Abstract

The growing resistance of Escherichia coli to antibiotics has increased interest in plant-derived antibacterial agents. This study evaluated the antibacterial activity of combined ethanol extracts of Clitoria ternatea flowers and Jatropha gossypifolia leaves against E. coli. An in vitro experimental study with a post-test-only design was conducted. Extracts were prepared by maceration using 96% ethanol and subjected to phytochemical screening. Antibacterial activity was assessed using the disk diffusion method at concentrations of 20%, 40%, 60%, 80%, and 100%, with ciprofloxacin and dimethyl sulfoxide (DMSO) as controls. Each treatment was tested in four replicates. Phytochemical screening identified alkaloids, flavonoids, tannins, phenolic/hydroquinone compounds, and steroids. The largest inhibition zone was observed at 20% (12.25mm), followed by 40% (10.25 mm), 60% (8.00 mm), and 80% (6.25mm), while no inhibition occurred at 100%. Significant differences were found among groups (p < 0.001). These findings indicate measurable but limited antibacterial activity against E. coli.

Abstract

This study analyzes the simultaneous effect of operational cost control and BPJS claims optimization on the perceived financial performance of RSAL Ramelan Surabaya in the Universal Health Coverage (UHC) era. Facing financial pressures from high operational costs and claim delays, the hospital requires robust financial strategies. Using a quantitative explanatory approach, data were collected from 102 managerial staff via Likert-scale questionnaires and secondary financial reports. Data were analyzed using SPSS version 31, employing descriptive statistics, classical assumption tests, and multiple linear regression. Results indicate that operational cost control and BPJS claim optimization jointly and significantly predict perceived financial performance. The regressionmodel demonstrates strong explanatory power (Adjusted R2 = 0.927). Due to substantial multicollinearity between the independent variables (V IF = 14.731), findings should be interpreted as evidence of a synergistic association rather than independent causal effects. The study highlights the importance of integrating internal cost-efficiency strategies with BPJS revenue-cycle management to support hospital financial sustainability under Universal Health Coverage. Because the dependent variable reflects managerial perceptions rather than objective financial ratios, future studies should incorporate audited financial indicators and advanced statistical techniques such as Structural Equation Modeling (SEM) or Ridge Regression.

Abstract

Work accidents among fishermen remain a critical occupational safety challenge in Indonesia, driven by inadequate implementation of health and safety culture in maritime environments. This research applies the IDEACM (Identification, Design, Evaluate, Adaptation and Control of Macroergonomics) hazard assessment sheet, a structured instrument grounded in macroergonomic principles, to systematically evaluate occupational hazards among small-scale fishermen in Batam. The IDEACM framework was inspired by occupational safety practices depicted in the film ’Avatar: The Way of Water,’ analyzed through Roland Barthes’ semiotics and Fogg Behavior Model. Data were collected through field observations, a 20-item questionnaire (n=10), Focus Group Discussions (FGD), and semiotic film analysis, validated via SPSS (Cronbach Alpha = 0.890). IDEACM assessment quantified hazard scores across five macroergonomic elements (Human, Organization, Technology, Environment, Employment), yielding 18 sub-criteria with priority rankings. The top four hazards were: Skill deficiency in Human element (Score: 4.448, Rank 1); Machine hazards in Technology (Score: 1.00, Rank 2); Equipment risks (Score: 0.79, Rank 3); and absence of K3 Policy in Organization (Score: 0.73, Rank 4). Based on IDEACM findings, three hierarchy controls were designed: Engineering Controls (engine noise enclosures, distress siren systems); Administrative Controls (SOPs, K3 policies, structured shift schedules); and Personal Protective Equipment (PPE). Elimination and substitution were found infeasible under current conditions. The IDEACM framework proved effective as a replicable, structured tool for maritime occupational safety assessment.

Abstract

Humoral immunity induced by the CoronaVac vaccine may decline overtime. This study assessed changes in anti-spike receptor-binding domain (S-RBD) immunoglobulin G (IgG) antibody levels at 1 month and 5 months after completion of the two-dose CoronaVac regimen. A prospective observational cohort study was conducted among 54 participants, with 28 completing paired antibody measurements. Serum anti-S-RBD IgG concentrations were measured using a chemiluminescence microparticle immunoassay (CMIA), and differences between time points were analyzed using the Wilcoxon signed-rank test. A significant decline in antibody levels was observed, with the median concentration decreasing from 521.1 AU/mL (range: 71.2–2109.4) at 1 month to 57.8 AU/mL (range: 49.0–512.4) at 5 months, representing an approximately 89% reduction (p < 0.001). The Hodges Lehmann estimated median difference was -426.125 AU/mL (95% CI: -633.550 to -260.500). These findings indicate waning humoral immunity within five months after the second CoronaVac dose and support consideration of booster vaccination strategies.

Abstract

Mental health remains a growing public health challenge, while psychoeducation services at the primary care level are not yet optimally implemented. This study analyzed factors associated with the implementation of mental health psychoeducation services among healthcare workers in Public Health Centers in Rembang Regency, Indonesia, using a multilevel analytical approach. A cross-sectional study was conducted among 233 healthcare workers selected from 359 eligible participants using stratified random and total sampling. Data were collected using validated questionnaires and analyzed using chi-square tests, logistic regression, and multilevel mixed-effects analysis. The results showed that 60.1% of psychoeducation services were not achieved. In the final multilevel model, profession (OR = 4.464; 95% CI : 1.882–10.584), knowledge (OR = 3.367; 95% CI : 1.465–7.738), and attitude (OR = 3.368; 95% CI : 1.458–7.782) were significantly associated with service implementation, while leadership was significant at the organizational level (OR = 4.418; 95% CI : 1.754–11.126). The model showed meaningful clustering across Public Health Centers (ICC = 22.28%). In conclusion, the implementation of mental health psychoeducation services is influenced by both individual and organizational factors, with leadership representing the most important organizational determinant. Strengthening leadership and improving workforce capacity may enhance these services in primary healthcare.

Abstract

Venous thromboembolism (VTE) is a major preventable complication in critically ill patients. Although low-molecular-weight heparin (LMWH) and unfractionated heparin (UFH) are widely used for thromboprophylaxis, comparative data from Indonesian ICUs remain limited. This exploratory retrospective cohort study included 49 ICU patients with Wells score ? 2 who received LMWH or UFH at Haji Adam Malik General Hospital, Indonesia, in 2021. The primary outcome was ICU length of stay (LOS). Because LOS was non-normally distributed, groups were compared using the Mann-Whitney U test, with effect size estimated by the Hodges-Lehmann (HL) median difference. Of 49 patients, 41 received UFH and 8 received LMWH. Median ICU LOS was 5 days (IQR3–11) with UFH and 4 days (IQR 2–7) with LMWH. The HL median difference was-1.0 day (95% CI-5.0 to 3.0; p = 0.288), indicating no statistically significant difference. Wide confidence intervals reflected substantial uncertainty due to the small sample size and severe group imbalance. As LOS included patients who died during ICU admission, early mortality may have shortened LOS irrespective of treatment. No significant difference in ICU LOS was detected between LMWH and UFH groups. Findings are exploratory and hypothesis-generating only; larger prospective studies with severity adjustment are needed.

Abstract

Dysmenorrhea is one of the most common gynecological complaints among adolescent girls and may negatively affect daily activities, school attendance, and quality of life. This study aimed to examine the as sociation between exercise habits and the occurrence of dysmenorrhea among adolescent girls in Banda Aceh, Indonesia. An analytical cross-sectional study was conducted among 96 female students aged 15–17 years from a public senior high school in Banda Aceh. Participants were selected using purposive sampling. Data were collected through structured questionnaires assessing exercise habits, including regularity, frequency, duration, and type of exercise, as well as dysmenorrhea status. Associations were analyzed using the Chi-square test, and crude odds ratios (ORs) with 95% confidence intervals (CIs) were calculated. The results showed that dysmenorrhea was highly prevalent among respondents. No significant associations were found between dysmenorrhea and exercise regularity (p = 0.289), exercise frequency (p = 0.203), or type of exercise (p = 0.398). However, exercise duration was significantly associated with dysmenorrhea (p = 0.026). Adolescents who engaged in physical activity for at least 30 minutes per session had lower odds of experiencing dysmenorrhea compared with those exercising for less than 30 minutes (OR = 0.31; 95% CI: 0.11–0.90). These findings suggest that longer exercise duration may be associated with a lower occurrence of dysmenorrhea among adolescents. Given the cross-sectional nature of the study, the observed relationships should be interpreted as associations rather than causal effects.

Abstract

Postpartum blues is a common emotional disturbance during the early postpartum period and may progress to postpartum depression if left unmanaged. SAFE education has emerged as a non-pharmacological approach to improve maternal emotional preparedness. This study systematically evaluated the effectiveness of SAFE education in preventing postpartum blues through a Systematic Literature Review (SLR). Literature searches were conducted in PubMed, Scopus, and Google Scholar for studies published between 2021 and 2025. Following PRISMA 2020 guidelines, 309 records were initially identified from three electronic databases. After the removal of 102 duplicate records, 207 unique articles underwent title and abstract screening. Of these, 112 full-text articles were assessed for eligibility, and 12 studies involving approximately 1,900 participants fulfilled all predefined inclusion criteria and were included in the final qualitative synthesis. The findings consistently demonstrated that SAFE education reduced postpartum blues symptoms, anxiety, stress, and mild depressive symptoms while improving maternal knowledge, self-efficacy, emotional preparedness, coping skills, and perceived social support. Due to substantial heterogeneity in study designs, interventions, and outcome measures, a narrative synthesis was performed instead of a meta-analysis. Overall, SAFE education appears to be a promising antenatal intervention for promoting maternal mental health and reducing the risk of postpartum blues.

Abstract

Outpatient waiting time is a critical service-quality indicator in Indonesian hospitals. At Hospital X, Sidoarjo, the Internal Medicine Clinic's average wait time reached 83 minutes and 58 seconds, approximately 40% above the 60-minute standard. This qualitative study applies in-depth interviews, fishbone analysis, and Lean Six Sigma (LSS) to identify root causes and formulate improvement recommendations. Findings identify systemic waste and defects, including complex B1–B6 clinical assessments, electronic medical record (EMR) constraints, high staff workloads risking burnout, and unreliable wait-time estimates despite an effective online registration system. In the short term, implementing e-Signatures to eliminate physical documentation workflows is identified as the most immediately actionable intervention. Ambient AI Scribe and AI Predictive Analytics are proposed as longer-term, infrastructure-contingent interventions requiring contextual validation before adoption. Reducing waiting times below 60 minutes through this combined strategy is operationally plausible but remains an unvalidated projection requiring further quantitative confirmation.

Abstract

Adolescent reproductive health remains a major public health concern, particularly in developing countries where access to accurate information is often limited. This study examined the association between smartphone utilization and reproductive health knowledge among female adolescents in Aceh Besar, Indonesia. A quantitative cross-sectional study was conducted in May 2025 involving 81 female students selected through proportional stratified random sampling. Data were collected using structured questionnaires and analyzed using binary logistic regression. Overall, 72.8% of respondents used smartphones to access reproductive health information, and 55.6% had good reproductive health knowledge. Smartphone use for reproductive health information was significantly associated with good knowledge (OR = 2.98; 95% CI: 1.33-6.67; p = 0.001). Duration of smartphone use (p = 0.004) and social media platform (p = 0.006) were also significantly associated with knowledge. Smartphones may serve as effective channels for improving reproductive health knowledge among female adolescents, although causal relationships cannot be established due to the cross-sectional design.
