Abstract

Abstract

Abstract

Hand injuries are among the most common musculoskeletal traumas and may lead to functional impairment, long-term disability, or limb loss if not properly managed. Among these, crush injury is a severe type characterized by damage to bones, soft tissues, and neuromuscular structures, often resulting from occupational accidents. This case report describes the diagnosis, management, and outcome of a 64-year-old male who presented to the Emergency Department of RSUD dr. Slamet Garut with severe pain and an open wound on his left hand after being struck by a concrete block. Data were obtained through anamnesis, physical examination, laboratory tests, radiography, and medical record review, then analyzed descriptively with reference to relevant literature. Ethical approval and informed consent were obtained. Examination showed an open wound, deformity, bleeding, and limited motion in the fourth and fifth digits of the left hand. Radiography revealed fractures of the distal phalanges. Due to extensive tissue damage and signs of ischemia, amputation of the distal phalanx of both digits was performed. The patient had a history of hypertension but no diabetes mellitus or other chronic diseases. He was diagnosed with a crush injury of the distal phalanges of the left hand caused by a work-related accident. Amputation was selected based on local tissue viability and long-term functional prognosis. Prompt diagnosis, appropriate management, and adherence to clinical guidelines are crucial to prevent complications and promote optimal recovery in crush injury cases.

Abstract

A 20 years old female presented to the ER with fever and multiple joint pain, especially in hands and feet. Patient also have dry cough without blood, and weight loss in the last few months. There were no other symptoms. Patient had a medical history of hypothyroidism with episodic thyroid enlargement since 2022. Medications were irregularly taken, and there was a relapse in 2023. After a few days of treatment. After a few days of treatment, new symptomps such as hemoptysis, dyspnea, high-grade fever and chills emerged. This was followed by vomiting, bloating, and difficulty in defecation. Laboratory examination revealed hypothyroidism, anemia, and thrombocytopenia with decreased kidney function. Complete urine test showed blood (hematuria) and protein (proteinuria) in urine. Chest X Ray indicated cardiomegaly and pneumonia with mild effusion. Subsequently, we suspected Systemic Lupus Erythematosus (SLE), and Rapid Progressive Glomerulonephritis, with confirmed diagnoses of hypothyroidism and pneumonia. ANA IF (for SLE), abdominal ultrasonography, and viral examination were performed to confirm our diagnosis. Several days later, ANA IF results returned significantly positive, confirming the diagnosis of SLE based on the ACR/EULAR 2019 criteria. This report underscores the importance of history-taking, physical examination and laboratory examination in suspected cases of systemic lupus erythematosus. Early recognition and appropriate management are crucial in achieving favorable outcomes for the patients.

Abstract

Pesantren as traditional Islamic educational institutions face challenges in transparent and accountable administrative and financial governance in the digital era. The Al Ajwa Al Islamy Islamic Boarding School with 420 students and 17 managers still relies on manual recording that is prone to errors, physical archives that are prone to loss, and digital gaps between generations of managers. This study aims to analyze the effectiveness of the implementation of digitization of pesantren governance through the GOPONPES-DIGITAL program using Google Workspace and Google Sheets-based Accounting Information System. The implementation method uses a participatory approach with stages of socialization, Google Workspace training, SIA implementation, cross-generational mentoring, and pretest-posttest evaluation. Participants involved pesantren managers, teachers, senior students, and UNIKOM students as companions. The training includes Google Docs, Sheets, Drive, Gmail, and Calendar, followed by the implementation of financial recording templates and digitization of administrative records. The results of the implementation show a significant increase in managers' digital literacy from basic to intermediate levels based on pretest-posttest evaluation. Financial records become more efficient, transparent, and real-time with minimal error rates. Structured digital filing systems reduce the risk of losing important documents. Cross-generational mentoring successfully bridges the digital divide, with the formation of an internal digital team that is sustainable. The monthly reporting process can be generated automatically with high accuracy. The GOPONPES-DIGITAL program has proven to be effective in improving pesantren governance through adaptive digital transformation without abandoning traditional values. This model has the potential to be replicated to other Islamic boarding schools as a solution to digitize educational institutions based on religious communities.

Abstract

This study aims to explore the government’s strategies in addressing stunting in North Aceh Regency and examine how policy implementation is understood and practiced within the region’s socio-cultural context. Stunting remains a critical public health issue in North Aceh, with prevalence rates exceeding the national average, highlighting the need for more contextualized and participatory approaches. A qualitative case study design was employed, with data collected through semi-structured interviews involving 25 key informants, participant observation in community nutrition and health programs, and document analysis of local policy reports and official records. Informants were selected using purposive and snowball sampling techniques, including health cadres, public health officers, local government officials, community leaders, and mothers of under-five children. Thematic analysis based on Miles and Huberman’s interactive model was used to analyze the data. Four major themes emerged: (1) cultural perceptions linking rice as the primary indicator of nutritional adequacy, (2) limited human resource capacity and sectoral coordination, (3) a gap between nutrition education and actual consumption practices, and (4) weak monitoring systems and case follow-up mechanisms. These findings underscore the importance of aligning policy implementation with local values and social structures. This study contributes to the development of policy implementation and social-ecological theories in public health contexts. Practical implications include the need for culturally adaptive nutrition programs and stronger community-based monitoring systems. Future research is recommended to adopt participatory approaches for developing more responsive interventions tailored to local community needs.
