Abstract

ABSTRAKDinas Perdagangan dan Perindustrian (Disperdagin) Kota Surabaya mempunyai tugas menyelenggarakan urusan pemerintahan dan pelayanan umum seperti pembuatan ijin perusahaan. Proses pembuatan ijin saat ini terlalu lama karena kurangnya informasi bagi perusahaan saat akan mendaftar. Pihak perusahaan harus membawa kembali dokumen formulir untuk mempersiapkan dokumen yang dibutuhkan dan kembali lagi untuk menyerahkan dokumen ke Disperdagin. Masalah lain yang timbul yaitu belum adanya sistem yang memberikan informasi untuk Disperdagin mengenai status perijinan yang dibuat.Berdasarkan permasalahan di atas, dibutuhkan sebuah sistem yang mampu mempermudah proses pembuatan ijin secara online bagi perusahaan. Perusahaan tidak perlu berulangkali datang ke kantor Disperdagin untuk melengkapi dokumen. Selain itu juga dibutuhkan sistem yang membantu pihak Disperdagin untuk memonitoring status ijin perusahaan. Sistem yang dibuat dikembangkan menggunakan metode Waterfall.Berdasarkan hasil uji coba yang telah dilakukan, sistem ini dapat memenuhi kebutuhan pada Dinas Perdagangan dan Perindustrian Kota Surabaya dalam proses pendaftaran ijin, persetujuan ijin, penerbitan ijin, pemantauan posisi dokumen, pembuatan laporan ijin usaha, dan ijin industri serta memberikan peringatan apabila ada ijin yang habis melalui sms gateway.Kata Kunci: Monitoring Perijinan, Perusahaan, Industri.

Abstract

Abstract:Pusat Penjaminan Mutu (PPM) is a unit that is specifically tasked with ensuring quality of service and teaching at Sekolah Tinggi Ilmu Kesehatan (STIKES) Hospital Foundation. Dr. Soetomo. PPM is currently having some problems in data management evaluation of the learning process. One such obstacle is because the data management evaluation of the learning process is still done manually. This causes the management process takes approximately two months, resulting in the evaluation of the learning process to be blocked.Based on these problems made design evaluation data processing applications web-based teaching and learning process that contains the process of making inquiries, the preparation of the questionnaire, questionnaires, and processing questionnaires. In the stage of preparation of this application using SDLC stages watterfall models.The results showed that the application of data processing evaluation learning process can help the STIKES. This is proved by the test results to Kaprodi, lecturers, and student evaluation data processing applications learning process has to meet the needs of each party, as evidenced by the results of experiments conducted to each party declared unfit for use.Keywords: Application, Evaluation, Questionnire

Abstract

CV. Harapan Jaya Tenda adalah sebuah perusahaan yang bergerak dibidang manufaktur. Perusahaan ini memproduksi berbagai macam jenis tenda sesuai dengan tingkat keperluannya. Proses produksi dilakukan berdasarkan pesanan dari pelanggan yang diterima oleh bagian marketing, untuk kemudian diteruskan ke bagian perencanaan. Setelah itu bagian perencanaan mulai membuat rencana kemajuan produksi, rencana kebutuhan bahan baku, rencana kebutuhan tenaga kerja dan rencana biaya lain-lain untuk nantinya digunakan sebagai pembanding dalam proses realisasi kemajuan produksi, realisasi kebutuhan bahan baku, realisasi kebutuhan tenaga kerja dan realisasi biaya lain-lain.Proses pemantauan yang masih dilakukan secara manual menimbulkan beberapa kendala dalam pengerjaan proyek, termasuk dalam pengendalian bahan baku, sehingga kurang dapat memberikan evaluasi yang baik untuk progress pengerjaan proyek dan kekurangan kebutuhan bahan baku dalam pengerjaan proyek. Ditinjau dari permasalahan diatas maka diperlukan suatu sistem yang dapat memberikan informasi dalam monitoring dan evaluasi, beserta pengendalian bahan baku yang digunakan dalam proses produksi, sehingga dapat dikelola secara efektif untuk dapat memberikan evaluasi dari hasil monitoring pengerjaan proyek berupa laporan yang dapat digunakan sebagai perbandingan antara rencana dengan realisasi.Berdasarkan uji coba yang dilakukan dapat disimpulkan bahwa sistem dapat memenuhi kebutuhan antara lain menampilkan laporan perencanaan dan realisasi bahan baku, tenaga kerja, kemajuan produksi, biaya lain-lain, laporan safety stock yang didapatkan dari perhitungan metode safety stock berdasarkan service level, dimana data yang digunakan dalam perhitungan diperoleh dari realisasi kebutuhan bahan baku per periode bulan sebelumnya

Abstract

Ashara Husada pharmacy is in cooperation with YAKES health foundation PT .Telkom Indonesia. This health foundation formed to serve needs of health for the employees, retired employees, and their families those residing in Sidoarjo and the surrounding areas. Clinics and pharmacies Ashara Husada not only serve the employees, retired employees, and families, but also conducts services for the public. The difficulty lies in the recording of the transactions are still manual , a period of of purchase the medicines are not periodically and stocks information of medicines being inaccurate. After were analysed the existing problems then it leads to Design of pharmacy. Design help to recording the transactions, handle a shortage and excess stock of medicines, as well as displaying dashboard of the medicines, medicines sales, stock of medicines, and rejection of the sale of medicines. From the trial of the application of the pharmacy can help pharmacists’s work there are 1 ) records the transactions of medicines, 2 ) handle deficiency and excess stock medicines with rop  calculation 3 ) displaying dashboard from sales of a medicines, the purchase of medicines, stock of medicines and rejection medicine’s sales.

Abstract

Dogs are common animal that found as pets in Indonesia. Indonesia as a tropical country has a high humidity, where fungi and parasites easily to multiply on animal skin. The fungi and parasites are one of the causes of dog skin diseases. Worsely, dog skin diseases can be transmitted to other animals as well as humans. When dog skin diseases are not treated properly they can excruciate the dog condition, because it can lead to the emergence of other diseases simultaneously, and it can even bring the dog to death.The limitation informative tools in identifying dog skin diseases can cause retardationin handling the disease. In this work, we provide an expert system to identify the dog skin diseases and give suggestion as to how to handle it. The skin diseases in dogs have similar symptoms between each other. The degree of belief of the symptoms determines the disease identification.Hence, in order to accommodate the identification process, an expert system is built by using the certainty factor method.Based on the result,it is shown that the expert system is able to identify dog skin disease with 91.67% accuracy derived from the twelve tests. The tests were done by the appointed expert in this area.

Abstract

The purpose of LPB-YDBA Astra Waru is to help SMEs to be "independent”, develop, and empower the local economy through entrepreneurial efforts, and aims to assist the implementation of the 5 pillars formed by PT Astra International, Tbk namely UMKM/IGA (International Grade Astra) program.Corporate performance appraisal basically is a crucial factor in order to develop an effective and efficient organization, its purpose is to provide an assessment as the basis for a reward, making a decision, and is used as a basis for evaluating achievement. At this performance appraisal, every SMEs is given the opportunity to get a reward from PT Astra International, Tbk and is facilitated the access to get capital borrowing at PT Astra International, Tbk companies such as Bank Permata and United Tractors (UT). SMEs will be assessed by a questionnaire that based on five pillars and collect some of the required financial statements to calculate the financial ratios.There are obstacles in the performance assessment carried out, because in the performance appraisal there is a process for assessing the financial ratios calculation of financial terms, while there is no system that helps in calculating the financial value. As a result, the grade-level in the performance appraisal report does not appear to be true and results in the performance appraisal is invalid.From the implementation of information systems implemented in LPB-YBDA Astra, it can be concluded that the system created can accelerate the process of performance appraisal of SMEs. Information systems of SMEs performance appraisal can be used to help the LPB-YDBA Astra in evaluating the performance appraisal of SMEs that took place on LPB-YDBA Astra.

Abstract

In an effort to repair the company PT. PAL changed the concept of performance appraisal of each division were previously more focused on the financial report into the performance appraisal with the concept of balanced scorecard, this is in line with the direction of the Ministry of SOEs as a state institution builder to each SOE. Merchant vessel division needs help to be able to ensure that the process goes according to plan and know how condition the object in the design, implementation and results. Based on the above issues authors propose Application Performance Monitoring and Evaluation Merchant Vessel Division. The author uses a balanced scorecard method in accordance with the concept of the PT. PAL. To process the existing data using the method of direct user rating. The data that has been processed will be the source of the data in the reports to be made. With the Application Performance Monitoring and Evaluation Merchant Vessel Division, the management can be easier to monitor and evaluate the performance of the Merchant Vessel Division. Thus the management can identify problems on the merchant vessel division in accordance with the concept of the balanced scorecard method

Abstract

Medical records are factual documents which related to patient condition, past and present treatment which is written by a doctor who treat the patient. (Huffman, 1999).RSAM have a diffcult situation to search, provide, and study patient medical records at Outpatient Unit because all of them are still in document format in archive room which could make it take a long time to deliver it to the admin room.To help solve this difficulty, therefore the hospital needs a medical records application at outake patient unit. This application is made to help doctors to decide what action to the patient so it will produce accurate and easy to search medical records. With the help of this application in Outpatient Unit, so Outpatient Unit at RSAM will become better and faster to give a service to the patient according to hospital service standard.With design and development of medical records application at outpatient unit in RSAM will produce a report and also medical record information about patient visitation, medical record registration, and income. This system is used by doctor to consider what action should be taken and trail access of patient medical records.

Abstract

PT. PEHAESTEX are supplying a variety of products Moslem. Competition in this area is increasing every year and make the sales volume is not stable at PT. PEHAESTEX, especially in products gloves. Problems that occur during this inventory every kind of goods are too little compared to the customer demand that can not meet the demand of customers pelanggan.akibatnya be waiting and customers can switch to another distributor. However, the other type of gloves are also frequent excess stock that made goods become piled up and need a place of their own warehouse for storage. , In the procurement of goods, manager of PT. PEHAESTEX only see the data based on the previous month's demand for goods only. The aim of this thesis was to produce a determination of inventory by forecasting applications meeting the demand of goods with exponential smoothing method winter in PT. PEHAESTEX to help make decisions for supplies after knowing the approximate demand for goods in the future.

Abstract

According to a decree number 290/KPT-03B/VII/2014 about implementation of Hybrid Learning Model clause six, lecturer Stikom required to implement Brillian as a learning medium. This is not the first time Stikom Surabaya implementing an e-learning application. From many kinds of e-learning that has been applied previously, none of application that can be accepted as a medium of teaching and learning, whereas already a lot of material and non-material sacrificed to implement a variety of e-learning. Try to overcome these problems by conducting a comprehensive evaluation and adequate empirical studies in accordance with theories. One theory could be used to evaluate the acceptance of information technology is the Unified Theory of Acceptance and Use of Technology (UTAUT) which was developed by Venkatesh in 2003. The result of this study case found the opposite result with Venkatesh theory. The four factors (Performance Expectancy, Effort Expectancy, Social Influence, and Facilitating Condition) does not influence significantly to increase Behavioral Intention or Use Behavior. Only Behavioral Intention proven to have significant influence on the Use Behavior. Besides moderating variables (age, gender, experience and volunteerism) was not found to have a significant influence in intent and actual use of Brilliant.
