Abstract

Introduction: Cardiorespiratory fitness (CRF) and physical activity (PA) in childhood can reduce the risk of cardiovascular diseases in adults. Further analysis related to the relationship between PA and CRFto prevent an inactive lifestyle is neededMethods: An analytic cross-sectional design in adolescents aged 13-15 years old, who had no cardiovascular and metabolic diseases, with light or medium physical activity, no deformity in thelower limbs and posture disorders. Cardiorespiratory fitness was assessed by the indirect method Bruce Protocol in the Faculty of Sport and Health Education’s library, The University of Pendidikan Indonesiaon September-October 2019. Physical activity was measure by the Physical Activity Questionnaire for Adolescent (PAQ-A).Results: Subjects consisted of 82 adolescents (41 boys, 41 girls), aged 13-15 years old with the Body mass index (BMI) was 18.5-29 kg/m2. The mean value of predicted VO2max (Bruce) for boys and girlswere 46.66 ± 8.765 and 40.06 ± 6.210 ml/kg2, while the value of predicted PAQ-A score for boys and girls were 2.54±0.756 and 2.16±0.416. The correlation between PAQ-A and VO2max in boys and girlswere r=0,190, p=0,234 and r=0.420; p=0.006.Conclusion: There was a moderate association between PAQ-A with VO2max in girls.Keywords: Cardiorespiratory fitness, Physical activity questionnaire, VO2max.

Abstract

Introduction: Indonesian Health Ministry had a strategy to make the primary care, including rhabilitation program by continuum of care. The purpose of this study was to determine factors related to the levelof knowledge, practice, and referral for continuum care cases that required rehabilitation services in Primary Health Care Doctors.Methods: The study design was cross-sectional. Participants were 23 Primary Health Care Doctors in Wonosobo District. Data collection was conducted in December 2017. The independent variablesconsist of gender, length of work, employment status, while the dependent variables were the level of knowledge, practice and referral cases of maternal, infants and toddlers, adolescents, adults and elderly.The measuring instrument was a questionnaire. Data were tested with chi-square, significance level <0.05.Results: Gender has correlation with the level of knowledge in maternal cases (PR = 0.121, 95% CI = 0.017-0.087, p = 0.026). Gender has correlation to the level of practice in maternal case (PR = 0.121,95% CI = 0.017-0.867, p = 0.026). Employment status has correlation to the referral rate of elderly cases(PR = 10.500, 95% CI = 1.015-108.577, p = 0.029).Conclusion: Gender has correlation with the level of knowledge and practice of maternal cases. Abstract

Introduction: Transverse myelitis is a neurological disease involving inflammation or lesions on the spinal cord with rapid and progressive onset of symptomsMotor training, sensory stimulation, andrehabilitation of autonomic functions are needed, to restore the function, reduce disability, and improve the quality of life.Case Presentation: A 34 years old male had weakness and loss of sensoric of lower limbs, accompanied by low back pain (LBP), chronic cough, and micturition dysfunction for 1 month ago, categorized byASIA impairment scale (AIS) D, level of injury at T6. The MRI of Thoracolumbal showed myelitis lession at 6th – 7th of thoracal, effusion at facet joint of L2-L5, and lumbal spondylosis. The rehabilitationprograms were active range of motion exercise with walking exercise (3 session/week, 30 minutes/session), Transcutaneous Electrical Nerve Stimulation (2-10 pps, 2 session/week, 20 minutes/session),sensory stimulation (2 session/week, 20 minute/session), and bladder training for 3 weeks. After 16 days, there were lower limb’s strength improvement by Manual Muscle Testing (MMT) were average four atlevel L2-S1, sensory improvement to T9-10, LBP was reduced, and normal micturition. After 3 weeks, lower limb strength improved to 55445/55445 at level L2-S1. Pat ient able to walk without assisted.Conclusion: Muscle strengthening, sensory simulation and bladder training, combined with pharmacological treatment, improved motor, sensory and autonomic function in Transverse MyelitispatientKeywords: Motoric training, sensory stimulation, bladder training, Transcutaneous electrical nerve stimulation, transverse myelitis

Abstract

Introduction: Early medical rehabilitation after Total Knee Replacement (TKR) was necessary to optimize healing process and reduce pain postoperative that can restrain patients’ activity.Case Presentation: A 66-years-old women had right TKR due to osteoarthritis genu dextra with chronic pain. Patient has muscle weakness on affected leg, and still not able to walk. Medical Rehabilitation(MR) program started one day after surgery for 4 weeks. After 4 weeks of MR program, patient had mild pain, and can walk for 100 m. Pain intensity by visual analogue scale (VAS) reduced from 7/10 to2/10, knee flexion increased from 70° to120°, right quadriceps muscle strength increased from 4 to 5 by manual muscle test (MMT), time to do physical performance by time up and go (TUG) was reduced from15 to 12.8 sc, activity daily living (ADL) by Barthel Index sco re increased from 4 to 8.Conclusion: Four weeks Rehabilitation Medicine program in geriatric patient with post Right TKR for 4 weeks, effective to improved muscle strength, balance, and ADL.Keywords: Medical rehabilitation, total knee replacement, geriatric, physical performance, quality of life

Abstract

Introduction: The frequency of natural disaster in Indonesia has increased recently, while there were still less capacity to overcome the respiratory problem as an impact of disaster. Subjects who havechronic obstructive pulmonary disease (COPD) need to have more attention in dealing with disaster setting.Methods: Literature reviews from the last ten years to find the data of the respiratory-related disease on disaster setting, which need pulmonary rehabilitation.Discussion: Subjects with COPD can get worse aftermath of natural disasters. The number of COPD exacerbation aftermath disaster was increased 1.5 fold. Pulmonary rehabilitation by trained patient forthe self-management of the symptoms was effective to reduce complication, increase the function and social integration.Conclusion: Pulmonary rehabilitation by optimizing community based training for self-management was effective as preparedness for COPD patients in natural disasters.Keywords: Chronic Obstructive Pulmonary Disease (COPD), Community based training, Natural Disaster, Pulmonary Rehabilitation

Three decades have passed by that Physical Medicine and Rehabilitation has beenexisting in Indonesia. In this time many improvements in professional practice andeducation have been achieved . Indonesian physiatrists have been developing skillsand knowledge regarding evidence-­based medicine which is a standard of clinicalpractices. Abstract

Objectives : To evaluate the effect of KinesioTaping on knee joint proprioception of patient withreconstructed anterior cruciate ligament.Methods: Randomized trial, cross-­over study design, was done in Rehabilitation outpatient clinic, Soetomo General Hospital, Surabaya, on 9 isolated anterior cruciate ligament (ACL) patients, which hasunderwent ACL reconstruction procedure. Each patient will become control for themselves. Subjects were treated with standardized Kinesio Taping technique for anterior cruciate ligament injury in additionto standard rehabilitation program for anterior cruciate ligament reconstruction. Response to treatment was evaluated with two evaluations: 1) Joint position sense of the knee for predetermined angle (30°,45° and 60°) 2) Threshold to detect of passive knee motion on 90° moving into extension. Measurements were taken two times within 3 days of interval, with and without Kinesio Taping application. Errorof angular displacement of active angle reproduction (joint position sense) and threshold to detect of passive motion (TTDPM) were measured in three condition: before Kinesio Taping application, afterKinesio Taping application and without Kinesio Taping application. Results : Nine patients (mean age 20.33 ± 4.062 y) who had anterior cruciate ligament reconstruction were included for this study. There were no other knee ligament injuries.There were significant different in active angle reproduction (joint position sense) at 30° and 45° extension between affected and unaffected knee (p = .030 and p= .15 respectively). Kinesio Taping application showed significant effect in active angle reproduction at 30° of knee extension (p=0.028). There were no significant different in TTDPM between all condition. Conclusions: Kinesio Taping application improves active angle reproduction (joint position sense) at 30° of knee extension. There is no difference in joint position sense at 45° and 60° with Kinesio Tapingapplication. Abstract

Objectives: To analyze the effect of ankle pumping exercise and other factors such as gender, age, duration of Diabetic Mellitus, nutritional status, history of smoking, hypercholesterolemia, ulcer degreeand HbA1c to Ankle Brachial Index (ABI) in type 2 Diabetes Mellitus patients with diabetic ulcer.Methods: The quasi experimental design study with consecutive sampling and one-­week interval of intervention was conducted in this study. Analysis of changes in pre-­post test ABI used paired t-­testwhile other factors were measured with multiple regression.Results: Ankle pumping effect to right foot after exercise as 0.048 while ABI 0,017 on left foot and ABI 0.038 after exercise on both feet. It was not a statistically significant increase on both feet (p>0.05). The relations of multifactors to ABI : Gender 0.47(p 0.829), age 51.32 (p 0.743), duration of DM 83.16(p 0.490), BMI 21.82 (p 0.452), history of smoking 0.47 (p 0.769), hypercholesterolemia 0.11 (p 0.195), degree of ulcer 3.74 (p 0.635), HbA1c 0.89 (p 0.798).Conclusions: There is statistically no effect of ankle pumping exercise to ABI. We also did not find an effect of gender, age, and duration of DM, nutritional status, history of smoking, history of hypercholesterolemia, ulcer grade and HbA1c to increase of ABI in diabetic subjects with foot ulcer after ankle pumping exercise. Nevertheless, we found a sufficiently noted increase of systolic blood pressure of Dorsal Pedis Artery and Posterior Tibia Artery after ankle pumping exercise in type 2 DM subjects although this not statistically significant.Keywords : Diabetes Mellitus, diabetic ulcer ankle pumping, ankle-­brachial index.

Abstract

Objectives: To compare the difference between sling suspension versus balance and proprioceptive exercise’s effect on pain and functional capacity of knee osteoarthritis patients before, 2 weeks after and 4 weeks after exercise.Methods: This longitudinal study with case series design was done on 34 subjects with knee osteoarthritis grade II and III in Outpatient Clinic of Medical Rehabilitation Installation in Dr. Kariadi General Hospital Semarang from November 2009 to February 2010. They were randomly assigned into ‘sling suspension’ and ‘balance and proprioceptive’ exercise group. Each group contains 17 subjects. Subjects conducted their exercise 3 times a week for four weeks. Assesment of VAS, WOMAC modification index, TUG test were done before, 2 weeks after and 4 weeks after exercise.Results: There were statistically significant changes in pain (measured by VAS category) and functional capacity (measured by WOMAC modification index subscale of pain, stiffness and functional activity, and TUG test) before, 2 weeks after and 4 weeks after exercise in both groups (p<0.05). However, there were no statistically significant difference in pain and functional capacity between groups (p>0.05).Conclusions:There were no significant diffrences between sling suspension and balance and proprioceptive exercise, although both exercise gave significant improvement in vas category, WOMAC modification index and walking time on TUG test.Keywords: knee osteoarthritis, sling suspension exercise, balance and proprioceptive exercise, VAS,

Abstract

Objectives: The objective of the study was to know the difference of maximum and ratio amplitudes of vastus medialis obliquus and vastus lateralis muscles before and after exercise with and without EMG biofeedback monitoring in knee osteoarthritis patients.Methods: Randomized clinical study design. This study was conducted in Department of Physical Medicine and Rehabilitation Airlangga University, Dr. Soetomo hospital Surabaya. Twenty eight patients with knee osteoarthritis were randomly placed into 2 groups, the first group received progressive resistive exercise with EMG biofeedback, while the other group received training with progressive resistive method only. The ratio and maximum amplitudes of vastus medialis obliquus and vastus lateralis muscles were analyzed before and after 8 weeks exercise.Results: The amplitudes of vastus medialis obliquus and vastus lateralis were improved significantly in group receiving progressive resistive training method with EMG biofeedback monitoring (p=.01 and p=.001, respectively). Comparison between two groups after 8 weeks of progressive resistive exercise showed significant improvements on amplitude of vastus medialis obliquus muscle (p=.014) and amplitude ratio of VMO/VL (p=.034) in EMG biofeedback monitoring group.Conclusions: Progressive resistive exercise with EMG biofeedback monitoring could improvesignificantly the amplitudes of vastus medialis obliquus and vastus lateralis muscles and amplitude ratio of VMO/VL in knee osteoarthritis patients.Keywords: knee osteoarthritis, progressive resistive exercise, EMG biofeedback monitoring.
