Abstract

Type 2 diabetes mellitus is a chronic metabolic disease with a rising prevalence; some patients require insulin therapy to reach optimal glycemic control. Long-acting insulin, particularly insulin glargine and insulin detemir, serves as basal therapy. This study describes the prescribing profile of long-acting insulin among BPJS outpatients with type 2 diabetes mellitus at Hermina Kemayoran Hospital during October-December 2025. The study used a descriptive design with a retrospective approach. The population comprised 1,868 outpatients with type 2 diabetes mellitus. Samples were selected through purposive sampling based on inclusion criteria, and all eligible records were then included using total sampling. A total of 204 patients met the eligibility criteria. Data were collected from medical records and prescription data, covering sex, age, HbA1c results, and the type of long-acting insulin prescribed, then analyzed univariately and presented as frequencies and percentages. Female patients outnumbered male patients throughout the study period. Long-acting insulin use was concentrated among patients aged 46-65 years and older than 65 years. Insulin glargine was the most frequently prescribed type, accounting for 91.46% in October, 90.74% in November, and 94.12% in December, while insulin detemir was prescribed less often. The prescribing profile of long-acting insulin was dominated by female patients, older age groups, and insulin glargine use. These findings can serve as baseline data for evaluating insulin utilization and planning pharmaceutical care services.

Abstract

Health logistics management ensures that medicines and health supplies are available at the right time, in the right quantity, and of the right quality. Previous research in Southeast Sulawesi still found stock-outs, overstocking, and expiry of program medicines; therefore, an evaluation of the logistics system is needed to inform policy improvement. This study aimed to evaluate the logistics management system at the Provincial Pharmacy Installation (IFP) Technical Implementation Unit of the Southeast Sulawesi Provincial Health Office across the planning, procurement, storage, distribution, disposal, and utilization stages. This qualitative study with a descriptive-evaluative design was conducted from April to June 2026. Nine informants were selected through purposive sampling, consisting of six key informants and three supporting informants. Data were collected through in-depth interviews, observation, and document review, analyzed using the interactive model of Miles, Huberman, and Saldaña, and validated through source triangulation and member checking. Planning was conducted hierarchically through e-Monev using a combined consumption–morbidity method. Procurement was carried out through e-purchasing/e-catalogue. Storage was the best-performing stage, supported by digital temperature and humidity monitoring and the application of FEFO/FIFO. Distribution relied mostly on the pull mechanism. Disposal complied with regulations through a licensed third party. At receiving facilities, compliance with LPLPO reporting has declined due to the implementation of the Sifarma application. The logistics system has met the principles of the right quality and right type; however, the right quantity and right timing have not yet been optimized. Strengthening upstream data validation, distribution fleets, redistribution of medicines approaching their expiration dates, and integration of logistics information systems is needed.
