respondents who smoke will have 105,125 times the risk of suffering from pulmonary tuberculosis compared to non-smoking respondents. From the results of 95% CI, it can be seen that the lower limit and upper limit values (CI values) not include 1.00, so the results of the study are statistically significant at the value of α = 0.001. Smoking behavior has an insignificant relationship with the incidence of pulmonary tuberculosis at Medan Sunggal Puskesmas in 2019. It is recommended that the primary care should provide support and sources of information about smoking habits, especially for patients who have been diagnosed with pulmonary TB.

Abstract

Renal involvement is the most common complication of systemic lupus erythematosus (SLE) and is also an important predictor of patient mortality. The incidence of flares is estimated at 65% each year in patients with lupus nephritis. Therapy in lupus nephritis with flare also uses high doses of steroid agents and strong immunosuppression agents. Mycophenolate mofetil (MMF) as an immunosuppression agent tends to favor for flare in lupus nephritis. We describe a patient who had a flare in lupus nephritis that resolved with high-dose steroid and MMF. The combination of immunosuppression agents and high-dose corticosteroids is effective for the control of active diseases. Cyclophosphamide as the steroid-sparing agent was discontinued because of adverse effects as well as hematuria. Partial remission was later achieved and maintained with MMF and corticosteroid after five months of protocol treatment. Thus, MMF while maintaining the steroid dose may induce remission for this case.

Abstract

Chronic kidney failure describes as the gradual loss of kidney function and it is irreversible, to replace the loss of kidney function a therapy for kidney function is needed, which is hemodialysis. The objective of this research is to know the levels of urea and creatinine serum for pre hemodialysis and post hemodialysis at Royal Prima General Hospital Medan. The methods that are being used for this research are descriptive with 54 patients as samples. The results of this research loss of urea and creatinine serum levels, with the average urea level for pre hemodialysis being 128.11 mg/dL, and the average urea level for post hemodialysis is 43.26 mg/dL with 1,8% of patient with low urea level, 50% with normal urea level, and 48.2% with high urea level. The average creatine level for pre hemodialysis is 11.56 mg/dL, and the average creatine level is 4.3 mg/dL, with all of the patients still having a high creatinine level. The conclusion is there is a drop in both urea and creatinine serum levels, but the urea level for half of the patients did not go down to normal, and for the creatinine level there are none of the patients’ creatinine levels that go down to normal.
