Abstract

Community-Based Midwifery Care (CMBC) is a crucial approach to improving the accessibility and quality of maternal and neonatal care, particularly in rural and remote areas. Integrating CMBC into primary health care (PHC) systems offers significant potential to strengthen overall health services, reduce disparities in access, and ultimately reduce maternal mortality (MMR) and neonatal mortality (NMR). This literature review examines the definition and principles of CMBC, successful integration models, and the benefits and challenges of integrating CMBC into PHC. The aim is to provide insights for policymakers, health program managers, and practitioners on how to strengthen the role of community midwives and maximize their contribution to achieving global health goals.

Abstract

Midwives play a central role in the health system, particularly in maternal and child health services. However, high work demands, stressful work environments, and significant emotional burdens often make midwives vulnerable to occupational stress. This literature review examines the factors contributing to occupational stress in midwives, effective stress management strategies, and the impact of occupational stress on the quality of midwifery care and midwifery health. A thorough understanding of these issues is essential for developing interventions that support midwife well-being and ensure the provision of quality midwifery care.

Abstract

Accurate, complete, and real-time maternal and neonatal data is an essential foundation for policy planning, clinical decision-making, program monitoring, and evaluation of maternal and neonatal health outcomes. Health Information Systems (HIS) have significant potential to transform the way these data are managed, from error-prone manual recording to efficient, integrated systems. This literature review aims to explore strategies for optimizing HIS in maternal and neonatal data management, identifying existing challenges, potential benefits, and best approaches for implementation and maintenance. The review indicates that integrated and user-friendly HIS can improve data quality, reduce duplication, accelerate reporting, support evidence-based decision-making, and facilitate epidemiological surveillance. Key challenges include system interoperability, the digital divide, user resistance, and the need for ongoing training. Optimizing HIS requires political commitment, infrastructure investment, and human resource capacity development.

Abstract

High-risk pregnancy requires intensive monitoring, multiple interventions, and complex care coordination to ensure optimal outcomes for both mother and fetus. In this context, case management has emerged as a crucial strategy for integrating health, social, and psychological services. This literature review examines the definition and principles of case management, the specific role of case managers in the care of high-risk pregnant women, and the benefits and challenges of its implementation. The goal is to highlight how case management can improve care coordination, service accessibility, and patient satisfaction, ultimately contributing to reduced maternal and perinatal morbidity and mortality.

Abstract

In primary healthcare facilities, quality management of ANC services is crucial to ensure that every pregnant woman receives comprehensive, standardized care. This literature review aims to identify, analyze, and synthesize various quality management models that have been implemented or proposed for ANC services in primary healthcare facilities. The review indicates that existing models often incorporate a systems approach, continuous improvement cycles, and multidisciplinary participation. Key challenges include resource availability, health worker capacity, and adherence to standards. An ideal model should consider the local context, be patient-centered, and supported by a robust monitoring and evaluation system.

Abstract

Midwives are the backbone of maternal and child health services, yet they often work in challenging conditions with limited support. Clinical supervision is a globally recognized key intervention for supporting and developing frontline health workers. This literature review aims to explore the role and effectiveness of clinical supervision in improving midwives' technical and non-technical competencies and work motivation. The review results indicate that structured, supportive, and ongoing clinical supervision is significantly positively correlated with improvements in clinical skills, adherence to practice standards, decision-making ability, self-confidence, job satisfaction, and intrinsic motivation. Effective supervision also helps reduce burnout and professional isolation, particularly in remote areas. Implementation challenges include lack of supervisor capacity, time constraints, and resistance to a culture of blame-based supervision. Optimizing clinical supervision requires investment in supervisor training, the development of a clear framework, and the creation of an organizational culture that supports learning and development.

Abstract

The adoption of new technologies in obstetric care, such as electronic medical record (EMR) systems, advanced diagnostic tools, or tele-obstetric platforms, has significant potential to improve the efficiency, accuracy, and accessibility of care. However, the implementation of new technologies is often accompanied by resistance, organizational culture challenges, and operational complexity, necessitating effective change management strategies. This literature review examines relevant change management models and principles, identifies key strategies to facilitate the adoption of new technologies in obstetric settings, and analyzes enabling and inhibiting factors. The goal is to provide insights for health facility managers, midwives, and policymakers in planning and implementing successful technology initiatives, which will ultimately improve the quality and innovation of obstetric care

Abstract

The midwifery care team, comprised of various professionals such as midwives, doctors, nurses, and other support staff, is a crucial unit in providing maternal and child health care. However, diverse backgrounds, experiences, perceptions, and high work pressure often trigger conflict. Poorly managed conflict can negatively impact team communication, staff job satisfaction, and, most importantly, patient quality and safety. This literature review aims to explore common sources of conflict within midwifery care teams, analyze their impact, and identify effective conflict management strategies. The review indicates that intrapersonal, interpersonal, and interprofessional conflict often arise from differences in role perceptions, time pressure, workload, and communication styles. The most effective conflict resolution strategies involve open communication, mediation, collaboration, and the development of negotiation skills. Successful implementation requires strong leadership to create a supportive team culture that promotes dialogue and is oriented toward problem-solving.

Abstract

The birth process, although a natural event, often involves a series of complex workflows that can potentially generate waste and risks to the safety of mothers and babies. Lean Management, a management philosophy focused on eliminating waste and creating value, has proven effective across various industries, including healthcare. This literature review aims to explore the application of Lean Management in the birth process, analyzing its impact on operational efficiency and patient safety. The review results indicate that the application of Lean principles such as value identification, value stream mapping, continuous flow, pull systems, and continuous improvement (kaizen) can reduce waiting times, medical errors, costs, and increase patient and staff satisfaction. Implementation challenges include resistance to change, training needs, and leadership commitment. The integration of Lean Management can optimize the birth process, making it more efficient, safe, and patient-centered.

Abstract

Obstetric emergencies are a leading cause of maternal and neonatal morbidity and mortality worldwide. Prompt, appropriate, and standardized care is crucial to saving lives. Standard Operating Procedures (SOPs) are documents that guide the steps in a process, ensuring consistency, efficiency, and safety. This literature review aims to explore the importance, principles, and challenges in developing and implementing SOPs for obstetric emergency care. The review results indicate that clear, concise, and evidence-based SOPs significantly improve response speed, adherence to clinical protocols, team coordination, and ultimately, clinical outcomes for mothers and babies. Effective SOP implementation can reduce variation in practice, minimize medical errors, and empower healthcare workers in crisis situations. Challenges include the need for socialization and training, ongoing updates, and resistance to standardization. Developing effective SOPs requires a participatory approach, leadership commitment, and integration with broader quality management systems.
